Wrinkly Penis Head (Burning at the bottom of the head)

Hello, I have been having this on and off burning and stinging sensation under the tip of my penis, it's very mild. I am HSV-1 positive but not genitally, at least according to the doctors. Last sexual encounter was 4 months ago, nothing else. Not sure what is wrong, nothing ever shows up there and it's been happening for a while so I do not suspect STI. It's also really wrinkly sometimes, but looks sort of fine when erect, aka there is still some wrinkling but not as bad. I attached a picture. I also had this eczema/jock itch combo rash on my groin and scrotum, doc prescribe a betaderm/clotrimazole mix, it worked ok for my groin but scrotum is still red sometimes (no pain there). So I am not sure if this penis head "rash" is related or not.
