I think I have a stricture

I have been having bladder problems since May of last year. I had a slow flow and 10mls of residual urine left in my bladder after a uroflow test. Urologist was concerned so he sent me for a flexible cystoscopy with a local. I had the cystoscopy and it found nothing. After the scope the tip of my penis was very swollen. I'm not sure if this is normal. It lasted for about an hour and then went back to normal. It did not burn to pee and there was no blood. 3 months after the scope I noticed that My urethra wasn't emptying urine fully. This went on for another two months and then I noticed that when I urinated, after I finished it felt like a sting/burn about 1/2 inch down from the tip of my urethra. I now have to milk my urethra to get all of the urine out of it. Strangely my flow has improved. There is no burning when urinating unless I really strain to get the last drops out and then I'll get a searing type pain in that spot. I went through a two week phase where I was getting stabbing pains in that area of my urethra. I also had pins and needles in the tip of my penis. When I sat down it felt like there was something stuck in my uretha and I could feel a type of pressure. I have no pain when I urinate, no pain with ejaculation. Ejaculate force seems ok, though I have to push out semen after I'm done as sometimes there seems to be a fair amount left over in my urethra, yet if I squat it seems to help. Strangely there is none is my perineum, mainly in penile urethra. Here's what I think is strange; after I have ejaculated, if i wait a while I will leak quite a bit of see-through post ejaculate. I always know when this happens because when it goes past that certain point I will get a burn, sharp sting. I know it's happened because As soon as I look at my penis the post ejaculate is there. I think I have a possible narrowing when the gland meets the urethra. I've spoken to others who have strictures and they say they don't have these symptoms. So I don't know what to do. I don't think an RUG would help due to the location. Maybe I will have to have another flexilble scope to check? What do you all think?

have a urethrogram and it will show the exact location and severety if you do have one, i had one done the other day and i have 2 strictures one penile and one in bulbar, had them dilated last year and everything is looking good. 20ml per second flow. iv never struggled with flow but do struggle milking all urine out after going the toilet, its more an inconvienance than a major problem

Hi. Do you know what caused yours? What symptoms did you have? I don't struggle milking the urine out, It's just strange that I have to do it. Sometimes My whole penile urethra can be full of urine, and I'm not talking 1-2 drops more like 6. Other times there's only a few, and sometimes there's none. I will be very annoyed if I have developed one from a procedure that was meant to check for one. If that is the case then I must be highly susceptible to scaring in that area.

i was born hypospadias but was very very minor and at the time they must have put something down the urethra, very common to develop strictures from these repairs in babys. Anyway in 2008 when i was 17 i passed blood, i had a cystoscopy and then a stricture in bulbar was discovered, was unaware all my life and didnt cause problems but they cut it (urethrotomy) then catheter in over night which wasnt expected and lived very normal problem free life for the next 10 years, i got a uti out of the blue about 18 months ago then i started having problems. another cystoscopy reveiled another stricture in penile aswell as the original one in bulbar, also my urethra opening is narrow, had both strictures balloon dilated about 12 months ago, catheter left in for a week and on my latest flow test i was 20ml per second, 50ml left in the bladder which doctor was happy with, also on urethrogram there didnt look much narrowing. So id say i pass urine fine, i do get little pain in pelvic area sometimes, but the biggest inconvenience is after, i have to milk urine out, i also dab opening with tissue, press on perinium and il get a little more out and dab with tissue again, if i go back and look 10mins later there will be another drop there, so its only a tiny amount but it annoys me and dont like to use urinals because of the excess milking

Sorry to hear that. Sounds like you've had quite a rough time. So they caused the bulbar stricture? or is that just down to your hypospadias? Hmm, I have the same problem with the milking and that's worrying me. I never had it until 3 months after the cycstoscpy which showed nothing. Strange how it developed after that. My tip of the urethra was also very swollen after I had it done. I have read that swelling to the urethra can cause a stricture to form. Sometimes when I go there will be a lot of urine left over in my my urethra, but other times there will only be a drop or two. It's the same with ejaculation. On the first bit it's comes out fine and can have a decent force to it, but then the last bit just seems to stay in my urethra. I'll completely milk it out and then I'll leak post ejaculate for ages after. Sometimes when the post ejac comes out the tip it feels like razor blades. There is no burning with urination at the moment. I really done know what to do because if I do have a stricture then it will be from the cystoscopy. Where I am from they don't do the RGU. So I will have to have another scope to check, and if I have developed one from that, then I am at more risk from more scopes. My stream was 12mls/sec even before I had the scope, but it does seem to improve and then it goes back down. I'm sick of it tbh. Do you know how long a stricrture takes to form? Could one form 3 months after a scope?

Did the Uro give you a reason as to why you retain 50mls of urine even though you have a good flow?

the bulbar stricture would have been caused when i had something inserted in the urethra as a baby. I believe when i had the cystoscopy 18months back after the UTI it made things worse. It cant be good to have things put in there time and time again but if you arent happy with the way things are you dont have much choice but to try things, are you in the uk? i have also seen a reduction in ejaculation force but experiance no pain at all and have normal sexual function. not sure how long strictures take to form but as soon as i got the uti things were never the same again

he said anything under 100ml he was happy with, i did 2 flow tests one i held 30ml other i held 50ml.

So if they come back, what is the next course of action for you? I am quite surprised that dilation has worked so well for you. It's good to hear that it has, though. Yes, I am from England. Our national health service is a bit like a post code lottery. I would much prefer to have a RGU. None of it makes sense to me. I developed the drip 3 months after the scope. But it also happened on a day when I felt like I wasn't fully empty so I started to strain a lot. Then it just got worse. Maybe it's a stricture, maybe it's my bladder neck, or maybe I have caused my pelvic floor muscles to become hypertonic along with my external sphincters. Or maybe it's a stricture that's causing me to having to use my pelvic floor muscles to push even more. I also take a while to start peeing. I don't know if that's psychological. When I do begin peeing I don't feel like it stop or slows down before where I think the stricture is. But then why would I get so much urine stuck in my urethra? Shouldn't it completely empty when I'm done? Something that is very strange is when I ejaculate, if I stand up then it's a much reduced force and there is a lot of it left in my urethra. If I do a sumo squat while doing it, then it seems a lot stronger and I have minimal ejaculate left over in my urethra. I'm really annoyed because I feel like, as much as I appreciate the NHS, I just feel like everything is taking so long! If they got me in for RUG, or did it where my bladder is filled and I void - they would see what is going on. This is taking over my life and has done for 1 year now. I know I shouldn't be moaning to you as you was dealt a s****y card when you was born. I wish urology was more advanced so they had treatments that worked really well for all of us. Maybe I should try a bit of dilation myself on where I think the stricture is. I know you can buy Sounds off of the internet.

some of our symptoms do sound similar, i dont feel much difference from before the dilation operation, i still pass urine well however i still dribble or have to milk, i still get slight bladder pain sometimes. so not much improved in terms of symptoms but my uretgrogram looked good, my bladder emptied fine with good flow. im doing my best to just live life and try not let it take over. If things worsen or strictures get worse i will probably have urethroplasty, however im in the north west and my urologist says there is a specialist centre that is in birmingham where he was trained up and this is where he will send me for further treatment, im guessing the guys there are the best of the best

From my experience a stricture doesn't give pain when urinating or ejaculating.
10mls of urinary retention is nothing. Or did you mean 100? Even then, it isn't unusual for someone 60 years of age to retain 50-100mls.
Think you better go back to the docs to check on the pain.

That was before I had the cystoscopy. The retention back then was 10mls. But 3 months after the scope I had to start sitting to pee so I can fully empty my bladder. When I had the scope my urethra was very swollen for 2 hours. Swelling of the urethra can cause a stricture and I think my urethra was a bit too small for the scope. I often have lots of urine left over in my urethra, even if i push with my pelvic floor I can't expel the urine. I have to manually milk it out. The penile urethra can be completely filled with urine. I'm 33. I have written to the urologist. I've spoken to lots of people who have had a cystoscopy and none have ever had swelling like me.

check out serrapeptase , homeopathy for urethral strictures - thiosanaminum and aloe meatal dilators from hunter urology online .. before you consider urethroplasty near tip of penis . . although the guy in birmingham is , ive heard, an expert in this field . just be sure to ask about the risks and make an informed decision ..
