10 Weeks post HoLEPmat NHS Newcastle UK

This will probably be my last post here, for a while, after a very successful operation. I've posted before at 2, 4 and 8 weeks about my progress and thought I would finish off my last report now that I am relieved of all my pre op symptons. I did say at the 4 to 8 weeks I had urgency which meant I had only about 10 secs to get to a loo and I was caught out a few times which having  had a ruptured achilles at about 5 weeks didnt help my speed at getting to a toilet. I've read about the urgency so although a bit peturbing I knew it was tempory so soldiered on and during the last two weeks it has completely gone. I can sit here watching tv with an urge and delay it for another 15 minutes, no problem.

With this op,,  I had no pain at any point, no clots but a bit pink for 10 days, no pills anymore, can pee 300 mlitres in 10 seconds compared to 100 mlitre in a min before.

No retention at night  and so can sleep through til 7am. Much to my amazement I am ejeculating, not much as I suppose 70% of the prostate is missing now, BUT, the orgasm I'm having is like the real thing which I've not had for some 13 years because of the pills drying me up.

I knew I had a big chance of BE so guess I am lucky and more importantly had a good consultant with modern kit.

Not having to get up all night is the thing I like most about this op and not having to take the two pills is good too. I have been reading that Dutasteride may be causing non alcoholic fatty liver. having said that the pills buy one time whilst deciding what to do , but I wished I had had this op sooner.

In the scheme of things it's ealry doors but am as confident as one can be that the results will be long lasting.

You had a good team there. When do you go back for a review ?

    Hi Breeze

Well, it's 7 weeks post HoLEP for me (St Albans, UK). Like you, no pain at any point and no clots. Trace of blood for about 3 weeks. Some discomfort in perineum when sitting down for about 4 weeks.

The only remaining effect is leakage, which was disturbing for about 5 weeks but is diminishing now. A close neighbour had the same op. and surgeon 2 years ago and said that leakage lasted 2 months - I can live with that.

When I see the surgeon in August for follow-up, I will be stating my disappointment regarding lack of advice and information received. There was no advice concerning pelvic floor exercise and no warning to expect leakage. Luckily I got the information from you and others on this site.

Best of luck breeze and to Phil25323, who I understand is having HoLEP later this week in Miami.

Hi Ken and all other posters,

Thanks for all your replies and well wishes and good luck to all.

I think doctors do have some choice on whether you lose your seminal fluids pipe work  depending on individual situations. Its wriiten that with HoLEP some 75% will lose get BE and I was accepting of this and would have still been over the moon with the outcome.

Phil good luck with your holep and i'm not sure if cipro affected my  achille tendon but I believe it would have snapped anyway. It was the rush of adrelinine when I saw my car free wheeling backwards,,, I forgot the handbrake. I tried to sprint off to stop it and it went. Needless to say the car, which was on a levelish road, came to a harmless stop by itself, A tip for all older men is not to push cars as that is one of the main causes of snapped achilles.

Geezer . I had light dribbling for about 2 weeks then ok, David in Uk is 7 weeks and is dribbling too but doesn't mention any of the of urgency that I had.

David Thanks, glad I helped you but that's why we all post on here. I'm sure the pelvic floor exercise helped me with urgency because if youve only got 10 secs to get to a urinal then you work those muscles like hell. Eventually your body repairing stops the urgency but the strong pelvic floor helps control the  urgency  sympton.

Derek... Hi, Keep up the good work. ps Newcastle are going to ring me at the 3 months stage to ask

how I'm getting on, so in a couple of weeks. I can't thank my surgeon and staff enough.

Kenneth,  Thanks for all your many helpful posts but I think you are coming across as too negative in regards HoLep at least. A lot of the newer techniques have not stood the test off time and I wonder if many will fall by the roadside. HoleP is an op that is proven to give good results without pain and too much blood and no clots. For older people, at least, it is a god send. Yes, I admire that consultants are trying to conquer the scurge of BPH but older people need something that is going to work for a long time because as you well know, after several years of having bad BPH you are begging for relief, and a relief that will be long lasting, and that at the moment is Holep or the other previous lazer techniques.

Breeze  This is true.  Laser procedure are much better.  Less bleeding and healing is faster.  But a man should not pick a procedure going into a surgery that it is going to be a fix all and the last procedure you are going to have. We all hope that's.  We all know that and we have men on here that have had many procedure.  There is a doctor in the UK Dr. Gordon Muir that wrote a article on retro ejaculation.  He state no matter what procedure you pick even a Turp if the doctor takes his time and stays away from the ejaculatory ducts, seminal canal and the Vas Deferens the patient does have a better chance of not losing his ejaculation.  But the doctor are in a rush and moves on to the next patient.  I hope Phils surgery go well on Wednesday and it is all he want it to be.  Only time will tell.  I hope god guides the doctor hands and does well by him.  Remember this is only my opinion God bless you all  Ken

      Hi Kenneth,

Thanks for your reply. I do agree there are doctors who do rush and maybe dont have up to date equipment. Here in the Uk we do have limited operating theatres and because consultants can be both in private and NHS care then some doctors may tend to rush their nhs patients so they can spend more time with their paying customers. Whether that is right or wrong I dont know but it does happen. Fortunately NHS patients can be referred to private hospitals for which I am grateful .

I know you have been a big contributor on here and wish you all well too.

I just saw my surgeon for one month follow-up.  Going in, I had 200 gram prostate; now it is 20 gram.  I was given instructions for pelvic floor muscle exercises plus physical therapy.  Due to the size of the reduction, I was told to expect dripping for at least two months.  No pain, blood is no longer present, nor are clots.  Looks like we in the US are finally catching up to the medical advances in the UK. Best of luck to all of you.

      Hi Geezer,

I think mine was just 70 millgrams but a guy who had his done the same day was as big as a cricket ball. Maybe that's a reason that HoLEP can give a long term result because they do take a lot away.I suppose that also may reduce the chance of having a cancer there.

Anyway glad the USA is catching up and good luck to you in the USA.

Did you guys who had Holep left with a catheter?

I didn’t and my urine is still red.  I’m drinking tons of water as yesterday in order to void.
