BPH Dutasteride Recovery Inconsistent

Hi Everyone,

Im 36 years old and have been diagnosed with BPH. my symptoms started about a week after a vasectomy. The urgency and incontinence were unbearable for about 2 months. long story short, I finally found a Dr who discovered I have an enlarged prostate. (Because of my age other doctors didnt think to look for it.

Anyway, Ive been on Dutasteride for 2 months and my symptoms have been doing better. i actually went 2-3 weeks where i felt "normal"! having said that, things have taken a couple steps back for the past week. have any of you experienced this- an inconsistent recovery? Thank you!

    No not yet and I hope not to anytime soon. what symptoms to you have other then to many bathroom breaks? prostatitas can cause inflammation. only asking because of your age.

      Its mostly urge to pee and light incontinence. its worse when sitting or laying down. Yes Ive had a PSA tests to check for inflammation. A Sonogram confirmed the enlarged prostate. it all started after a vasectomy which is strange.

    You might be suffering from chronic scrotal pain which is a nerve pain that originates in the spermatic cord nerve bundle that travels from the scrotum up through the epididymis and related plumbing and finally to the brain. Most urologists are not familiar with this condition so you need to request a consultation with a urologist who specializes in scrotal pain. Vasectomies have been implicated as a major cause of scrotal pain. The fact that you have an enlarged prostate is likely unrelated to this pain but BPH, if in fact you have it is also a serious, but separate problem and needs to be addressed though you have time for that one. I have both conditions as well and both can be managed well with the right doctor. Dutasteride is way too early a treatment for you and has long term bad consequences if taken for more than a year.

      Thanks for writing Howard. Do you think it could be this scrotal cord condition if Im having no pain? Ive felt no pain, only the urinary symptoms.

      I apologize - I thought you were also having severe scrotal pain so if you are not having pain then it is unlikely due to what I wrote. Have you been checked for a UTI? Prostatitis is also a real possibility following a vasectomy even if a urine culture comes up negative. I round of antibiotics like Cipro might be warranted for you. Also have you been put on anti-inflammatories? These are simple things to try.

      Yeah- I was on Bactrim for 6 weeks, Cirpo for 2 weeks and an anti-inflammatory for a while. good times had by all!

      You said earlier that the cystoscopy did show an obstruction - do you know what kind of obstruction? Cystos usually look at the condition of the urethra, the prostate, the bladder neck and the bladder wall. There can be issues with all of these that can cause your symptoms. There could urethral strictures, prostate median lobes that block the bladder neck, bladder stones, bladder neck obstructions, diverticula, severe bladder trabeculation which makes it difficult to contract the bladder muscle and pee, and internal sphincter muscle damage which could cause incontinence ( also external sphincter muscle damage caused by the cystoscope itself if it was a rigid one as opposed to a flexible one).

      It might be worth the cystoscopy and compare it with the first one. You could also have what is called a neurogenic bladder where the bladder signals do not get to the brain to contract and pee. The urodynamics test would study this.

      I still would not rule out spermatic cord nerve damage from the vasectomy.

      Finally if you can get it try one 50mg prednisone pill - not more! Prednisone is a dangerous drug if taken for a few weeks but if taken a few times is ok. As your doctor!! It will suppress any inflammatory-related causes to your problem so those could be ruled out if there is no improvement.

      Wow, you know your stuff. I did have a Urodynamics test and the Dr said everything is functioning together properly (ruled our neurological issues). I didnt ask what specifically the obstruction was. Thanks for your time and help. Its greatly appreciated!

    I personally do not believe in coincidences so if you were ok before your vasectomy then that procedure has to be related to your current problems. If I were you I would google "complications from vasectomies" and start your investigative research from there (if you haven't done so already). Vasectomies ligate or tie off a main tube that carries semen from the testicle to the prostate gland ( the vas deferens ) where it mixes with fluids to form sperm. It would seem to me that severing that tube upstream has caused problems for you downstream in the prostate.

    So ... maybe they could reverse the vasectomy and see if that fixes your problem!

    There is a theory called Gat-Goren that claims that BPH is caused by testicular varicoceles due to the high fluid pressure in the testicle that gets transmitted to the prostate and causes it to expand. Many men have had their varicoceles fixed to see if it helped their BPH ( I am one of them). The results have been mixed. In my case it did not help but it shows how the whole system is closed so cutting a tube in the testicles can affect the prostate. Good luck!
