Blotchy red rash/irritated skin on penile head (lasted for over 3 moths now)?

Part of the skin on the head of my penis has become slightly red and is causing some pain/irritation.
It first appeared the day after I used a condom so I think I must have had a reaction to the latex or some other substance in the condom.
The tip of the penis became super sensitive and noticeably hurt whenever it touched my underwear.
The skin was and still is noticeably red close to and directly around the meatus. This redness gets worse and bigger after I've come out the shower or masturbated.
An hour or so after ejaculation, I get a slight burn/irritation at the very end of the urethra when I urinate and this can last for a few days before subsiding. This has put me off from masturbating as much now.
This has lasted for about 3 months now with no signs of improvement.
I saw a doctor and was prescribed timodine/hydrocortisone cream and this didn't have much effect and am slightly worried it may just make things worse so have stopped using it for now.
It almost seems as if the redness comes from within the skin rather than on the surface.
I don't think its balanitis as I haven't had that for over 12 years when I was a kid and that was with the usual smegma discharge etc, whereas this is just redness.
I've attached images to show how it looks straight after showering, which is when the redness is at its worst and the contrast between the normal skin colour and the redness can be clearly seen.

First post for a few years... been reading a lot of your posts tonight and it's interesting stuff but I still don't really know where to start and have had the issue for circa 8-9 years now. Here's a little recap:
I use the Child's Farm cream but can't really say it helps at all. I've tried a lot of the other recommendations. No magic cream seems to exist which I think we have all established that.
I'm 39 soon & it comes and goes and still bothers me but not too much. That said, I'd love to fix it. Mine doesn't raise really until I have sex or masturbate. It doesn't itch (although I am fussy about underwear) & it doesn't break or dry out. I drink a lot of tea and coffee but not much alcohol. I have had some back and neck issues and also bleeding gums but received treatment for it. I don't feel in bad health and am fit for my age - in 2020 I ran a lot and lost some weight. I don't think I have Crohn's but could possibly improve my gut so am going to try some probiotics.
I'm married and my wife has never mentioned anything or got anything so I think I'm being a hypochondriac but at the end of the day we spend every day with our appendages(!) and as I've got older the sex life has slowed down (not my choice lol!). I can completely understand how this can mentally affect you youngsters in the primes of your lives with meeting new girls though. Please don't do anything silly - you're not alone (clearly)!
I'm going to look into this pudendal nerve stuff too but I really don't think I have that issue.
Keep updating as it's helpful too all, I will aim to do better after my 2 year hiatus!

I have this exact issue to the T. Same exact looking rash on my penis. I believe it to have happened after receiving oral on the first date with a very promiscuous women. Have been tested for every STD possible. Have taken multiple courses of antibiotics, Have put every type of cream/lotion to try to cure the rash. Have done multiple urine and blood tests for infection. Have spent a dick ton of money and time with what feels like chasing a ghost. It has been close to 3 years and i have no answers. I am completely lost in what to do next. I have been told I have prostatitis and do feel pressure/pain in my prostate. pain in lower back area. Whether this is related to the rash I have to assume so. My two theories: #1 it is some super form of bacteria that she transfered to me through sucking my weewee and that bacteria has now worked its way all the way up into my prostate causing the prostatitis problems. #2 it is some type of nerve getting pinched in my lower back area that is causing the prostatitis symptoms, (i have read when you have problems in the region it will show symptoms in head of penis). The fact that it is this unknown thing and all these doctors look at me like i am crazy is amazing to me. I have come across a few forums online with this exact condition and no one has an answer. I made this account just to post this rant and pray one of you dudes finds the answer to this hell we live in. l refuse to live like this forever, i am still aggressively trying to solve this issue and will do my best to update this forum and others. please let me know if anyone solves this. My symptoms are frequent urination, dribble after urination, the rash has been there for over a year, and my prostate feels like a balloon and filled up. I am taking 14 days of bactrim right now as there was one time in this sy 3 years where I did have relief from a month on bactrim. I am going to see a different urologist next to get a new opinion, but I am not hopeful. This entire experience has shown me how sy the american health care system is. If you want something done you have to be your own advocate, it is just super demoralizing to never get answers. I am 28, instead of owning a house, I own receipts from medical visits and tests. Hi, long time reader first time poster- i think to potentially get a better understanding on other peoples symptoms and small wins which have helped in one way or another maybe we try gather some information about each other to see what crosses over ? i can start a few things
-If have a hot shower i get a dribble of what looks to be urine
Improvements -
i use to drink coffee but gave this up and it improved by 50% within 3 days
Wash with Fatty Cream - one that i use for eczema / dry skin
Look forward to seeing some trends

Sorry, late to this one. Would be great to hear if anyone has actually had much success in sorting these issues. Lots of mini tips reading back through the thread but nothing really breakthrough which is frustrating. With the background pandemic trying to sort this issue is a nightmare, keen to learn how people deal with the mental side of this.
If helps with the above - main symptoms are under foreskin. Skin looks irritated, patchy, shiny at times, horizontal wrinkly lines and purple marks on the rim of the glans. Sometimes can look fine, was painful now just sore. One bout of Epididymitis. No issues with urination or meatus. I pass all tests thrown at me. Have tried the following to no avail - Doxy x2, Co-amoxyclav, levofloxicin & mupirocin cream. Clobovate (maybe used for too long). Coconut oil. Still doing CPPS exercises.
Doc's just state non specific balanitis. One contact dermatitis but no real answers just what it's not rather than what it is.

hello I have the same issue been almost a year and still not resolved. Also gets insanely dry after masterbation like it crystallise over goes all shiny and flakey but once that goes just have red blotches of skin which gets abit sore.

I had the exact symptoms for 6 months. i tried every over-the-counter cream and nothing worked. I finally went to my dermatologist and she prescribed me hydrocortisone ointment twice a day. Turns out it was stress induced eczema it was gone in one week.

Hi I've been following this thread for a year now.
After lots of research I've come to the conclusion that it's pudendal neuralgia. With the possibility of mild seborrhoeic dermatitis. I have had the same symptoms since last April. There is no sign of infection of any kind. Blood tests, urine tests all come up clear. But if you guys have had a sore groin. Spells of frequent urination or spells of weak urination. With other weird sensations including sore tip It's probably nerve damage.
Search "neuralgia penis glans" or "Dysfunction in Glans Penis"
Your short term solution is to wash your glans with a fatty emollient. And apply some kind of pain relief like hydrocortisone or lidocaine on a night on your meatus (check with your GP first as it's not good to use long term).
The long term solution is to get your GP to look in to pudendal neuralgia as a possibility so that you can get this investigated and hopefully the appropriate therapy.

Thats my thought process too. I feel mine has been brought in my HSV1, as tested positive by blood for it. I have seen a neurologist/anaethesist who have agreed to prescribe preagablin and do a nerve block procedure to test the theory. Currently awaiting to be booked in.

I have been dealing with this for about a year now. This first showed up during a period of time when I was especially stressed at work and working outside in hot/humid conditions. I have always had high anxiety when it comes to situations when I have had too much to drink and the previous night is blurry and always convince myself that I have done something nefarious and somehow was unloyal to my wife. With this being said, following drinking one night, this showed up and I began to go down the rabbit hole of "what did I do?" and asking everyone around if I did anything "bad" the night before. The answer was obviously no, but my anxiety continued to rise until, boom, the rash showed up on my penis.
After consulting doctors and having multiple STD tests come back negative, I assumed I had a case of jock itch or something of that nature. I overloaded on gold bonds and jock itch spray for a few months to no avail.
Finally, I went to a dermatologist who suggested that this could be caused by anxiety and stress and therefore the body is just reacting to it in a "strange" location. After this visit, i took her other advise of wearing loose fitting cotton boxers, washing penis only with water, and otherwise just trying to keep dry and cool. This has helped TREMENDOUSLY.
So here is my theory. I have had odd rashes in the past due to my sensitive skin (foot rashes on top of foot from boots, deodorant making my armpits break out, etc.). I think that anxiety is somehow related to these breakouts and people who have sensitive skin are likely to have these show up in unusual places. Due to the location of this specifc rash, depression and anxiety skyrocket, therefore making the condition worse and worse.
I have noticed on the forum that there is a common theme of "all tests came back negative but I must have something". I did this as well. I believe that this thinking is what helps contribute to our problem.
I know that this is a long post, but I told myself that once I got this under control I would post on here because I know how much depression this can cause. Stay strong guys, just try to relax (as hard as that may sound) and trust the doctors. This is simply a skin condition and will go away with proper maintenance and treatment. I hope all is well and everyone gets through this.

so i saw you write on a couple different forums. So ive went through the same exact thing. its been pushing about 5 years now. i had a period of about a year maybe a little more that i had no pain , pinkness at times. but no pain. im 38. married for 11 years . been to so many doctors now its ridiculous. my doctors say its prostititis but i dont think it is. i originally took flowmax. helped for 2 weeks and it came hack. then cipro for 6 months . the first three months i was great . it came back. after that i was on antibiotics for 2 maybe 3 years straight . different ones of course. some would work but it would always come back mid cycle. i remember when mine started . i had an intense itch. it hurt when i used the bathroom rght after but it hasnt hurt urinating since.since then i have had periods of time that it wouldnt hurt at all, then ill go for several months with pain all the time. i guess my question is. has anyone found an answer to this . i know people havent responded in years but i would like to know if anyone got an answer that worked . i will say . the reaccuring times are not typically as bad as the first 2 year period

THE SOLUTION FOR SOME.
I posted more than a year ago. I even posted photos if you want to compare your case.
I also was through panic, antifungals, oils, dermatologies, doctors, etc for several months.
I just decided to post again my solution in case it helps:
To me it was solved when I started to pay attention to dry my gland with a towel or even toilet paper after masturbation or sex, specially some minutes after. The natural lubricant that comes out for a while after sex or masturbation is what irritates my gland and skin. I don't know why it changed since it wasn't like this before. I also attributed the problem for a long time to a one-night stand.
*That natural lubricant comes out for 5-10min so pay attention to that. It is not enough to make sure the gland is not wet right after.
Water is not a problem. washing products are not a problem. I am back to a normal life just with this extra rutine that is new. I´ve got use to it.

i am currently trying ti get an hpv test i think im going to do an at home one because no doctor will test me without warts or lesions
i dont think what we have has a name or is studied
i would like to share my story
i went in a depressive drug binge and had sex with multiple women and men some protected sone not i have the same rash on my tip snd a little in my balls i am cut
after the first std test i went back to havibg sex and doing drugs no ine i had sex with caught anythng from me
mine got worse almost everyday switched laudry soaps and body soaps underwear dryer sheets
here are a few thinga that have helped
i switched to a shae butter bodywash and it keeps everythihn smooth and moisturized
i used aquafor on my tip when it is sensitive and rubbing in my boxers that helped a ton with redness and irritation
i stopped doing drugs and having sex with strangers i dont thinj that did anything
also havnt masturbated in 4 months ( if i do it seems to get worse because of the semen) but it has helped alot
ive started working out and eating right lost 15lbs and that seemed to help alot as well
i cut all coffee and sugar from my diet
one thing ive seen that i havnt heard anyone talking about is artificial food coloring seems to trigger it if someone can test this out i dont jnow if its true or just a mental thing but it helps me
i hope some if yiu can find sanity soon it has gotten better but is still there good luck my friends i will post again in about 3 months or so with an update and any other information i find
