Arthritis thewith torn gluteus minimus and medius

After 7 months of pain, physical therapy, limping, weakness in my leg, not sleeping, I had an MRI which showed glut tears, labrum tear and lots of arthritis in my hip. 4 days ago I had a diagnostic injection of cortisone and numbing stuff under fluoroscopy deep into the hip joint to separate the glut pain from the arthritis. Getting some relief in weakness and hip pain, but gluts still painful and restrict a full step when walking.  We talked about an anterior hip replacement, which I hear most docs are doing here in the US.  The recovery is shorter and since they don't cut muscles to insert the implant, there is no restrictions or PT after. Scar is hidden by underwear elastic, which would appeal to a younger patient. He said he would repair the tendons at that time.  Question  I can't seem to find any information about how the gluts repairs are sutured to a metal femoral head. I do find info on repairs after THR, but that's 2 surgeries.  I realize every patient is different in the exact tear, size, etc, but I want to know more before I sign up, and the notes I read on this forum and so informed and helpful I hope someone out there has some thoughts or info to share.     No advice other than to private message Graham (Rocketman SG6UK) who is one of the forum's sages and he created a layman's guide to thr. He has had 2 thr and a revision and may have advice on this or recall specific discussions that might help you.

    I was wondering what state you are located in? I moved from NY to Greenville SC 3 years ago, I am going in next Tuesday for postior incision LTHR, I couldn't find 1 Dr on this side of the tate to do an anterior cut. Sorry I don't have advice for you, wishing you well!!

    Hi Maryboo,

    You need to find a surgeon who will do your THR through a direct lateral approach.  That way tears and arthritis are fixed in one operation.  Forget the anterior approach.  If you have it through the front it may make the glut tears worse but will not be able to fix them.

      I still am unclear if the tendons are affixed to the new metal head. These muscles are sometimes referred to as the rotator cuff of the hip. In the shoulder the torn muscles are attached by sutures to the humoral head and have a lengthy recovery time till they heal into the bone. In the hip I am wondering where exactly the tendons get screwed to since the femoral head is removed in s replacement

    Hi Maryboo,

    The tendons are reattached to the bone and the time to do that is when you have a hip replacement.  The reattachment is a procedure all on its own and not every hip surgeon does this type of operation so you need to find someone who is familiar with the problem.  If interested, check out https://patient.info/health/greater-trochanteric-pain-syndrome

    It will clarify this for you.  Also you may find that you only need your tendons repaired and perhaps not even need a hip replacement.

      I have been through several diagnostic procedures and the gluts do need repair or debridement which he said he will do at the time of replacement. I know the tendons are sutured to the bone - I've had a shoulder rotator repair earlier this year. My surgeon had done a number of these and I should have asked him when I was there but when I see him next I will ask. I will check out your info. Thanks for sharing
