Pain after rotator cuff surgery

I’ve read some discussion ref the pain post op. My surgery was 2/13/18 with 2 anchors placed and bone spurs removed. The tear was one of the worst per the surgeon. My pain is still present 3 wks out. PT consists of passive ROM and at home I do pendulum swings n shoulder shrugs 3x/day. I wear my sling except when showering, watching tv and sleeping with my surgeons permission. I had my 1st night of sleep in my bed last night. Before that it’s been the couch. Recliner forget it. I’ve been taking Percocet 10 mg and 7.5mg every 4 hours. Requested my meds be changed to something else. Am I the only one having so much pain this long post op? Not to mention the pain in my neck literally from the sling.

     First of all I can’t believe your surgeon wants you to do “pendulum swings“ this soon!  You could really do some damage to your shoulder repair. But I’m not quite sure about his protocol. My surgeon did not want me to do anything for four weeks except for wear this immobilizer 24/7. Sleep has been nonexistent with excruciating pain. My surgery was January 30, 2018. I’m almost 6 weeks postop and just started last week doing stretches and hanging my arm like a wet noodle.  My doctor told me absolutely do not swing your arms. I think what happens is because the tear repair is approximately 15% per month, the rotator has not adheres to the bone yet. And by swinging that disrupts the repair. Can undo the repair.

     I did not take a shower until after my four week postop appointment. I was doing sponge baths with the help of my daughters. Now I’m taking the sling off during the day and resting my arm on a pillow and I find the pain is mostly in my elbow. But my shoulder does ache periodically. Last night I try to sleep without my immobilizer, and I was in such excruciating pain I ended up putting it back on so I could lay back on a 45° angle with pillows & a wedge and get a few hours sleep. 

     By the way I had the exact same surgery as you, I had a 95% tear, Dr said was shredded  with two anchors in place and calcium deposit/bone spur removed from the backside of my shoulder from a tear that happened 10 years ago.  

     Hoping to start physical therapy next week which will be my six week mark postop. I know everybody’s different, and every surgeon is different, but I think pendulum swings only three weeks in, sounds too soon to me. 

    Good luck with it all... it’s a painful process but will get better after PT starts I’ve heard.

      As a nurse I knew about my surgeon and checked him out with the state and actually have taken care of his pts. He’s the top shoulder surgeon here in Melbourne,Fl. I was at PT the day after surgery. I have been assigned shoulder shrugs, hanging of the arm and the pendulum swings are nice and easy. PT still performs passive ROM and she was able to move the arm above my head without any pain. I wear the sling like I said at all times except the shower, watching tv with a pillow supporting it n during my exercises. They said I can sleep without it and I have no issues. I don’t thrash around when I sleep it’s just this nagging throb just below the shoulder. I alternate between ice n heat whatever I’m feeling up to. Every surgeon will have his protocols. He must be doing something right as his success rate is very high. I asked around to other physicians I know beforehand who they would choose and every one recommended my surgeon. There are a lot up here so I have no choice but to keep plugging away. As luck would have it, I had a heel spur crack off and ended up in an ortho boot halfway up my shin the week after the surgery. I look like a train wreck lol

       Let me suggest to everybody that has had the surgery. It’s very important to get the muscles around the surgery site massaged regularly. The knots form in your neck because underneath your scapula which is near your armpit knots form and pull on the tissue which makes your shoulder hurt, which makes your neck hurt, which makes the inside of your scapula hurt. I was getting massages twice a day from my daughters for 10 or 15 minutes releasing those knots. I could not believe how quickly the pain went away in my shoulder. As soon as I started getting more self-sufficient my daughters are not coming as often. So what I’ve noticed is the pain is back in my neck and the knots in my   Shoulders. So what I did was I get a lacrosse ball I put it on the wall and lean up against it and roll up and down on my back and my neck and right under my armpit in the back. This releases the knots therefore the pain is not in your shoulder and your neck. I highly recommend Massage as much as possible to keep those knots loose so they don’t tighten up the tendons in your shoulder.

    I second everything Mia has said. I have heard some surgeons are less conservative but having had exactly the same op I would want to take it conservatively! As for pain I was in agony and on mega prescribed pain killers for many months so pain quite normal. I am 18 months post op on r arm with partial tear on left. I still experience discomfort if I do not do  not exercise both regularly I have found swimming (not front crawl...yet) a boon.

      I’m a swimmer as well with a partial tear on the left. This all came about from being a 30 yr trauma flight nurse lifting heavy pts into aircraft with heavy equipment. I’ve been surfing for 49 yrs and a lap swimmer daily doing freestyle so all of that has caused this. The things I am passionate about have caused my physical ailments. I’ve retired from nursing and have to change my swimming stroke and surfing, who knows.

    You sound just like me.  My surgery was 2-15-18 so I am 3 weeks post op.  I wasn't having much pain until I started doing the passive ROM and pendulum swings last week.  I'm not taking any big time meds, just extra strength Tylenol and ice ice ice.  I can't wait to start physical therapy in 3 weeks.  My experience with therapy is that they want you to be out of pain and will give you all kinds of help and advice.   Also I want to say the first time I  had rotator cuff repair 2 years ago, it seemed like I was in pain all the time.   When I saw the surgeon at 3 months, he injected some cortisone.  It was miraculous for me.  Took down the inflammation quickly and the pain was gone.  When are you starting PT?

    Hello Jule. I've had a SLAP surgery back in Aug. and my pain lasted sometine. I'm still dealing with pain off and on; even in my neck which started 2 months after my surgery! I've had a few cortisone injections for pain, but it didn't last but a few days. Therapy was ok, but then as time went on, my pain only continued, and I just got frustrated. I'm about to have surgery again to have scar tissue removed and after therapy I should be much better! I'm praying, because I want to finally be able to do the things I enjoy or haven't been able to do in some time. I know everyone's body is different, and so is the healing process, but you have to take your time, and not rush things either! It could take up to 6 months for partial healing and 12-14 months for complete healing for shoulder surgery. Stay positive and do what you need to, in order for you to heal right! This is my 2nd shoulder surgery in the same surgery which was over 20 years ago, but this time it was more intense. I pray you can get sime relief real soon! Keep us informed.

    I had the same surgery 17 weeks ago and the Dr that done the surgery never has gave me anything for the pain since the surgery was done. I have went to him several times and told him that I can't sleep because of the pain, he has told me every time that there is nothing that he can do or give me, for me to go to Walmart and purchase some Tylenol PM to help me sleep. I told him that I don't need anything to make me fall to sleep because I can go right to sleep,but within an hour I wake up in so much pain I can't go back to sleep.

    He wanted to give me a cortisone injection in my shoulder the last time I seen him, I told him that he needs to schedule an MRI and find out what the proublem is before he does anything, well I went last Tuesday and had the MRI and this Tuesday I go back to see the Dr that has been telling me that there is nothing that he can do for the last 27 weeks, well I am afraid that if he says anything about having to do surgery again we gonna have proublems.

    That surgeon is an a**hole. He should’ve prescribed something post op.

      Yes I have to go back to him tomorrow morning, I have been trying to get a different Dr,but it's a workers comp ordeal and they are the ones who have any say so on any medical treatments I have, I have been to a hospital, and private Dr but workers comp won't cover any of it because they didn't approve it, I am afraid when I go to the Dr tomorrow and if he tells me that I will have to have surgery again I may be escorted out, because of him telling me that there is nothing that he can do for the past 17 weeks. I do know that he will not be the one that does any cutting on me for anything

    I had calcium in and around the rotator cuff. IT was a VERY painful condition. My surgeon went in and cleaned out all of the calcium, shaved some bone down , put in a suture anchor, and sutured the rotator cuff back together. I had my surgery on Sept 25, 2018. It is now 2 weeks and 5 days. The first week and a half was pretty painful . Im still in pain but not as bad as the first week and a half. I sleep in bed but have to have about 6 pillows to prop me up. I still cannot sleep flat. My sleep is still very interupted. And i imagine it will be that way for quite some time. At 2 weeks, my surgeon had me start PT. He starts with a tens unit and heat on top of the leads which feels good. I am doing some pendulum swings, but not a lot.. I am using some pully's to slowly pull my arm up with my good arm with the pullys. Im doing ok with that movement. My therapist is gently stretching my arm and shoulder which actually feels good when he does that. I have to wear my sling for 6 weeks. When i reached the 2 week mark, and went to see my surgeon, he said i always need to wear the sling for 4 more weeks except when im watching tv, i could rest it on a pillow or my lap and at bedtime. When i move my arm a certain way, I get a really bad sharp pain , bad enough that i would yell. So, Im guessing its from the surgery.. I hope. Im also having neck pain probably from the sling. And sometimes pain in the bicep muscle on my surgery side. Im a little concerned about the amount of pain that i have when i move my arm in certain ways. But, im fresh out of surgery, 2weeks, 5 days... so hoping its from that.. We'll see......

      THANK YOU CHRISTINE FOR YOUR RESPONSE AND SUPPORT. SO GLAD YOU ARE DOING SO WELL BUT HAD TO PAY THE PRICE FIRST RIGHT? THIS IS A HARD RECOVERY PROCESS BUT YET I NEW IT WOULD BE. MY SURGEON SAID IT WAS GOING TO HURT PRETTY BADLY AND HE WASN'T KIDDING. I HAD AN EXCELLENT SURGEON AS WELL AS A PHYSICAL THERAPIST. I'M JUST GETTING STARTED WITH THE HEALING PROCESS AND I KNOW THIS IS GOING TO BE A LONG HARD ROAD FOR ME FOR A FEW MONTHS TO A YEAR.. I WAS READING ABOUT EVERYONES SURGERY AND RECOVERY ON THIS THREAD AND EVEN THOUGH THIS THREAD IS OLDER IT STILL HELPED ME REALIZE THAT THERE ARE SO MANY OTHERS LIKE MYSELF GOING THROUGH THIS. IT GIVES ME SOME COMFORT TO SEE AND COMPARE NOTES. THIS SURGERY IS NO JOKE OR SHOULD I SAY RECOVERY IS NO JOKE. I'M JUST THANKFUL I DO NOT WORK AND I CAN TAKE MY TIME GETTING MYSELF WELL AGAIN.

      Renee.....A good therapist is worth his/her weight in gold. Sounds like you have one. You are doing good if you can sleep in your own bed this early. For me it was more like 3-4 weeks and even then at first it was only for part of the night and then I was back in the recliner. Also you really should not be moving your shoulder at 2 weeks except under the direction of your doctor or under the supervision of your therapist. Your body is telling you...don't do that....that's why it hurts. 😃Don't go too fast, take it easy. It is a long road but I bet you will eventually be better. I am 7 months post op and do things without thinking anymore. Well except I know I will never lift anything heavy overhead. But there were times over the summer where I thought I would never get better. Then it seems all of a sudden, everything fell into place and I was lifting my arm way up.I also had neck pain. A lot.Actually I returned to my therapist to work on this just last week. I'm sure it was from the sling and from holding my arm in odd positions to protect the surgery. Good luck...let us know how you are progressing.

    Hi Julie

    Just to reassure you, what you are describing is quite normal. Calcium on the bone..... the consultant described the pain as having a shoulder packed with tiny slivers of glass, he was so right!

    I went through everything you are going through. That was in November 2016. Even with excellent physio not fully recovered until October 2017.

    However today I am decorating the whole house......steadily. I also have a partial tear in my other shoulder so any pain in either from any activity, I get the trusty stretching bands and do my exercises. I am on my 4 th set, they have been a great investment.

    i had my surgery sept10,2018 and i am in the same amount of pain as when i came home from hospital. i have 2 anchors as well, severe dibrevment but i cant sleep in recliner, couch , or bed, im just miserable. i start therapy this Tuesday and I am scared about that because of the pain. How you know if you have had a failed repair?

      IM STILL IN SOME PAIN WHEN I MOVE MY ARM A CERTAIN WAY. BAD ENOUGH THAT I WOULD YELL. MY RANGE OF MOTION ON MY RIGHT ARM IS STILL NOT BACK YET. ITS HARD FOR ME TO LIFT MY ARM . THE PAIN IS TOO MUCH IF I TRY. HAD MY SURGERY ON SEPT 25 2018. THERAPY IS ACTUALLY HELPING. ITS HELPING TO LOOSEN MY ARM/SHOULDER UP. SO I COULD MOVE IT MOVE FREELY, WITHOUT SO MUCH PAIN. HE REALLY IS WORKING ON MY ARM/SHOULDER. HE IS THE OWNER OF THE THERAPY PLACE. I DIDNT WANT ANYONE ELSE BUT HIM. HE KNOWS HIS STUFF. HE DOES A LOT OF STRETCHING OF MY ARM AND THAT FEELS GOOD. IF ANYTHING HE DOES HURTS, I LET HIM KNOW AND SOMETIMES HE JUST SEES IT IN MY FACE AND HE'll STOP. DONT BE SCARED OF THERAPY . THAT STARTS YOUR WAY TO RECOVERY! ANYTHING THAT HURTS, LET THEM KNOW. I SLEEP IN BED BUT MUST HAVE 5 or 6 PILLOWS BEHIND ME. SO IM PARTIALLY SITTING UP AND FALLING ASLEEP. I AM JUST NOW ABLE TO LAY ON MY LEFT SIDE, (NONSURGICAL SIDE) , WITH MY RIGHT ARM, (SURGICAL SIDE) CLOSE TO MY BODY AND I AM COMFORTABLE FOR A WHILE. I HEAR IT TAKES 6 MONTHS TO HEAL. SO ANY PAIN YOU HAVE NOW IS MOST LIKELY NORMAL. BUT TALK TO YOUR DOCTOR AND LET HIM KNOW ANYWAY. SHOULDER SURGERY LIKE WE HAD IS A VERY PAINFUL, SLOW RECOVERY. BUT, EVERY DAY THAT GOES BY IS GETTING US CLOSER TO FULL RECOVERY! HANG IN THERE.. GOOD LUCK WITH YOUR RECOVERY!!
