Is this Balanitis? I'm 21, had it most of my whole life, no treatment has worked

Hello, I've seen a doctor many times about a mild condition I have on my penis glans, have been prescribed 3 different creams including Betnovate and a Hydrocortisone, a course of antibiotics and a swab which said I was clear and still the condition remains. I used to have a tight foreskin up until last year which is now sorted from stretching, and I think the condition began during my teenage years when I wasn't able to wash the glans. It often feels dry and can sometmes feel slightly painful when the foreskin retracts, almost like its rubbing and the sensitivity everywhere is quite high still, not sure if this is due to the condition. The urethra opening is always red and inflamed and red areas appear frequently around the glans itself. Any advise on what I should do would be much appreciated, have attached a photo too. Many thanks!

I got mine 9 months ago from masturbating to the point where my was producing lots of smegma/dry. the rash was widespread but clotrozimol got it "under control" Anti fungal creams (ive come to trust Nyastin mainly, though before that Clotrizamol was okay). I have a red patch on the front which seems to shrink, at, best from Nyastin, which is mainly good for stopping redness from spreading over the entire penis.

At one point, maybe 6 months ago, i didn't use creams for a couple of days and i had a worrying outbreak and got prescribred one dose 150 flucozomole tablet (spelling may not be correct). Over 4 days i watched it all shrink back in joy, but it stopped, and some of the big rashes that had come up, pushing me to go and get such treatment, seemed to still exist on the penis as small red dots. They are still there now. I went and got more flucozomole tablets but these did nothing. They were 50 size dose tablets, one a day for 2 weeks. I might try another single 150 dose soon.

I've mostly been keeping it like this with nyastin until, for an unrelated foot mole concern, I went to a dermatologist, who also looked at my penis rash. I tried a potent steroid cream called Advantan, which seemed to work on the first day (penis suddenly almost looked normal) but the effect stopped over the week that came and redness actually spread all over the penis and i had to use nyastin again to get it under control. I am having a harder time keeping the redness minimal now since then, i don't know if that's due to skin thinning by Advantan or if it's just because the rash infection itself just had space to flourish. i went on 1% hydrocortisone last week - first three days were amazimg results, the central core of the infection which seems to the the front red patch like OP's pic was being wiped out. Then i had a b*****b and the redness/irritation all of a sudden got kind of bad. I thought i could just apply more hydrocortisone but it seemed to now just be making my penis redder immediately after use, but then i realised this all went away after showering and then my penis looked close to perfect, and then one application of hydro left a burning sensation with the redness. i'm going to take a short break from hydro and treat with e45 shampoo and moisturisier and other things suggested in this topic. it was just weird having hydro instantly making my dick red only then for that to wash off in shower leaving it look almost cured, any insight into whether it would have been safe to continue will be appreciated.

for me mate it been exposed for a long period of time acc helps me, ensure you dry your penis as well as keeping it dry really does help ! that means after you shower leave the foreskin pulled back for 10-15 minutes. after you wee wipe the end. And for me sex has gottena lot better, i can actually have sez now and it not hurt like a b***h. However recently after using this other cream the tip of my penis had gotten red and i have had dischrage. Back to the doc yet again on friday goah its annoying

My Balanitis wouldn't go away until i started using a cream 3 times a day called Cetraben. The skin began to heal and the itchiness disappeared.

yeah ive had this for years and it sucks. ive always had a fairy tight foreskin, to the point where i went and had the frenulum taken out (which made life better). one thing i will say is ive undergone a great deal of stress and anxiety over the past 1-1.5 5 years and i think thats probably made it worse. the past 2 weeks have been savage, ive just been cleaning it day to day with luke warm water. i sit here and think ive got some kind of STD but ive been tested and its all come back negative. it comes and goes, the more i stress and dont sleep, the worse it gets.

ive never put any anti fungal cream/steroids or whatever on it, and when it flairs up i stay away from spicy foods and touching anything with irritants (car oils/fragranced stuff). I quit smoking at the start of December and went on to vaping. it started coming back again - im wondering if there some chemicals in the vape that might be triggering it. nevertheless im going to stop for a week and see how it goes.

I have lost patience with this problem, after having it for 4 years and seeing various doctors on the NHS I am left with two options, one is a nerve blocker to reduce the pain/discomfort, another is some drug to do a similiar thing

I have always said I wanted a cure not just to mask it, so unfortunately I seem to have hit a dead end

The main things I had done were circumcision, which has helped but ultimately I do not recommend, various anti fungal creams/oral tablets, the creams helped initially but then started to aggrevate the area more, steriod creams which visually made it look better but never actually did, oral steriods for 4 weeks that did nothing at all except made me hungry. I did try a course of anti-histamines which did nothing, but my doctor wanted me to start a monthly injection which is much more effective, but also expensive so the nhs were not going to give it out easy, we're talking years on the waiting list

So, in all this time I have been putting savings aside for 4 years bit by bit, and now I have reached a dead end with the NHS I am going to go to harley street to see a dermatologist. These are some of the best dermatologists in the world.

I will let you know what they say

also just to say, the Urology department were adament that this is nothing to do with urology and is purely dermatology related

Hey man, have you been to the dermatologists yet? I have heightened sensitivity in my glans as well, and my urologist recommended me to visit a dermatologist too. Any updates?

hey, not yet as I actually need a referral letter from my gp first, so in the next day or so I should be able to arrange an appointment date, 1-2 weeks max

the guy who I will be seeing is a professor, dudes written books and specialises in penile dermatology, so if anyone knows of a solution its gonna be this guy

I have the referral letter, put it through now I am just waiting to get an appointment date, they should let me know tomorrow

best of luck bud , feel like we all vultures circling above your appointment for scraps of hope haha. Hope he can get you sorted !

OK lets meet here again in two weeks, or let us know when your appointment is done. Suggest to your doctor whether this can't be some sort of allergy. I'm looking this way in this phase of looking for cure.

My Urologist told me there is nothing I can do for you. You will have to figure it out on your own or find a different doctor. He took my money and I left and will never go back to him ever in my life. He was useless.I guess he could have kept guessing and prescribing so maybe it was in my best interest he gave up instantly.
