Zoloft not working as well after 5 years on it

I started off using Zoloft 50mg in the middle of 2015 because of depression and slight anxiety (Stemming from the depression). It worked really well and quickly, but the dosage was upped gradually over the past few years now to 125mg that I've been on for about 2 years.

I've had relapses, but nothing longer than a day or two. That's until this month, when I've had the longest relapse and also have similar feelings that I had before i started Zoloft (depression in morning and afternoon, a bit better later at night). I also had some new symptoms like sleepiness, ears feeling full, and head feeling stuffed. It's like a fog on my head that's telling me not to do anything, even though I know I should be doing stuff. I have an appointment booked in a few days with the doctor I've been seeing for a few years, and also saw a mental health counsellor for advice. I'll also ask the doctor for a blood test just to rule out any other causes that I saw online that could cause this.

Anyone else experience Zoloft (Or any other antidepressant) just not working as well anymore? What will the doctor likely recommend in this case? Having an antidepressant work well for 5 years and then just stop is pretty difficult.
