Constant urge to pee. Burning at tip of penis. After bowel movement pee urgency increases. What is?

Hi there,

I'm a 23 year old male and I started having problems around 3 months ago.

It started after I finished an edging session (masturbating). I felt the constant urge to pee. I didn't give it much notice because something similar happened in the past, in a similar circumstance. Usually after a nights rest it will disappear. However, not this time around. I was visiting the toilet every 30-60 minutes. And always have the urge at the tip of the penile area.

I visited my local GP, they prescribed me antibiotics for a UTI as well as performing a urine test. I finished the course of antibiotics with no difference in my condition and test results coming back clean. No UTI or STD. (cephalexin 500mg).

My GP referred me to get an ultrasound done on my bladder and a blood test. And during the meantime prescribed me overactive bladder medication in hopes of reducing my symptoms.

The ultrasound was performed and nothing abnormal was found. No thickening of the bladder walls and prostate was normal walnut size. Blood test returned with everything normal. For OAB I was first given (oxybutynin) giving me heavy side effects, dry mouth, dry eyes, losing my voice, barely any saliva in throat, etc. Then alternatively given (solifenacin 5mg or Vesicare) giving me no side effects. However, neither of those medications gave me relief.

At this point I was referred to a urologist.

Seeing the urologist I explained to him exactly as above. That this started after my edging session. He dismissed how it could be linked with my current issues. He believed it was OAB. Giving me (mirabegron 50mg or Betmiga). He also put me on the waiting for a cystoscopy.

Still needing to visit the toilet every few minutes it was clear the OAB medications aren't effective.

At this stage I was really unsettled. I was really depressed about everything. 4 medications with no luck. I live a very active life. Doing cardio and resistance training for an hour each day. My BMI is optimal. My abdominal muscles are defined. I would perform core exercises and yoga as recovery. My nutrition is managed. No caffeine, no alcohol. Balanced diet. But with this issue I've had to stop exercising due to the discomfort.

So I went to a sexual health clinic. Providing them all the tests I've done beforehand to see what they believe could be the cause. Nonspecific Urethritis or NGU was a possibility. I was prescribed (doxycycline 100mg).

This also proved unsuccessful. Being 1 month and a half my symptoms sort of subsided some days. I was going every hour to 2 hours. But the burning and urge was still constant. Always at the tip of the penis.

I figured I would try some vitamins to see. I've had urinary tract support vitamins, hiprex for urinary tract infections. Still nothing.

After waiting what seemed an eternity (2 weeks) I was able to see the urologist again. He prescribed me (zithromax 500mg) 2 tablets at once. In hopes of getting rid of any infection or bacteria that could be causing me this discomfort.

This was also unsuccessful.

However, I noticed that during my bowel movement my penis felt burning. I would pee alittle while pooing and it would trigger my urge to pee even more. After I would finish pooing the urge to pee would be every 30 minutes. I believed it could be a prostate related problem.

I returned to my GP and explained what's been happening currently. She believed it could be prostatitis. She had me do a urine test again and blood test to measure the PSA levels. Prescribing me (trimethoprim 300mg) a 2 week treatment. I've currently entered my second week of treatment with no signs of improvement.

Now 3 months with this issue I'll explain my current condition.

The urge to pee is still constant however I can hold it for 3 hours or more sometimes. Going 5 - 6 times in a normal day.

I don't wake up at night to go, I can get 6 - 8 hours rest.

I've stopped masturbating for 2 months.

I can get erections and maintain them without issues. I feel no testicular pain or abdominal pain.

I've been performing pelvic based exercises every 2 days (kegel exercises), I average between 8000 steps daily (which is below my standard before this issue happened).

Cut out any foods that may trigger my bladder.

I've found the only times I get relief is after a hot shower.

When I'm seated, with my legs crossed over (sometimes).

When I'm laying down raising my knees to my chest.

Gently massaging the area between the testicles and anus.

These are the only times.

But now after I poo the urge is extreme. I do my bowel movement once a day. Sometimes I'll skip a day. And during that day I'll go 5 times to the toilet but have light burning & urgency. Bowel movement really triggers it.

    Hi Anthony,

    I have been having this problem since 6 weeks now after an edging session. Can hold my urine for 8-9 hours easily but once i pee, the urge comes back immediately. Did cystoscopy, blood and urine tests, RGU, bladder imaging... All came back fine.

    If you dont mind me asking, have you found a solution to your problem?

      I'm in the same boat.. had an intense edging session over the week

      and I haven't been the same since. constant urge to pee with burning sensation.

      how are you feeling now? did it get better over time?

    Hey Anthony, I just encountered the same issue as you 3 days ago. I did a long edging session of masturbation and after orgasm a few hrs later I had the constant urge to pee with burning at the tip. Now it's been 3 days and nothing has changed. After reading your experience I have a feeling we have messed up our urinary track & prostate. I'm not going to the Dr bc it seems like a lost cause after reading your experience. I have been drinking Nettle tea which has been proven to help the prostate. How are you feeling now? do you still get that burning sensation and urge to urinate?

      After just 3 days, I wouldn't advise you to self diagnose so soon. Hoping you resolved your issue, I am having a similar one for 3 weeks.

    This sounds exactly like pelvic floor dysfunction, the key thing I notice is your onset of symptoms after masturbation. You should NOT be doing kegals. They will make it worse. Try reverse kegals to relax the pelvic floor

    Not sure if you are still here OP, but this is exactly what I have been going through. As others have pointed out it could be pelvic floor dysfunction. My issue was similar, constant urination with urgency, at some point I believe I may have either contracted a UTI or my pelvic floor muscles were so messed up that I wasn't completely voiding and the foods I was eating caused bladder inflammation.

    I had a virtual visit with my urologists nurse last Friday and she suggested a PT, I have an appointment tomorrow and can tell you if it helps.

    I'm not a doctor but I think following a IC diet until your issue resolve may be a good thing (I wish I had known this as it could have avoided a lot of pain for me). Please do not do the kegal exercises, the first urologist I saw (horrible person) recommended I do them without really testing me for anything, like you I started doing them all the time and it made things much worse. My issue is a little worse than yours (or perhaps further along) since I have rectal pressure (along with urethra discomfort) all the time as well now, when going to the bathroom to urinate or poo, do not push, try to let the urine and poo come out with as little straining as possible, in my case the pushing was making things worse. Do the cystoscopy to rule out anything else, my urologist found inflammation throughout my bladder and urethra and put me on steroids which helped reduce some of my symptoms. Hopefully both of us can overcome this issue.

    Hi everyone,

    Its been sometime since I've last posted any sort of update about my condition. When I was initially searching online for help I would always get annoyed at people disappearing after doing tests and never providing an update. So I'll do mine.

    I'm now 25 years old and somewhat able to manage my symptoms. I can't lie and say I'm cured but I'm better then what I once was.

    Picking up from the original post. I took many more medications and even at one stage antidepressants in hopes of relaxing the nervous system. This was probably my worst experience and I wasn't able to complete 3 days of treatment. I was expected to complete 1 month. The side effects were horrendous. I felt blank. It temporarily distracted me from my urgency but created other problems. This was also on a low dosage (30mg) from the top of my head.

    I attempted electrical acupuncture in hopes of stimulating any nerves my bladder was getting triggered without success. Even tried a full 2 months of chiropractic treatment thinking something in the lower spine was creating false signals to the nerves around the bladder. Still nothing. Even homeopathy.

    I visited another urologist and they were considering performing a urodynamic which was the last thing I could've done. From the way he explained the procedure I really really didn't want to do it.

    So I reached out to some pelvic floor specialists. The first one was more exercise based. Got me doing exercises straight away and didn't really believe me so much when I told him my condition. I performed all the exercises for a month and if anything my symptoms became worse for some reason.

    I went to a different pelvic floor specialist and this one was interesting. And I apologise for being so blunt. She stuck her finger up my anus in attempt to feel my pelvic floor muscles as I performed a kegel. Whilst having an ultrasound machine near my bladder. So you could see the muscle contraction and she could feel the tension.

    Her suspicion was that my muscles were too tight. Overly tight. However I didn't feel them tense at all. If anything I felt that area quite relaxed. She wanted to continue to perform tests on me each visit however life was becoming very very busy and I wasn't able to visit her again.

    That was the last time I spoke to any professional about my condition.

    What I've learnt and currently do:

    • A large portion of it does become psychological I can't deny that part.
    • Don't stop masturbating cold turkey, I had to do it once a week. I found instead of laying down I have to do it standing up to minimise my symptoms afterwards.
    • Taking daily vitamins (magnesium and vitamin e b12) its somewhat helped relax my body.
    • Getting on with life as much as possible to get over the mental hurdle. Keeping yourself busy to distract yourself. Even slightly remembering a toilet would trigger me to go.
    • After voiding make sure to gently squeeze from the testicles towards the head of the penis so no drops remain in the urethra that could cause irritation. Even gently pressing the taint area just underneath the testicles to ensure everything's out.
    • When laying in bed and I have urgency ill massage the kidney area and behind the hip bone near the top of the gluteus. Not a circular massage but one of pressure points. This helps minimise my symptoms.

    One thing for certain. Edging is fine if you do it once every so often but make sure you climax. I made the mistake of edging and edging without release. I believe this played with the nerves and muscles surrounding the bladder. Masturbate with moderation and control yourself.

    Thank you for anyone reading this. We may not get cured but I think ways to manage it is the next best thing.

      this sounds like a stubborn uti. probably infected yourself from masturbating or unprotected sex. you should take uti antibiotics plus general antibiotics through IV. there was a time i had your symptoms and went through tests and the drugs you took including zithromax and nitrofurantoin. burning stopped but the constant urge to pee remained. i did get cured some weeks later when i was treated for food poisoning using antibiotics but through IV. let me know how it goes

    Considering all tests have come back normal could I suggest acupuncture.

    It may give you the relief you are looking for.

    i am currently going through the symptoms you have just described again. just finished a dose of levofloksasin 500mg 5 days and yet i am not cured. probably got re infected through sex or maybe masturbating and not cleaning up properly after or excess masturbating..dont know exactly the cause.

    some months ago i had these symptoms but with a discharge from my penis . i got some improvements with the discharge after taking some antibiotics zithromax, nitrofurantoin but still had the constant urge to pee. i did a

    urine test and urethra swab which came back clean after the antibiotics treatment.

    but what really cured me after was actually taking antibiotics through IV which was 2 weeks after my last tests. i had a bad case of food poisoning and got treatment for it using antibiotics through iv. after treatment i discovered i no longer had the urge to pee.

    my appointment with the dr is in a weeks time. i will ask if that treatment can be repeated again. dont know if i my request will be granted. or if i will get a relief. maybe i can also get the name of the drugs. wish me luck

    Firstly, thank you for your story it was very well described and very help full.

    I think I might have some help, and believe it might be related to kidney stones.

    I was experiencing nearly the exact same thing. What i felt was just as you described, and also become noticeable after masturbation.

    I felt tingling in the head of the penis, and the need to pee far more often. Then about 2 days later I begin to feel pain in the stomach, go to the hospital after a CT scan fine a kidney stone.

    What I believe we might have been feeling were very small crystals (not quite yet "stones").

    This I think solves the tingling in the head, as well as the frequent urge to pee. The body produces more pee to try and flush the stones/crystals, while the crystals get stuck in that last drip at the head evaporating causing the tingles.

    This is only my logical drawing of conclusions, but I hope maybe it helps as your story helped me.
