Extreme anal fissure pain

So I've dealt with anal fissures before that went away on their own but NOW it's much worse a couple of years later.

I am in SO MUCH PAIN - I've been considering the emergency room. I'm hoping I can get an appointment first thing Monday morning because I'm desperate.

I've used everything from Wheatgrass cream to my dad's prescribed ointment he used for hemorrhoids to baby wipes to Neosporin for external use. I'm CRINGING in pain. Can't sit or lay down in certain ways...

Will I be able to tell the doctor I want the surgery? That's all I want. . Just knock me out and get rid of all this.. please help!!!!

    Everyone! I am healed with my prescribed ointment AND miralax!! Thank the heavens!!!!

      So very glad to hear.  I  myself am doing ton's better.  Miralax is a wonder.  Keeps everything moving.  I will never change my diet either.  I follow a strict high fiber, low fat, no meat only fish, no pasta's, no rice.  Only fruit and veggies.  I am very creative with my food creations too.  Should start my own Anal Fissure Diet show on the Food Network.  LOL  Keep going Strong Julia!!!

    I know exectly what you mean .Try sits bat for 15 to 20 min afterBM.

    Try to do enemas if you can for few days to five your anus a break .You splinter is in spasam that is why is hurting so much .make suposatories at home from coconut oil and Basil essential oil Try to alternate between 3 warm aplications and one cold .if you sit at work use a heating pad to bring more blood flow to heal .This one is key after the sits bat dry your area with a hair dryer on cold .The anal fisure dont heal because of moisture.And dont use soap down there .It dries your skin so you are more suseptable to tear.Anal fisure are coaused by stress try some relaxation to help relax the splinter.

    i have been experiencing severe pain for 4 months. With Covid the doctors only offer telephone appointments. it took 2 months to get a referral, but the earliest appointment was September. Everything prescribed has had no effect. it all started after a colonoscopy in February. My life has been on hold ever since. Hot baths and ice packs give some temporary relief, but you cant spend all day with an ice pack up your backside or sitting in a bath. its horrendous. Its incredible there is nothing they can offer for pain relief, and to be made to wait 6 months to be seen is torture. I am sure this has lead to people taking their lives as its a totally unbearable. Thankfully my constant phone calls have got me a cancellation and i have now had an examination. It's a fissure, so now on Diltazem for 4 weeks. This should help relax the sphincter. I thankfully have no gallbladder, so soft motions are normal for me, but the pain after a soft movement is ridiculous and lasts all day. The fear of an operation and being left incontinent fills me with dread. I hadn't heard of the Botox option, so that sounds hopeful and I might get a nice looking pouty bum lift in the process. I have already spent a long time in hot baths over the past 4 months, sometimes hopping in and out several times a day, but that made no difference to the healing process. I am not convinced this cream will work. if its all about encouraging blood flow, why hasn't it healed in all these months of sitting in hot baths.

    My diet is fine. I eat a lot of fruit. No gallbladder, so soft movements thankfully, but if anyone has any advise for pain relief that would be very welcome.

    3 weeks to go with the cream, but no change yet

    Julia, I too suffered from a chronic anal fissure for 13 years. Over the years the most relief was from using Beadreaux Butt paste. That didn't heal the fissure , just gave relief.

    I believe that I may have found a cure that is actually healing the fissure. I began this treatment only 11 days ago but I no longer suffer pain and the blood spotting has stopped. I can now clean the area pain free for the first time in these 13 years.

    This is what is working for me;

    Buy a jar of vaseline and a packet of paper napkins (mini pads will work in place of napkins).

    clean the area gently

    apply a thimblefull of vaseline to the area, insert some if possible

    put the napkin/liner over the area inside underwear

    Repeat the above steps at least 12 times each day. Continue for 4 weeks.

    Julia, I am convinced this is healing my fissure. I got this idea after having recent skin surgery to remove a cancerous growth. The surgeon had me keep the wound covered in vaseline for 2 weeks.

    I would consider surgery only as a LAST RESORT.

    I'm right in the middle of 7th circle of hell right now. Last night I was able to get some sleep around 430am and got up at 7am.. So glad I found this forum.

    Background: First anal fissure I probably had a year ago. It felt like small peanut with extreme pain, but with Diltiazem, sitz baths, and Metamucil fiber I got better and I was fine for a year. (no med exam)

    It was all good, till about a month ago. I ate a lot and the stool was solid and frequent. Normally, I don't have solid stool but it all looked very healthy. Anyhow, I started to feel the stubbing pain during bowel movement on the same place.. and shortly after the peanut blew up and I had blood in my stool. I freaked out.. but the pain after that was minimal. Every next bowel movement I would see traces of blood and I got worried. So, sitz bath and Diltiazem worked great for me. I had pain during BM, and shortly after.. than I was fine for rest of the day. Still, traces of blood were there .. and my wife panicked "oh, that could be something serious". Talked to my doctor, and she says the blood is bright nothing to worry about.. I pushed and asked for a specialist opinion. I've got first call for a doc that does colonoscopy and explained to him my case. He said oh you are 50.. you are in high risk, I had colonoscopy 9 years ago and it was perfectly clear.

    So, the doc insisted on colonoscopy and he scared me telling me that that anal fissure could come from cancer and that I am in great risk. I told him I'll do colonoscopy, but I'm worried that fissure will be ripped even more since it's just inside. He said do not worry, we will be carefull we'll exemine you lets rule the bad news out.

    On March 9 I had colonoscopy, when I woke up the nurse gave me flyer .. how to treat hemorrhoids. Of course I was ecstatic to have such positive outcome.. and I sad wait.. how about my fissure?! The nurse told me there is no fissure. I was so confused... as I explained to his team exactly where it is right before the procedure.

    Great, colonoscopy done, I'm all good, love life. Two days after colonoscopy the pain scaled up, traces of blood. First episode of insane pain for 8 hours. My doctor got me in office, and after 15 seconds of the exam she saw fissure. She says, she saw many like that one, and bigger. Fiber, sitz bath and bla, bla.. I explained her the pain again and over the counter drugs do flat zero to me. She prescribed me morphine(thanks God).

    From that point every day was worse. About a week ago, the pain and the tension went next level. Extreme pain would last at for hours, and the pressure would stay permanent. I was fasting and using laxative.. living on soup. My stool once a day was more like bird stool. No blood.

    The pain:

    The pain when extreme was like striking needle on the spot. I would be yelling, screaming.. cannot sit, cannot walk, cannot lay down, cannot stand.. "The needle strike" would go one after another one and it's inhuman to put it mildly. It is more like some nerve got exposed and only that can hurt so much. Im thinking pudental nerve(I don't have any dis-function). I would lay down and get up thousands of time.. a small pill of morphine helped me to say alive.. but even under morphine the pain would be there.

    Anything I use any cream would inflict more extreme pain.

    The worse thing is that warm sitz bath would hurt me even more.. in seconds. Any ointment, water, suppository would lead to disaster. Diltiazem would not do anything, and used to be my superstar.

    Right after I get some sleep the pressure would be there and slowly increasing.

    Of course I asked for a specialist.. that's pending.

    Pelvic relaxing exercises help a bit.. sometimes.. 5%... The pressure I guess sphincter on the left side would create tension that would steadily go up, and it's always there. Last 6 days I'm in hell.. don't have any silver-lining.

    Is it possible that colonoscopy induced more damage to my fissure.. ? Just cannot believe the doctor did not see during the procedure.. He destroy my live.

    I'm not depressive individual, I can take the punch.. but this amount of pain and inability is beyond human capacity.

    Current state:

    Waiting for the specialist, I was told maybe two weeks .. my god.

    One specialist responded on e-questions from my doc and says to go to see colonoscopy doc again.. and I just demanded another doc. WTF... We have a colonoscopy report and images 20 days ago. For the weekend he prescribed Lidocaine cream to freeze the wound... and that was absolute disaster .. led me to 4 hours of the needle shooting pain... I had to wash it and that was first time I took morphine. Jesus.

    I don't have a conclusion here. I couldnt find anyone to this level of pain. I read, and watched tons of material online regarding the matter. It's most likely extreme anal spasm .. that probably pulls the fissure and exposes some nerve .. maybe?! The brain really squeezed things down there.. or maybe the muscle went off with spasm... dont know.. The hemorrhoids from the colonoscopy were very minor.

    All I can say, I tried everything.. there are good videos from dr. Chang on youtube .. new content .. "your friendly proctologist.. watched everything many times.. and I did everything right and much more than he explains.

    Sorry, for disorderly post.. haven't gotten any break last five days.. hope I won't lose my mind. Made like six breaks writing this post.. laying down putting legs on comp. chair and did some breathing.. Pretty unusual for the guy that last 50 years used advils maybe three times in life.. never been sick, operated.. well .. I'll keep you posted.. or maybe I shouldn't?
