Blotchy red rash/irritated skin on penile head (lasted for over 3 moths now)?

Part of the skin on the head of my penis has become slightly red and is causing some pain/irritation.

It first appeared the day after I used a condom so I think I must have had a reaction to the latex or some other substance in the condom.

The tip of the penis became super sensitive and noticeably hurt whenever it touched my underwear.

The skin was and still is noticeably red close to and directly around the meatus. This redness gets worse and bigger after I've come out the shower or masturbated.

An hour or so after ejaculation, I get a slight burn/irritation at the very end of the urethra when I urinate and this can last for a few days before subsiding. This has put me off from masturbating as much now.

This has lasted for about 3 months now with no signs of improvement.

I saw a doctor and was prescribed timodine/hydrocortisone cream and this didn't have much effect and am slightly worried it may just make things worse so have stopped using it for now.

It almost seems as if the redness comes from within the skin rather than on the surface.

I don't think its balanitis as I haven't had that for over 12 years when I was a kid and that was with the usual smegma discharge etc, whereas this is just redness. 

I've attached images to show how it looks straight after showering, which is when the redness is at its worst and the contrast between the normal skin colour and the redness can be clearly seen.

    I also have still no solution for the exact problem.

    Goes on for 6 months already now.

    But my closest guess is: candida albicans

    Even though I treated it with 10 days clotrimazole and diflucan pills for 3 days.

    Candida can be really hard to treat.

    2. Guess hpv high risk

    Even though all doctors say, probably not and also the swab test for hpv showed negative. I can't get it out of my head.

    3. Non typical herpes flare-ups.

    I suggest you to do a hsv1 Hsv2 test, and hpv swab test.

    I just did a new candida swab test and I am waiting for the results right now.

    This may be an unwelcome theory, but I'm wondering how many of these cases are atypical manifestations of herpes.  Lots of people here are saying they've had "full STD tests", but those don't usually include an IgG blood test for HSV 1/2. You have to ask for it, and often the doctors will refuse if you don't have the typical blisters and sores.  (Understandably, the results can be ambiguous and can create more questions than answers.)

    HSV infects the nerves, and I'm wondering if this irritation is caused by nerve irritation caused by that infection. Irritated nerves release substances that cause inflammation (cytokines, substance P, etc.) Some 80% of HSV infected people don't even know they have it, because their immune systems are keeping the virus suppressed.  So perhaps what we're experiencing is a "mostly suppressed" HSV infection that has still resulted in neuropathy/nerve damage.

    FWIW, I've been in contact with Dr. Hook at the American Sexual Health  Association, who used to post here, with this theory and he did not think it was plausible.  Every other doctor (2 derms, 1 urologist, and my GP) also think this is not possible.  The Drs are all telling me that "post herpetic neuroalgia" only occurs with herpes zoster (shingles) and not HSV, but I see many reports of this on various HSV websites. That said, I am a skeptical person and it's plausible that these people are OCDing about their infections and creating their own somatic symptoms due to anxiety. It's tempting to get sucked into anecdotal stories from anonymous people.

    I really hope my theory is wrong. I twisted my GPS arm and got her to order me an IgG blood test, which I took a few days ago. I'm anxiously awaiting the results.  I've been experiencing this same constant irritation for the past 3 months, which came on suddenly, 5 weeks after a massage that involved some mutual touching but no genital-genital or oral-genital contact.  I never had a sore, bump, or visible lesion, just rawness and irritation. 

    Questions for anyone reading this:

    How many of you are certain about your HSV status?

    Has anyone passed this mystery irritation on to a partner?

      This may be the case for some , make sure you also get hsv 1 igg , I'm 6 months from possible exposure and I'm negative for both hsv 1 and 2 through igg. I can't take a swab test because I have no lesions or blisters. Yesterday I took one more test which would be 25 weeks, I won't get my results till Tuesday. If negative I'll try seeing an hpv specialist because idk what may be causing my symptoms. What was your exposure?

      Update: My IgG blood test was negative for HSV 1/2. So I can scratch "crypto-herpes" off the list of potential causes, thankfully.  

      I don't really know where to go from here.  I've wondered if it's some weird manifestation of HPV, but I can find no evidence of that ever producing pain.  Maybe it's fungal or bacterial.

      I definitely had the full battery of tests for all the nasty and common STI's at any number of intervals following my unprotected contact.  I used one of those discreet online services to get a full battery of tests at the following intervals: 30 days, 60 days, 90 days and again approximately 6-7 months after contact.  Given the symptoms I was experiencing I saw no other possible explanation save for some sort of STI but everything came back negative.  The miracle of modern medicine and laboratory testing tells me I do not have an STI.  6 months is it.  If you don't test positive by six months then you are NOT infected.

      Not true, ppl don't produce antibodies ever. I've talked to a couple of ppl who tested positive hsv 1 at 12 months and another after 2 years for hsv 2. Your body might have a strong immune system which doesn't need the antibodies at the moment, either that or you're actually negative. I'm 6 months in and still negative , even though I still believe I may have been infected.

      Im pretty sure the production of antibodies by white blood cells in humans to fight bacteria and viruses is a well established scientific principle so I’m not sure why you would make such a statement. Please explain.  Thanks. 

      While there are some rather exotic circumstances where anti body production is delayed or detection by lab tests is more difficult such as folks undergoing chemotherapy or other diseases that compromise the immune system, that is relatively rare and I have no such special circumstances. I am a normally healthy person and have been so for all of my 40 years. 

      Different types of STI’s are definitively detectable at different time intervals. Larger point being, given that I tested negative at 11 months (I went back and checked dates) the CDC and any professional doctor would certify me as STI free.  Therein lies the conundrum.

      Balrog... you just described my same exact theory about the origins of this condition. Luckily... a couple of doctors near me actually agreed that it wouldn’t be a stretch to connect HSV to possible neuropathy.

    I have the same issue going on about a year now.

    My doctor said perhaps auto immune like BXO after a biopsy. Nothing has worked.

    If it was HSV then it would involve fluid filled sacs. The symptoms Jason and others are having does not match this. It's not HSV period.

    HPV - Yes there are strains that manifest differently but they like an infection they go at their own pace where to the symptoms for this red blotchy is fairly static. To aggravate the symptoms friction, heat, water are needed (at least for myself) - this does not follow a pattern of infection or HPV.

    Systematic Candida - If you took a Fluconzale pill and it has no affect, its not this.

    Penile Psorisis - maybe....

    Auto immune....maybe

    Post Inflammatory Hyper pigmentation  - These are more like moles. Jason and others issue is nothing like a mole.

    Like many others I'm so pleased to have finally found a thread matching my issue.. maybe between us all we can find a solution..

    My symptoms are identical to those described by Jason at the start of this thread and like many of you I have had numerous trips to the doctors, sexual health clinics and recently the top genital dermotologist in the U.K. since my issue begun over 5 years ago. Ive tried all the usual creams mentioned and am definitely clear of ALL stds. The latest 'top' dermo guy just gave me the usual contact dermatitis diagnosis I've had several times now. Nothing ever changes or improves. I use all the special creams for washing and protecting my old boy with no changes. 

    Interestingly after a recent hair analysis test I have been diagnosed with stomach and gut issues (I have had problems for as long as I can remember with digestion) I took this test after feeling low and fatigued for most of 2017. Apparently the state of my gut system is very poor, overrun with bad bacteria and over acidic, most likely caused originally by an antibiotic course and maintained by my poor diet. The specialist said she feels all my issues are related to the poor state of my digestive system. This seemed to fit with some of the other comments I've read here. So I am currently on high doses of probiotics and vitamins whilst eating a controlled diet, eliminating sugars and trying to alkalise my body and restore my good bacteria in the body. This is all fairly new so I will repost if I feel I see any correlation with the changes. Before this I ate a poor diet, sugar rich and was prone to binges of chocolate and sweets which may relate to the way my symptoms have flared up now and then seemingly for no reason. 

    My only other thought 'outside the box' is that through use of viagra the blood vessels in my penis have become damaged as the most obvious change for me over the years of these symptoms is the appearance of tiny vessels which have slowly covered my penis head just under the skin. I hope we can sort this one guys, I'm still trying after 5+ years and it gets my down sometimes!

      I have intermittent irratation which comes and goes, my symptoms are just like the description in the original post, the discomfort almost feels under them skin. I have to say that it has reduced since I stopped using any mainstream soap or products down there but it has never really ceased altogether.
