Is this a case of fissures?

Hi guys, the concept of getting my rectum examined greatly embarrasses me and I have been hesitant about getting the help i needed.

The anonymity on the internet gives me courage and hope I get some information to see if I should get professional help or not.

I have been suffering from anal bleeding since 2015 but it goes on and off. It is accompanied with constant itches and wetness which is discomforting but I deemed it as not serious. Over the past year I have been monitoring the situation of my rectum by taking photos, and recently (couldn't have been more than a week) noticed that a small lesion has been developing.

I do remember extremely painful sensation (like someone putting a knife) during bowel movement which would remain sore for a while. I'm not entirely sure if this has got to do with Hemorrhoids or anal fissure. I will attach a before and after image (I do apologize for you guys having to see the image). Recently with the lesion, I realized that the discharge from my rectum has worsened in terms of odor, and now smells very fishy.

Any help is greatly appreciated.
