Blotchy red rash/irritated skin on penile head (lasted for over 3 moths now)?

Part of the skin on the head of my penis has become slightly red and is causing some pain/irritation.

It first appeared the day after I used a condom so I think I must have had a reaction to the latex or some other substance in the condom.

The tip of the penis became super sensitive and noticeably hurt whenever it touched my underwear.

The skin was and still is noticeably red close to and directly around the meatus. This redness gets worse and bigger after I've come out the shower or masturbated.

An hour or so after ejaculation, I get a slight burn/irritation at the very end of the urethra when I urinate and this can last for a few days before subsiding. This has put me off from masturbating as much now.

This has lasted for about 3 months now with no signs of improvement.

I saw a doctor and was prescribed timodine/hydrocortisone cream and this didn't have much effect and am slightly worried it may just make things worse so have stopped using it for now.

It almost seems as if the redness comes from within the skin rather than on the surface.

I don't think its balanitis as I haven't had that for over 12 years when I was a kid and that was with the usual smegma discharge etc, whereas this is just redness. 

I've attached images to show how it looks straight after showering, which is when the redness is at its worst and the contrast between the normal skin colour and the redness can be clearly seen.

    having the same issues and can not find a answer for what it is and what has worked for you any one please help?

    Blood flow issue. You are a Caucasian man, its like that you will be able to see blood flow on the genitals as you would on the face. Its nothing serious.

    I've looked through hundreds of these threads and countless hours of research (leading to severe stress and anxiety and depression) and based on my circumstance and others with similar symptoms and based on my unprofessional self diagnosis the only thing I can think this is is HPV related, or small chance that it's herpes related.

    Rash remaining on glans could be microscopic wart-like HPV rash, or balanitis caused by HPV. Urethral discomfort could also be urethritis caused by HPV, or urethral warts caused by HPV. HPV can still be contracted even with condom use. Also the fact that symptoms remain and not responding to any topical ointments prescribed by derm or any antibiotic would lead me to believe it's viral, again leading me to HPV.

    Things to also consider when looking at your symptoms:

    1. Any change to urine stream? Notice any splitting of the stream at beginning or end or throughout?
    2. Any vein inflammation or new veins noticeable on penis? Notice any tiny red veins near the glans or below it?
    3. Any pain or discomfort in urethra? Does it feel like urine gets stuck in urethra even after urination? Dribbling?
    4. Does rash on glans appear worse right after a hot shower?
    5. Does meatus/lips look a bit more pronounced than it used to? Any redness inside tip of urethra?
    6. Does rash on glans get worse when you apply certain ointments, such as steroids?

      I have just a dry skin on the glans and inner foreskin, since 2 years, getting worse in the november 2020, and more or less the same since now. I've been to 8 doctors, and done a lot of tests, all come back negative, even HPV tests, come back negative.

      To respond at your questions:

      1. No change in urine stream, just sometimes very little pain in the zone under belly and up to penis.

      2. A lot of new veins on penis, but only in the inner foreskin, the internal part when you pull down the foreskin, and some red small veins in the left side under the glans.

      3. Pain in urethra when pull down the foreskin with soft penis or sometimes when move the penis in flaccid status. My urologist said that could be nerves or a little inflammation of the urethra, without giving me any antibiotics, but told me to do an urethral swab, but I'm afraid.

      4. I don't have rash on glans, just dry skin, but get worse after shower.

      5. No changes in lips of the meatus

      6. My penis felt better without steroids cream, I applied steroids cream on penis in March 2021 and my skin started istantly to burn and only now, after months seems to get some recovery feeling better.

      I've made every tests possible, expect one for herpes virus and allergic reaction. I'm supposing it's a kind of skin condition like psoriasis, and I'm afraid of lichen sclerosus, but I have no signs or symptoms, just a damn dry skin that seems to heal very slowly or not healing at all. All the doctors said that I have absolutely nothing, that was all fault of excessive masturbation get extremely worse by steroid cream that in fact left my skin terrible damaged for months.

      We did have that one guy use monistat as a suppository and he said it worked, but I believe he returned later and continued to try to come up with other solutions so perhaps it didn't quite get it all? It's also interesting how many people have said that certain things like antibiotics or antifungals would work for a bit and then symptoms would resurface. That wouldn't make sense with it being viral. Of course that also means there are many people looking at this and some have different causes for this or similar issues. That being said, I'm curious about the HPV theory so I'll answer your questions. By the way, did you come up with those based on some list of symptoms related to HPV?

      My background: have had symptoms for 2.5 months. Same as most on here. No STDs (standard panel) and have only treated with clotramizole lotion so far. Redness and soreness of glans and under foreskin. Keeping it clean but it's persistent.

      1. Sort of, I have more issues with voiding bladder and some dribbling now. Never had this happen before this aside from having UTI or being very drunk.
      2. Yes, definitely noticed a change to vein structure under foreskin and just today I noticed inflammation of a vein on my shaft. That's what prompted me to find this thread.
      3. Some discomfort, not really painful but maybe a little burning. At this point it's hard to remember what I used to feel like though. Definitely a dribble.
      4. I don't take very hot showers usually, so not a great data point. The redness is most pronounced if I am sweaty and unable to dry my groin for a period of time.
      5. Yes, meatus slightly more pronounced now and it was even worse during the initial symptom flare-up.

        6.The clotrimazole cream seems to have helped initially (been applying it for over a month now...), but it could also just be more regular cleaning/drying that alleviated the worst of the irritation and redness. I dont have the dryness issues some people on here have shown, but a little bit of wrinkling of skin. I am about to try Hydrocortisone starting today per doctors orders.

        Another symptom I have is very small purple dots, not raised, under the skin of part of my glans. I saw a picture depicting this symptom in an old medical textbook on yeast infections while googling about this but haven't seen anything else on it. I suspect it's related to the vasculature in the glans.

      I think its viral as well and literally everything you are saying is spot on but I test negative for both HPV and Herpes. I got it from oral sex in 2015 and it has its good days and its bad but as I am getting older it flares up worse when I am stressed. I have tried every imaginable cream including the expensive herpes and cold sore creams. Steroids make it worse. I can forget about it when its at a low point and then when it gets bad, I stress and try again with the doctors. I am at a breaking point at the moment as its the worst its ever been. I hate the rough scale look of my meatus and I believe urine makes it worse. I just pray every time I open this site that someone finds a cure. I will keep grinding and if I find something I will be sure to post it here. I hope everyone else does the same. I would post pictures but it literally looks almost identical to what Jason originally posted.

    Have had exact same issue for 6+ months now. All tests are clear. Seen multiple urologists + derms. Last urologist said looked within normal range and likely just chafing. Also said it was well documented in medical literature that these things are also induced by stress and anxiety after an encounter you might be shameful of. Also could continue to be irritated by various topicals that you may have tried to treat it with. Recommended all natural creams/lotions. CeraVe Healing Ointment. Derm said next step would be to biopsy, but my urologist strongly recommended against it based on what he observed (Harvard urologist specializing in sexual health)

    I personally think it's something viral. My lingering symptoms:

    -Red vein inflammation (could be caused by steroid cream usage in in that area

    -More pronounced veins (not sure what caused this, maybe the steroid creams as well

    -Rash on glans / near meatus

    -Change in my urine stream (split stream at beginning and end of urination) Feels like middle of meatus lips are not opening when urinating and is sticking together

    End of the day - doctors/derms/urologists will say it looks within normal range when all the tests come back negative...but I think we all know our bodies well enough that things are not normal. But also important to keep in mind that we are all probably highly stressed and anxious for months thinking about this, which definitely cannot be helping.

    I've been to another doctor too and keep say its nothing, this honestly looks like a circus.

    He just tell me to use moisturize cream totally random if I feel the necessity, but still I dont have a diagnosis.

    I'm honestly thinking that random theraphy would be useless, but I've tried everything:

    • 12 doctors
    • antibiotics pills
    • antifungal pills
    • antifungal cream
    • antibiotic cream
    • steroid cream
    • moisturize cream

    Zero results, I'm wondering if this could be a skin condition like psoriasis or lichen, but I need a proper diagnosis and I have to use a proper steroid cream, but the problem is just one, if no one suggest what I could have, how could I treat myself?

    I read here that could be a kind of HPV or Herpes infection, but I tested negative for both, so what now?

    DOES ANYONE HAVE HAD A PROPER RESULT?

      I saw a dermatologist who specialises in genital dermatology. He said it is a very mild case of Lichen Sclerosis. His credentials stack up as written books about all sorts genital disorders.. I'm not convinced as looks nothing like the pictures and what i have read about but thats what he said. All from oral sex. Good luck guys, so s**t.

      What is the name of the doctor who examined you?

      Because I too fear I have some very mild form of lichen, I also went to the lichen sclerosus experts and they ruled out this pathology, but I continue to have difficulty improving. Have you posted any pictures of your situation?

      Got an appointment with a genital dermatologist on Tuesday. Will let you guys know what they say. I'm not expecting much but need to try something as its gotten the worst yet. I am hoping it dies down again to a manageable point which it has always done over the last 7 years. Has never gone away, but there have been good weeks where I barely notice. Wishing for one of those weeks right now.

      Can I ask you guys something. Did either of you do something like wash your penis extensively with soap after you received the oral sex? I remember going in the shower and using antibacterial soap to wash like crazy because I am a little on the paranoid side. I know its almost comical but I am trying to figure out why this started if its not sexually transmitted. Lichen can start due to severe skin irritation and combine that with stress and there you go. It makes sense that its auto immune as it seems to follow my stress and health levels. I got pretty sick lately due to lack of eating regularly and only drinking caffeine and eating nicotine pouches until like 3pm every day. Damaged my stomach and lost a ton of weight. I have managed to get my health back by eating well and cutting out the the caffeine and nicotine but I did a number on my body. Everytime I get sick this happens and it makes sense that I was really stressed out about my weight loss and thought I was dying. All of my tests came back negative and the doctor told me I was an idiot for "drilling a hole in my stomach". While I healed my stomach, my penis is suffering from the continued stress of the situation.

      While I am not convinced its not some sort of new STD, My wife has shown Zero symptoms or anything in the last 6 years. I guess I am trying to find a common denominator that is not sexually transmitted. The only thing I can think of is the crazy scrubbing I did which may have really irritated my meatus. I saw another guy on here who mentioned he used hand sanitizer on his penis and it started after that. I originally thought he was crazy but my wheels have been turning and despite it being a long time ago, I remember doing something almost as dumb after getting oral sex from a girl I met at a casino that I didnt know. The sad thing is I thought I was being safe by getting oral sex. Oh the Irony!! Anyway its nice to get some responses and again I hope we can figure this out.

      One last thing. I am reading on Lichen and I recently started taking Aleve (NSAID) pain reliever for the pain in my penis because Advil was not working. Looks like thats a severe NO NO along with Advil. Wonder if it is why my symptoms have gotten this bad. Funny the more it hurt, the more Aleve I have been taking.

      I loose a lot of weight due because liver problems, in the past, at the end of the diet, I got a balanitis and after all good for all 2020, then started dryness after I used a fleshlight, but the worst after I used steroid creams. Doctors said that now I don't have a disease, I've been to lichen sclerosus experts too and said that I have absolutely nothing, that I had a terribly bad skin reaction to the dexamethasone cream and now I'm recovering, slowly, but recovering. Hope they are right.
