Lump on big toe joint after gout

I am 36 yo and had massive gout attack just before New Year, i had 4-5 attacks over the past 5 years but this one was the longest and most painful.

I was taking painkillers and indomethacin and it managed to get rid of it after 2 weeks of pain and misery, however, there is lump left on my big toe joint where attack happened and i can not bend my big toe all the way like i can on my other foot.

I felt pain and gout didn't completely disappear.

I went to dr and he gave me prednisolone to take for 3 days and colchine (which i didnt take btw as there no pain per se) only when i press that bump feels kinda painfull.

I finished with 3 days of prednisolone and while it helped a bit the lump is still there and i can say that my big toe joint doesn't feel as healthy as the other one, its like 80% ok but not quite there and i always feel like it can get nasty

I am a bit worried about this, what can it be? it is definitely not normal compared to my other foot, What can i do to fix this, x-rays, medicine...

Im thinking of going back to my dr and tell him that meds didn't quite fix the problem but i would like to hear from people here who had this issue.

    Im taking colchecine, it makes me feeling weak, tired and depressed, i can sleep 10 hours a day and feel tired as,foggy brain.

    Anyone else had similar sides.

      I felt nauseous and generally rough but I found it did treat the attack, the only other options are steroid tablets or a steroid injection directly into the affected joint

      The thing with colchicine is to find the right number of tablets to take a day which don't make you sick, but I found this very much trial and error. The drug is after all toxic, so will make you ill at some point

      I have been prescribed colchicine 500mcg twice a day, small tabs but they seem to be working better than indomethacin.

      I dont know if its high or low dose, I have never taken it before.

      What is the BEST first line of defence for gout attacks, I have taken indomethacin and it didnt didnt stop it , I have to find right meds when so when I go on holiday i dont get full blown attack ruining my time when I should be enjoying myself. thaT is exactly what happened last time and I had to cut my holiday shortsad

      Any sugestions

      I personally find no problem with colchecine. However, during a gout attack your body is being flooded with inflammatory chemicals which may be responsible for your symptoms. But there are other medicines which will do the same job as colchecine - try them.

      You are correct - the dose of colchecine which will effect one person may not affect another. It is trial an error. Personally I'm okay on 2 and on 4 I get a bit of a tummy upset, but nothing drastic. But I find spread over 48 hours will do the trick.

      The only thing I have read in some research is that cutting the (tiny) colchesine tablet in half, and in your case taking it every six hours (same dose if you see what I mean) reduces sensitivity. At 2 tabs a day you are 50% below the level that most people get upset stomachs.

      As for getting attacks on holiday - do you drink more alcohol or sugary drinks on holiday?

      500 twice a day is a low dose

      defence - get your uric acid levels down using allupruinol or benzbromarone or Sulfinpyrazone 

      drink plenty of water, some exercise to lose a bit of weight - personally i've never found alcohol to be the reason for an attack - once you get your uric acid levels down you should be able to lead a normal life

      *Excellent advice.*

      However treating the hypereuricemia during an attack is not advisable.

      Remember 85% of uric acid comes from your own body not diet. Therefore slimming down is great advice.

      Yes i tend to eat and drink a lot more when on holiday, im start to think

      Sugar is main culprit.

      In regards to allupruinol or benzbromarone, sulfinpyr, its uric lowering drug so it needs

      To be taken daily to keep levels down.

      I stopped eating sugar and im starting to experiment with green tea, its healthy,

      It makes me feel good and its not heavy like coffee, plus its good for slimming.

      Im thinking it might help with keeping uric acid down along with cutting down sugar completly

      Anyone tried that

      There is one other major trigger of gout which could impact you on holiday. Trout is triggered by heat waves - or I guess someone moving for 15 days from somewhere not very hot to say Malaga and Cancun and drinking a few beers, sweating like a piglet and not drinking the extra litres of water ..... that could easily bring on an attack.

    I have had 4 attacks, and my big toe joint is constantly swollen, and attempting to bend my toe back always hurts. You just have to learn to live with it

      If your toe is constantly swollen you should get it looked at, as one wouldn't expect permanent damage so quickly.

    It has been 15 months since  gout attack and i still cant bend my toe downwards like I used to.

    When you compare with the other toe you can  see the difference in movement, lump on the side of toe is almost unnoticeable(but still there)  

    What are the options, xray, surgery... since medications failed to fix the problem?

    Hey david, Iam having four for the past 10yrs .. It has increased up to 11 % where the avg should be 6% .. Its purely depending on Diet ctrl and plenty of water intake.. And with medidation, But we will not follownit on regular basis.. Now my uric level is 6.5.. Hope it should continue..
