Blotchy red rash/irritated skin on penile head (lasted for over 3 moths now)?

Part of the skin on the head of my penis has become slightly red and is causing some pain/irritation.

It first appeared the day after I used a condom so I think I must have had a reaction to the latex or some other substance in the condom.

The tip of the penis became super sensitive and noticeably hurt whenever it touched my underwear.

The skin was and still is noticeably red close to and directly around the meatus. This redness gets worse and bigger after I've come out the shower or masturbated.

An hour or so after ejaculation, I get a slight burn/irritation at the very end of the urethra when I urinate and this can last for a few days before subsiding. This has put me off from masturbating as much now.

This has lasted for about 3 months now with no signs of improvement.

I saw a doctor and was prescribed timodine/hydrocortisone cream and this didn't have much effect and am slightly worried it may just make things worse so have stopped using it for now.

It almost seems as if the redness comes from within the skin rather than on the surface.

I don't think its balanitis as I haven't had that for over 12 years when I was a kid and that was with the usual smegma discharge etc, whereas this is just redness. 

I've attached images to show how it looks straight after showering, which is when the redness is at its worst and the contrast between the normal skin colour and the redness can be clearly seen.

    2-5 days ---> tingles and nibbles at tip (ignored)

    5-10 days ---> tingle turned into irritation/heat/buzzing/raw pain in HEAD/TIP (thought would ride it out)

    10-14----> Major raw pain in head, extending into Urethra, inflamed/tight, infected feeling, came down with 1-2 days of flu symptoms, chills, sweats, achy joints etc. Visited clinic. Was prescribed Azo and Cefix, tested for Chlam, Gonno, HIV, HSV-1/2, Syphillis = all negative. Did NOT have any discharge, lesions or any outward signs.

    14-28 days, slow recovery, swelling and inflammation subside, Urethritis still present but 2/10 pain. Some days better, some days will flare, at night during sleep always worse.

    1.5 months, most symptoms gone except for some ever so slight tingling/irritation at very tip now and then, feels like nerve damage almost... however, everyday seems to be improving. Feel 95% back to normal (helped from my own treatments  outline further below)

    **side notes, I also concluded that because I was rough on my stick during the experience, I caused some micro trauma to the tissue in addition to whatever else was going on. Research, "grip of death". Definately more mindful of that now...

    - during the buildup of pain, noticed I felt tired and run down, almost like I had a cold or that feeling of fighting off an infection. I would get a warm/flush/sensitive feeling in my left inner thigh...the pain/irritation would almost jump from thigh to urethra within a few hours time... then in later stage to my prostate...

    - about a week into the course of antibiotics, I noticed the head of penis looked really blotchy (red and purple) and leathery looking (like an avocado surface). Really alarmed me because it looked like a nasty snake head...meatus was slightly red sometimes also, slightly inflamed. As mentioned, a triangle shaped patch of redish (irritated, inflamed) looking skin on top of head also. Shows worse sometimes, othertimes almost goes away.Cont.

      This can happen to anyone, even low risk, monogamous relationship etc. it sucks but it will always get better with time and this can happen faster, if you make the right steps, treatments and changes in lifestyle. I hope I really helped people here, you'll never get that sort of info from an MD, especially a GP. They're either to busy to go into specifics or are just concerned with pushing the latest medications to get their kick backs... I think pharmaceuticals should be a last resort... they'r mostly band aid solutions. 

      Another thing I forgot to mention, you need to get your urine on the alkaline side, and your body PH also. When your urine is acidic, it just serves to irritate the urethra, bladder and tip even more...

      So I was a coffee drinker, about 1-2 medium cups day....nothing crazy but still....I think over time that lead my body PH and ofcourse my urine PH, to lean towards acidity.... when I first got this infection, I bought some urine analysis strips (highly recommended, great preliminary way to diagnose problems ranging from infection, to disease to Ph levels)... I tested probably 40 strips throughout the first couple weeks of the infection, this was before I made changes to my diet.....and noticed I ALWAYS had 5.0Ph(most acidic) and high specific gravity (lots of particles in urine)....  Once I started eating well, lots of veggies etc. for the first time, I hit Ph 7 (perfect, neutral) and lower specific gravity... it would still go back to acidic now and then but urine Ph is very diet sensitive.... it seems like the better I eat, the more prone the urine is to go towards alkaline. I attribute that to cutting out all the bad stuff like coffee, processed foods and eating greens/veggies/fruits etc.

      Getting your Ph in balance is super crucial, acidity in body and specifically in urine (in our situation) is only going to exacerbate Urethritis, Cystitus etc.

      If anyone has any other questions. I can try to answer them to the best of my knowledge...

    Same thing i am suffering from!!!! This incident happened approximately 8 months ago - I had sex with a woman who had a bad reputation for sleeping around a lot. I used a condom however the condom broke midway and didn't realize it until we were done with having sex. When i saw the condom broke i freaked out. I took a bottle of Scotch/whiskey and washed my penis (like an idiot) with it hoping that i wouldn't catch any diseases. Less than 2 days later my penis was feeling different and got a rash and tight forskin (either from her or the whisky)

    I went to the doctor around a week later and i got tested for most common STDs - All came back negative. The doctor said i have balanitis and he gave me some anti-fungal cream and i did that for a few months and not much of a difference happened, after several months with no change i was prescribed a stronger anti-fungal cream and steroid creams and still no change. After doing my own research, i purchased products that would help and nothing really worked.

    There is tight forskin. The head of the penis does get swollen when i masterbate and penis gets super dry  my main problem is the rash like look (see pic)! I have tried using almost all products drying up my bank account yet, nothing works! I have even switched to natural products like coconut oil, Aloe-vera, Castor oil and even tried Tea tree oil that almost burnt my penis as it was potent. I even stopped using any products for a few days to see if it was an allergic reaction, yet nothing has changed. Sometimes it turns red other times it turns brownish, but mostly red.

    This has greatly affected me mentally! I keep saying im going to start a new leaf and forget about all this and focus on what is important in life, but its hard to do when you got to take a p**s every couple hours. 

    I feel most of us will have a cure for skin related issues in another 8 years time! by that time, technology would be at a stage where all it will take is a spray or a tiny tube can contain nanites or some kind of miracle technology.

    Either in 8 years or maybe im watching too many futuristic movies.... Either way it will come eventually (maybe even less than 8 years) however when it does come out it there is high probability that this will be super expensive so start getting rich or find away to get rich. Concentrate on your goals and this will be a possibility. Focus on what matters now rather than what could be. Will update you all on my progress. Be strong, we all once were, we cant let this be the fall of us. We are much stronger than this.

      Same story. Had an encounter with a body to body massage escort. Didnt have penetrative sex but alot of naked genital rubbing. A week later i started getting testicle pain. Had all the tests but all came out negative. Was on courses of cipro and doxy at different times. Nearly been a year since exposure and even though my testicle pain had 90% gone my penis isnt the same, its a weird shape i get the odd itch and tingle, i get strange lines and marks on them that come and go.

      Only thing i can think of is herpes or yeast infection.

      Can i ask everyone that is active on here with similar issues are u having sex with your partners and are they symptom free?

       Read my post above....

       Get tested for herpes atleast 3 months after exposure to rule that out. If it's not herpes than most likely HPV virus...

      Its so hard to get a herpes test in uk. Especially if you dont have a blister.

      I have a urology appt again next month so will try and persuade him to do a herpes test.

      That's too bad... yeah, definitely try to get tested for that somehow, it will be just one more thing to check off the list....

      Have you tried Nystatin cream? It is the third ointment I have tried and it seems to be really working and I am almost symptom free after 6 days.

       Partner is symptom free, went for tests for chlam, gonn, Syph, HIV ---> all neg.

       Not tested for BV, Thrush etc. but recent pap smear, like a literally last week, her doctor gave her green bill of health so if she had an overgrowth, bumps, rash or anything out of the ordinary, I'm sure her gyno would have saw it. She also has ZERO symptoms of anything, just went through testing to help me....

      However, all ok on pap smear does not mean she doesn't have HPV with no symptoms.....which is most likely the case. She didn't redo the tests for herpes but said she was all ok from previous testing since she was last active.... I'm going back for herpes testing in a few months just to finally rule that out...

      As I menioned, because I had a cute, very tiny, little wart develop, I lean toward HPV in my case. But still not 100% certain, just wish there was a damn test you could take to at-least confirm HPV in system. I'd pay good money for that just to have the final peace of mind...

      Also, ever since this started, I get tingling in my right big toe, wondering if it could be pre-diabetes related also. I'm fit and healthy, exercise, reasonably good diet...no glucose or keytones in urine... have some of the symptoms of type 2, but they're pretty vague... almost everyone can check a few off the list...

      Hey Dan, Yes i have tried Nystatin cream, Triamcinolone, Tacrolimus, lanolin, all of the terrasil products and a bunch of other products. Nothing seems to work. I feel that using all these products will cause more harm than the simple rash/irritation shown in the picture but i feel that i cannot stop trying as it is mentally effecting. Although since i have tested out mostly all the products available, i will have no choice but to wait till a miracle medicine is created. This miracle medicine is some bulls**t hope that i keep telling myself so that i wont be affected as much but its still better to have some kind of hope than non at all.

    I have been watching this thread for several months now and am ready to add my saga. After reading through lots of the responses, I am convinced there are a range of disorders/problems contained so there can't be a single solution to all of them,  I have been dealing with mine since mid January - 4 months now, and the good news - knock on wood - is that it seems to be finally gone for me. So read on or go to the bottom to see what has helped me the most.

    Here are some details: I am 53, circumsized caucasian. I use viagra, and I have had unprotected sex with 4 different women over the last 5 years or so. My problem occurred the day after I had typical vaginal intercourse in January, and I developed a dark red, circular/oblong patch of the right side of the glans about 11mm long and 6mm wide. Like many have described, it was very irritated and made it difficult to walk. I felt the tingling/throbbing/pins-and-needles that some of you have described and i indeed felt a little discomfort in urination, but almost more of a restriction that caused me to dribble when done which was new. I thought I had really done it now and contracted a nasty std. In retrospect, I recall that I had felt a little discomfort in urination and ejaculation before it "came to a head" with the irritation. After a week or so of suffering, I went to my GP and got a full lab panel done, and like most of you here, they all came back negative. And on the day I went, the symptoms had subsided so they didn't look all that bad for the Dr. She surmised it was contact dermatitis.

    That's when I found this thread and it dawned on me that I might have a fungal/candida infection. I contacted the GP and she prescribed a 7 day run of flucanazole 100mg daily and an anti-fungal and steroid cream Clotrimizole and Betamethasone. After the 4th day of the pills, my symptoms finally subsided and I thought I was done with it. But within a few weeks, it came roaring back. I let my GP know and she referred me to a urologist. I kept using the cream but realized it did nothing. I actually bought some over the counter yeast infection cream which seemed to relieve the pain a little more than what the Dr prescribed.

    Right after Christmas, I had a nasty cold, upper respiratory infection, and I took a self-prescribed z-pack antibiotic cycle. When the rash was really at it's worst, I also had the mouth sores, so it made sense that I had contracted thrush. (That's another retrospect realization.)

    Between the time I re-developed the irritation and seeing the urologist, I traveled out of the country on business, and while there, I went to a pharmacy and bought 5 150mg of fluconazole since my GP did not want to prescribe me more until the urologist. I took them daily, and even though they were hard on the guts, the symptoms went away again. By the time I got in to the specialist, once again the symptoms had subsided.

    By the end of the week however, the same red spot appeared and I had symptoms again, but not as bad as the initial outbreak. I contacted my GP and let her know that the urologist thought I had balantitis and prescribed me the exact same cream that I had already tried. I told her that the fluconazole seemed to work and that maybe I needed a longer cycle. I also asked her to prescribe Elidel cream - because I had seen it mentioned here on this thread. I got a call from my pharmacy to come get my two new scripts. She provided me a 2 week cycle of fluconazole and a new cream. 

    I decided to wait on taking the pills and see if the cream would have any effect on its own. After about 5 days of Nystatin cream twice a day, my symptoms have almost all disappeared for the first time in 4 months. I may be jinxing myself and have it blow up tomorrow, but I honestly feel better now than I have all year. 

    I know that this won't work for everyone, but this has come as a real relief for me. So for those who have similar symptoms and some treatments haven't worked, keep trying and stay on your Dr to let you have different meds.

      Can you give a percentage of normality reached after the Nystatin... 80, 90, 100%?

      Did the redness and "obstruction" symptoms go away also?

       Why do all penile creams have PEG's(polyethylene glycol) in them, Nystatin has polyethylene glycol 400 in it. That's known to cause kidney damage if absorbed into the bloodstream.... mucosal membranes like the penis will absorb it x300 compared to skin.... most skin products have it, don't think it's much of a problem on normal skin, but the penis is mucosal and highly absorbant.

       I found a very indepth study that looked at PEG's and their effect on mice....didn't turn out so good for the mice..

       Sucks cause I had picked up this penile health cream on-line and it seems to help but I noticed it has PEG 100 in it (that's the smallest particle type, even more absorbable) so I can't use it anymore....

      I'm going to try aloe vera now and suragel.

      ive tried a bunch of things as well but what really helped me so far is something called Elocan (mometasone ointment) .   Its only available by prescription .   Hopefully one of you guys will try it and get back to us.   I will keep following this forum to keep each other updated

      The Nystatin cream seems to be pretty generic - it says "Nystatin Cream, 100,000 USP."  I googled it and it looks like all the US brands seem to be all the same. I will say that after these weeks, the symptoms have not completely gone away - I still have that blotchy red spot - not as obvious or painful - and at sometimes, I feel it against my underwear more than others. But I do believe that the cream has certainly kept it all calmed down. I have had sex one time since I wrote the post and it affected me some, but not horribly. I have not felt the obstructed feeling at all. And since this has lingered a bit, I went on and started the Fluconozloe 100mg and am on a 14 day cycle - about 1.2 way through it. My hope is that the cream has slowed it down, and if the fluconozole can help, it might have a better chance of curing if this thing is on a downward trend. After all of this I am really surprised nobody has an answer. But then again, I still think that even though we seem to have some similar symptoms, there could be a wide range of causes.
