Blotchy red rash/irritated skin on penile head (lasted for over 3 moths now)?

Part of the skin on the head of my penis has become slightly red and is causing some pain/irritation.
It first appeared the day after I used a condom so I think I must have had a reaction to the latex or some other substance in the condom.
The tip of the penis became super sensitive and noticeably hurt whenever it touched my underwear.
The skin was and still is noticeably red close to and directly around the meatus. This redness gets worse and bigger after I've come out the shower or masturbated.
An hour or so after ejaculation, I get a slight burn/irritation at the very end of the urethra when I urinate and this can last for a few days before subsiding. This has put me off from masturbating as much now.
This has lasted for about 3 months now with no signs of improvement.
I saw a doctor and was prescribed timodine/hydrocortisone cream and this didn't have much effect and am slightly worried it may just make things worse so have stopped using it for now.
It almost seems as if the redness comes from within the skin rather than on the surface.
I don't think its balanitis as I haven't had that for over 12 years when I was a kid and that was with the usual smegma discharge etc, whereas this is just redness.
I've attached images to show how it looks straight after showering, which is when the redness is at its worst and the contrast between the normal skin colour and the redness can be clearly seen.

I've struggled with the same symptoms for 5 months. In my case this came on after working out two days on an elliptical machine wearing blue jeans . Stupidly I didn't shower afterwards and a rash developed in my crotch area . That went away using Desitin cream . Noticed a weird fishy smell coming from my foreskin . Headed immediately to the doctor, because I never had anything like this before . Primary care doctor was unhelpful. He said it just looked irritated .
Desperate, I tried many of the same OTC medications as you guys to no avail. A dermatologist in my area now has me on 3 drugs, all creams. I mix them together twice per day. These include a steroid, an anti fungle, and an anti bacterial. Been on it for two weeks with a goal of one month.
Progress is slow. I can confirm that helps, but the symptoms return quickly if I miss a dose.
I will follow up with the dermatologist in 2 weeks if it isn't 100% healed. Then may seek help from a urologist and proctologist.

...be careful with the steroid, especially on your glans. long term use can really mess your skin and cause it to thin. do some research and you will find some nasty threads about steroid use. there is a prescription cream that was recently recommended to me that did not have the steroid. it has an antifungal and antibacterial topical mixed with aloe. you may want to talk to your doctor about it. PM me if you want the name as I wont advertise. I am not sure if it works (can't validate if it is effective) but am going to start using it myself because there is less risk than using a steroid.

Yes , I have had urologist to look at it. He had no clue and I was referred ultrasound scan for groin and abdominal all came back negative. I am waiting for results of a CT scan will keep you guys posted. After lot of research I think this is related to see skin infection probably thursh that have spread to urethrta as I didnt continuously treat for it. The problem is lot of medications that we received in this thread is trial an error and I don't think that is good. We need o find the route cause. As I read correctly gluten could be an issue in my case as I developed lactose intollerence after this. Maybe intendtal parsitis? Did you guys ever tested for it

I think it can be recurrent if not treated the underlying cause. And specially uncircumcised doesn't help as it can grow under foreskin. My current irritation is around the foreskin. It is very irritated to touch. So I am hoping oral + cream Will help. Also stress level and eating patterns have lot to do with this. The flair ups are almost tied to what we eat I think. Especially when I eat spicy food it flare more

Same here irritation depends on what I eat so I think it can possibly be leaky gut and we should start to eat anti inflammatory foods and get on diet like that for a few months and see improvement

It has been a couple of months since I posted - I think my original is somewhere on page 21 of all of these threads. As a refresher, I have had a very similar experience as most of you that included not only redness and irritation, but stinging pain when I walked, pins and needles in my head when still, and originally some amount of problems urinating. I went through the entire gambit of treatments, trips to GP and urologist, three different medicated creams, two rounds of fluconozole, and home remedies. Here is my take on all this after dealing with it since mid-january. I now am certain that I had a fungal infection/thrush. I still have a couple of rounded splotches with dark dots in the middle on the head of my penis that look like pics of ringworm that you can find online. I am now convinced that the fungus itself was cured early on with the creams and fluconozole that I took. However, I believe that the outbreak did some tissue and/or nerve damage to the affected area. As I have never felt like the spots have changed or gotten bigger, I think that what I (we) are dealing with is the aftermath. This thing has finally settled down for me and I am able to forget about it since it no longer irritates my glans. As an aside - and I do not know if this has anything to do with it - I got some cream that contains CBD (cannabidoil) and thought I would try it since so many people are touting it's miraculous benefits. I applied it twice a day for about a week, but I decided to quit because I suddenly got irritated again. I had also been using the Vitamin C Oil and Lanolin (which really made the irritation bearable when present.) But after I used the CBD oil, even though I thought it was making it worse at the time, I have been symptom free for the longest stretch since this all started. I have also put the tool to the test without any issues.
So for me, I think this is like an injury that will eventually heal and I will forget all about it. I do not have an active infection, but the one I did have in January created enough trauma that it just takes a long time to heal. I also wonder if the Vit C Oil, Lanolin, and CBD might have helped out. But for the last couple of weeks, I have not treated it with anything and I am pain free.

I appreciate you following up with your experience so far. I’m also glad it has seemingly gotten better for you as time has gone on. I have had very similar experiences with redness, irritation, pain with rubbing against clothes, pins and needles, skin changes, urinary troubles, etc. Have been to countless doctors... all clueless. Unlike you though, I have my doubts about what the initial trigger was that set off my symptoms. After many tests, specifically a lower lumbar MRI, the tech who read he images noted a Tarlov Cyst at S2. This is the exact location that feeds the nerves for dermatomes in the genital region as well as nerves that control bladder and bowel function. I urge you to get an MRI of your lower lumbar, sacral area and report back. Do some reading on Tarlov Cysts. There is widespread misinformation in the medical field that these “do not” cause nerve damage and or pain. That is false, they compress the nerves against the bones in your sacral area and can absolutely cause symptoms. I will post more later. I would definitely urge anyone that is experiencing unexplained skin pain, redness, irritation of the genital area to get checked out for issues relating to the nerves. Ask for a copy of the MRI findings because some doctors won’t even tell you the “incidental” findings.
