Blotchy red rash/irritated skin on penile head (lasted for over 3 moths now)?

Part of the skin on the head of my penis has become slightly red and is causing some pain/irritation.

It first appeared the day after I used a condom so I think I must have had a reaction to the latex or some other substance in the condom.

The tip of the penis became super sensitive and noticeably hurt whenever it touched my underwear.

The skin was and still is noticeably red close to and directly around the meatus. This redness gets worse and bigger after I've come out the shower or masturbated.

An hour or so after ejaculation, I get a slight burn/irritation at the very end of the urethra when I urinate and this can last for a few days before subsiding. This has put me off from masturbating as much now.

This has lasted for about 3 months now with no signs of improvement.

I saw a doctor and was prescribed timodine/hydrocortisone cream and this didn't have much effect and am slightly worried it may just make things worse so have stopped using it for now.

It almost seems as if the redness comes from within the skin rather than on the surface.

I don't think its balanitis as I haven't had that for over 12 years when I was a kid and that was with the usual smegma discharge etc, whereas this is just redness. 

I've attached images to show how it looks straight after showering, which is when the redness is at its worst and the contrast between the normal skin colour and the redness can be clearly seen.

    Mine is still same its been 2 months,was using protopic and allantan.But i have realized Travocort reliefs it immedietly but not cures.Maybe the cortizon it has helps to relief.When i use it redness fades away.But soon comes back.

    I think my condition is lichen sclerosus. I am not sure whether this makes sense. But this is what dermo diagnosed as

      All right.

      This lychen thing is by far the most accurate diagnosis that makes some sense. However exact cause unknown.

      I think by now we all have came to the conclusion that what we have must be an autoimmune disease (and it might just be gut related.)

      No local viral or bacterial infection will linger on for this long (in my case 1.5 years) without either going absolutely out of control or disappearing.

      I had both antibacterial and antiviral courses of tablets and no response.

      Don't get confused by the fact that the initial trigger could have been some sort of infection that ruined the skin of the glans.

      I had a rough infection on the glans and right testicle was in pain but my body fought that infection and this thing was left behind.

      I can live with it now, I only have mild symptoms. I can just feel that the skin is "off" and a little bit inflamed but no real issues.

      The only thing that helped me:

      I started "wearing" my foreskin pulled up. I have short foreskin "turtleneck" style :-), so it stays up when pulled up.

      It takes a good month for your glans to get used to this new life but it worked for me.

      What I have tried:

      Antibacterial tablet courses

      Anti herpes tablet course

      Numerous creams but they always made it worse.

      No alcolohol for months.

      No dairy products for months.

      No softener for my underwears.

      Other problems I have:

      Stuffy nose all the time.

      Perodically reappearing rashes on my hands, some cheap soaps trigger it some don't.

      I was coming out of depression and had an anxiety disorder when it started, but I am completely healthy from a mental point of view now as I have turned my life around and I am as happy as I can be.

      I have done all the tests, all negative.

      Swab, urine, twice. Comprehensive blood test. All negative.

      My ex at the time was positive with Candida Albicans, Shingles (Herpes Zoster), and one bacterial infection I can't remember the name of anymore.

      Thanks for the information Richard. I don’t believe this to be lichen sclerosis. In my case, I believe it to be one of two things in my case. The first is nerve inflammation/damage secondary to a Herpes infection. The resulting diagnosis of pudendal neuralgia makes a lot of sense. The other possible diagnosis for me is the trigger, Herpes in my case, caused the Tarlov Cyst located at S2 in my sacral spine to become symptomatic and start causing me all these issues. Tarlov Cysts are widely thought to not cause pain, at least in the medical community, and unfortunately that is 100% false. I had an MRI of my lumbar area and the radiologist notes an “incidental” finding of a Tarlov Cyst at S2. Further research has led me to believe these can cause major symptoms in the genital area and pelvic area. I would definitely look into getting a lumbar and or sacral MRI.

      Hi Richard,

      Your situation sounds kind of similar to mine, except there was never really an obvious trigger with me. I find that it feels more “off” and uncomfortable after I have showered, exercised, or after a long day; it’s very bizarre I was wondering if these situations where it’s worse is the same for you? If so we may have a similar issue going on here. Let me know, thanks.

    Hi All,

    For similar condition topical application of "Clotrimazole Betamethasol disproportionate (combined anti fungal and a steroid) daily once for 7 days has helped me. The Dr told me this might be Balanoposthitis.

    Hope you all recover soon from this menace.

    you are right, it doesnt makes sense that lichen started all of a sudden. Maybe this needs to be investigated still. Did anyone get a biopsy? will this will help to identify the route course?

    I have suffered from a rash and the red dots for well over a year, but I was finally able to determine the cause on my own. The Drs never seemed to get it for some reason, but the first thing anyone having issues needs to do is make certain you are tested for STDs, particularly if you have had unprotected sex. If you get tested and you are clean, or you cleared up your STD and you continue to have this rash and the red dots, then you probably have the same problem I had, so stop fretting over HPV and some new unknown STD, because that is very unlikely if you have had this ongoing for months without any treatment success and you tested negative for STDs.

    Here is what my problem was all about. I noticed I would always have a small dribbling problem after urinating. It was very small and almost unnoticeable most of the time, although some days it would be worse than others, but this was the cause of my entire problem that nothing would cure. What is happening is that your skin can become sensitive to the constant barrage of your urine. I'm circumcised and it still bothered me, so I can imagine it will be even worse for those that are not circumcised.

    Understand that your urine is full of things that your body/kidneys are filtering out of you, and after prolonged contact over and over, your skin starts to react from it. If you have PH issues, it can be even more exacerbated, so as someone else already suggested, test your PH and if it is acidic, work on your diet to further help clear up this issue. If you are getting yeast infections and they clear but come back, the reason you can't beat it is because the underlying cause is still there, and it might be your urine. Again, you could just have a dribble or two, so it might be mostly unnoticed but still the issue.

    Now, for the important part and how I beat this problem. The way I finally kicked this was with carrying personal wipes and always wiping after urinating and then applying what is called a barrier ointment. They make these individually packaged wipes, so just keep one or two in your pocket. You can find these along with a barrier ointment for incontinence on Amazon and you will find many choices. Try Amazon and read the reviews to find the good ones. Just clean off with the wipe, then apply the barrier ointment. This cleans the issue and prevents any further moisture or urine form reaching the skin due to the barrier ointment being applied. I also use a lot of gold bond in the green bottle. It's helps keep things dry and the menthol is a natural anti-fungal agents as well.

    You can google Incontinence-Associated Dermatitis, as that is the medical name for this issue. It's not an STD and it's probably not anything you can pass on to a partner unless you have active yeast, bacterial or fungal infections along with it. Keeping your member clean, dry and urine free will allow you to finally heal this completely. It's basically the same thing as diaper rash for adults and it causes bacterial infections, fungal infections and yeast infections. Most people think this is only a problem for diabetics, but that is not true. You do not have to have sugar in your urine to have problems from urine contact, although you might want to check for sugar as well as the PH of your urine to be certain those are contributing factors.

    One other tip to help the dribbles is once your are finished urinating, push lightly just under your testicles and you will notice it may push out a few remaining drops. The right spot is just at the base of your testicles, so just push there and you may notice it pushes out a few more drops.

    I have always suffered from benign prostate enlargement due to high testosterone levels, and this causes my dribbling problem. Some of you probably have this same issue or some damage to your urethra from a previous infection that causes you to dribble, and it can be only a drop or two, but it can be enough to give you problems. Eliminate any urine contact to the skin of your penis and you will see it finally start to heal up for good.

    Hopefully this will help some of you to find some relief and to help you worry less about having some unknown medical condition, because it's really pretty simple once you understand what is causing this. This may not be everyone's problem, but I would bet many of you that found this started after a UTI or STD infection have some damage in your Urethra that causes dribbling of your urine. You may also find that your PH is really acidic or you may even have sugar in your urine, which really becomes a problem for yeast and fungal issues.

      Makes sense, also for all those here using castor oil and vaseline.. Cause that makes a (small) barrier too.

      Thanks so much man! I really think this is the solution. Mine started with a chlamydiainfection 2 years ago. I noticed that the rash was close to the urethera opening and looks better the further away on the glans. When i read your post one week ago I had the idee of making a small diaper out of toiletpaper. Then fold it over the opening to absorbe the leftover p**s after i went to the bathroom. And in just one week of doing this my glans looks better than it has for the last 2 years.

      EQ (or Bowen Disease if not on glans) is HPV-caused cancer. The pictures you see on the Internet are of full-blown hard core cases; the lesions in earlier stages may look like a rash that doesn't go away. My doctor took a biopsy to check for that and other things but he did not do the biopsy on the top of my glans (he did it underneath and on the shaft, away from the problem area). I am hoping he will biopsy the red area around the meatus.

      Anyone had a biopsy right next to the meatus? It sounds tricky?

      Unfortunately I think for most of us, the short answer is yes... just dealing with it. When you go to doctor after doctor and they can’t offer you any solution or relief... what is one to do except constantly search the internet for others with the same problem. Mine has been flaring the past day or two and really bothering me... and it gets me so upset. I truly 100% believe this to be a nerve dysfunction due to some triggering event or injury. Has anyone else followed up about sacral lumbar MRIs to see if Tarlov Cysts putting pressure on the nerve roots is a commonality here? It has been linked to vulvodynia in women. I have a Tarlov Cyst myself at S2 but not much is known about them and they are largely thought to be asymptomatic and “incidental findings.”

      I wouldn’t say I’m giving up forever. I decided to stop obsessing over it and to live my life while I casually keep my eye on possible remedies. I mean REALLY stop all this obsessing and you know what...I feel freed and my symptoms have subsided to the point where I’m basically over whatever this is. Its not going to rule my life any longer.  I was obsessing for the better part of two years, getting STD tests for a year, going to doctors non stop and letting this run my life. I feel like a new person. Reborn one might say.  Something is going on and its not right but I don’t have an std, my Johnson isn’t going to fall off and I’m not going to make anyone else sick.  Its a minor nuisance and not much more.  Done.
