Little worried. Spots on penis head, that happen every now and agian. Any ideas what it is?

Is it itchy? if so then is yeast infection, you need to see a urologist.

yes. with my girlfriend. she has had thrush before a while back and giving it to me. but she has not had any symptoms of it. I said could be balanitis but she would listen to me. I have stopped using any soaps and such.

I also had a swab taken and it came back clear.

The problem with vaginal based issues such as thrush, or any yeast / fungal based issues is that most of the time there are no symptoms of these. Unfortunately, to some degree or another these are always present - it is when they multiply out of control that they can cause real issues for sexual partners who do not regularly use condoms.
It is all too easy to think that the issue has resolved - only, a circular action can happen and you simply reinfect each other.
When symptoms show up like you have in the picture get yourself some Clotrimazole (Canesten etc) and follow the instructions for dealing with genital based issues .......

I have been using canesten. now and before and it has worked well. is there a way to resolve the issue completely? I acknowledge always using condoms is a solution but is there a way to stop form happening over and over again. is is my girlfriend (who stubbornly think she does not have thrush/yeast infection due yo no symptoms.) is it her that is her that is infecting me?

Perhaps you could try condoms for a number of weeks, or non penetrative sex for a while? Simply, if you are getting yeast / fungal or even bacterial infections on a regular basis then penetrative vaginal sex is the No 1 suspect.
If you have been going to a sexual health service perhaps you could get some guidance from them on this .....
