Chronic Groin Pain

I am really pleased to find this discussion group as I sometimes feel that I am fighting a loosing battle. I have had severe groin pain for abour 10 months and no matter who I consult, GP to specialist or anyone in between there doesnt seem to be a tried and tested treatment for these symptoms. Despite all test, including STD's or  PSA's everything appears normal. The pain some days is excrutiating to the point that just sitting becomes a nightmare. Pain killers offer slight repreive but  I really dont want this to be the only solution...any advice would be welcomes..

Just as the nerve supply to the end of the penis is different from the root... the nerve supply to the scrotum is also different - the back part is very different to the front.

Which part of the scrotum is painful... back or front or both?

I forgot to mention... you didn't say whether the left or right side was more painful...?

The scrotum pain is more towards the front of the scrotal sack and at the base of the scrotum..quite difficult to distinguish between the left and right side pain levels..there doesnt seem to be a huge difference between the two sidesbut at a push I would say the right side is slightly worse...sometimes just sitting pushes the pain to unbearable..and finding the right position to aleviate this pain requires a few postional changes...

So are you saying that sitting aggravates the pain in both the scrotum and the penis?  And is the end of the penis pain also very slightly more on the right?

It seems that sitting does make it worse..I have tried wearing different  types of pants and underpants as the loose fit doesnt aggrivate it as much, with the scrotum hurting more in the sitting position. The pain at the end is more cylindrical as opposed to left or right...If the pain does become unbearable I tend to stand and walk around a bit for some relief. not always easy when eating out...I take over the counter pain meds from time to time offering a very slight improvement..but not lasting at all..and not a solution that I would like to rely on long term..

It is possible that your pains are neurological - ie referred pains and not an infection or a disease.  The nerves that supply the areas of your pain would typically cause more pain sitting; affect the parts of your body you mentioned - ie the end of the penis and the anterior portion of the scrotum.

I would consult a neurologist or pain  specialist and suggest the above.

Thanks for the advice. One of the most astounding statements I read when I first started researching this condition is to be prepared to go from Dr. to Dr. in order to find a solution that works for you...it's a terrible journey to undertake as so little seems to be on offer as a long lasting solution. Are you in the medical field? What do you think is the cause of this conditon. I have read so many conflicting theories ranging from bacterial infection UTI's and STD's..all very confusing..

Yes I am, but I am not a urologist.

I regularly see patients with pains in the groin/testicle/anal/vaginal/perineal regions and have a lot of experience in both diagnosng and, when appropriate, treating them.

The only symptom you mention which I am not familiar with is the feeling that your pubic hair feels like steel wool.  I habve not encountered this before.

My suggestion is to do as I advised in my last post and investigate a neuological cause for your pain.

Thanks so much for taking the time to answer my queries. I feel desperately alone in this journey and dont really know where to turn. The problem is that I live in a small coastal town where specialists are very scarce. I dont think that there is a neurologist in the area but will try and find one as close as possibe. Do you think that the pain could have been from an over zealous physical activity?

If physical then yes... but if you don't remember any particular incident then it is likely to be half a dozen or 119 - who knows - different tiny incidents that conspired to get you.

If left un diagnosed or un treated does the pain eventually subside..I am just so dreading going down another dead end road...nothing seems to help..

Unfortunately that is impossible to answer... until the diagnsis is made noone can  advise on time scale/results/outcome/etc?

Perhaps go for the investigations presuming that they will find nothing wrong - this will not build your hopes up so much, but another appropiate avenue of investigation has then been  done.

Hi I'm not a guy but some of the symptoms that you are describing are similar to the groin pain I get made worse during movement struggling to sleep and pelvic issues

Now mine were created by an accident I had a few months ago and as I had pervious sciatica but no groin pain a lumbar and scartal scan showed problems with my spine movement pars defect and prolapsed disc causing or compromising the nerves I have been reffered to neuro surgeon

Your pain like someone has suggested sounds like the source is somewhere else and radiated through nerves which supply sensation to those areas I would seriously think about going down the line of neural investigation with the possible source been in the spine particularly if you do suffer from lower back pain the pins and needles is a big indicator as I have alot of that

hi fozia. Thanks for the advice. This pain is really starting to get to me. One of the Dr's suggested that I try, with medication, break down the neural pathway that the pain has established between my groin and the brain. It was only moderately succesful and I was advised that there are many different medications to try if the first one didnt work...I guess that I am hoping for instant releif but sadly this doesnt seem possible...I have had this pain for such a long time that I seem to be taking it for granted that it wont go away...not a healthy approach...have you had this pain for a while as well..

Totally agree ... see a good chiropractor.  Avoid ones who sign you up for lots of sessions...  never go more than 4 or 5 times without seeing/feeling good results.

Same for physios - don't get ripped off - rememeber that their treatment is not usually of much use unless they are qualified to do acupuncture - physio treatment like ultrasound/exercises/interferential/heat/etc  is not recommended by the NICE or other official guidleines.

      Hi PainDoc

I would recommend that if you don't feel a difference after the first session do not go back they are not qualified enough or do not know what they are doing. This guy did not attempt to make me continue after the 12 booked sessions and had it not been for this accident I would not have had a reason to go back either
