painful when urinating

Hi my doctor suspects i have LS. I have been suffering with severe urine infections for the last month for which i was given antibiotics, they did not help. As part of this condition if i have it, is it common to have so much pain going to the toilet and have constant stinging/burning? i have been given 1 ointment to use, but no follow up information or any proposed referral. i have ordered emuaid to try after reading reviews that this helped. would be grateful for any advice as this pain and discomfort is very distressing and affecting my sleep also.

    Hi Alison,

    This does sound like a major flare-up of LS. If you've only seen your GP about it, you might need to push for a specialist. The ideal one would be a vulvar dermatologist or a vulvar clinic. What ointment have you been prescribed? There's a list of topical steroid strengths if you google the phrase and LS is always treated with one from the very top of the list – 'ultra-potent'.

    Emuaid or any other thick, oily balm – even just Vaseline – is a necessity to form a barrier against urine, which is really irritating to LS. If you're under a lot of stress or eating a lot of sugary foods, those are the two things you can address without the help of a doctor and which will work quickly to prevent and calm flare-ups. The steroid ointment is long-term. It takes many months for it to chip away at the thickened layer of dead cells and penetrate to the real problem – the inflammatory cells in the 'basement'layer of the skin.

      Thank you for your reply, I am currently using betamethasone and so far no relief! My urine still has infection but the doc said leave a week before submitting another sample to be tested. I watched dr Goldstein video, I'm just not convinced I have LS, my emuaid arrived today, very expensive for such a small tub! Hope it's worth it .

    Hi Alison - having written a further post reinforcing what Morrell suggested and added my tuppence i deleted the lot!- so here goes again!

    Buring and pain - yes if you do have LS and a Flare Up this is one of the nasty parts of the condition

    Drink a lot of clear fluid and cranberries and the juice - to neutralise the urine

    Remember concentrated urine burns and stings more than dilute urine

    Does your urine smell? normally  it wont when it leaves the body it is only after oxidation that "that" cowhshed or fishy smell occurs which may indicate and infection . Did the urine test come back fee of infection? Did the GP LOOK at your undercarriage- have you? if not get a magnifying mirror a good light and look! Is there any change in structure /colour?

    Eat live yoghyurt to counter the affects of the antibiotics that may promote thrush.

    No bath time smellies or powders. Long bath pat dry massage in the steroid into the SORE areas for 90 seconds - YES NINETY - wait 20 minutes - it may well sting - bear with it - then apply your bland moisturiser to keep the mucosa supple and then the barrier cream / ointment

    Keep urine and all body fluids off the skin - hence the need for a barrier cream  and some spray themselves with dilute bicarbonate of soda in water after urinating pat dry and again apply the barrier cream. Many use coconut oil and emu oil. Others, emulsifying ointments-

    no tight clothing - tights or knickers or trousers or pants

    Keep stress levels to a minimum - execise to increase natural endorphins and reduce the cortisols floating around the body

    IF you DO have LS your condition will be similar to some and very different to others and the way you manage  the condition will be different too- we all suffer differently. HOWEVER the aspect we all seem to suffer from is a reaction to stress and stressful situations then flare ups happen- so Do try and manage the stress in your life and keep a GOOD eye on what is happening to your undercarriage

    Push for a referral as Morrell suggested  And good luck - i hope you just have a urine tract infect - that will pass in time sadly LS is for life

    As others have said keeping the area free from urine is the most important thing to do if you have ls or not.  Usual advise like wiping back to front and washing without soap, you can wash with Epaderm or other similar emmollient washing product like Hydramol.  This  is also great for moisturising the area after washing which is the most essential habit.  Protect, Pee, clean and protect every visit.

    I purchased a citzbath from the internet which I use daily to help keep the area clean and also add a pinch of bicarbonate of soda to help nutralise the urine.  This also helps to prevent the stinging when weeing as you can sit in and wee into the water discard, refill and clean off.  So easy.  Like they say ask for a referral to a specialist.  I have a dermatologist who is lovely and seems more informed than my gynaecologist was.  Good luck.  This site is full of information.

    A bit a 'shot in the dark' while there is no clear diagnoses.  However, I will add my two cents worth and encourage baking soda/bicarbonate rinses after every bathroom visit. (small spray bottle with three pinches of bicarbonate in warm water) (very soothing)  Fights infection as well.

    hi allison

    from my experence i only burn on the outside skin around my vagina area and use amolin baby nappy rash cream, it doesnt hurt while i pee just when it hits the skin you need tosee a skin specialist and get a biopsey done or it my be a bladder problem i hope that is it .

      How quick do you benefit from using this cream? I'm just not getting any relief from anything I'm using .

    Hi Alison,

    I've always been told that LS does NOT travel up into the vaginal passage.  I had reoccuring UTI's also.  URAL sachets may give you temporary relief for the burning and stinging which are little satchets of orange/yellow colouring tipped into a glass of water.  Anyone woman with continual UTI's should be checked for Cystitis or Interstitial Cystitis by a specialist Urologist.  This symptoms of this condition are ongoing burning and stinging with and after urination and it often runs in families. Consider going to another doctor, they should ask you to provide a urine sample so it can be checked.  UROLOGISTs are the best specialists in this area but as I was told if you have LS you may need to get that settled down before going in for a Cystoscopy which is a test that allows your doctor to look at the inside of the bladder and the urethra with an instrument called a cystoscope under general anaesthetic (day procedure).

      I'm contacting my doc today and submitting another urine sample as I still have symptoms of infection, I am going to ask to be referred to a specialist, thanks for info.

    I would get a second opinion if he didn't make a follow up appointment he isn't a good doctor and you should get a biopsy to see if you have it

      No follow up appointment and was told supplier had problems getting the most potent steroid cream, going to different doc next week. I'm sure I still have infection as I'm constantly going to toilet and never feel I've finished.

      Where are you? In the UK there shuld be no difficulty in supply of Clobetasol- get a prescription and go to a pharmacist who can obtain it.

      Did you look at your undercarriage? is it inflammed / any white patches / or changed structure?

      Do drink cranberry juice it reduces urinary tract inflammation

      Do drink a lot of clear fluids- concentrated urine is acidic and will burn more

      when you wee pour tepid water over the area at the same time - even better with bicarb in it to reduce the alkalinity or a teaspoon of salt in a pint of water that is the same salinity as the body so wont sting

      Scrupulous hygiene is ESSENTIAL wash after each trip to the bathroom as Hanny would say!

      Is your urine cloudy / smelly / concentrated ? Did the doctor /nurse test your urine while you were there?

      if your temperature goes up or you get a pain in your back under your ribs go to the doctor asap - you do not want the infection to take hold

      You can get dip sticks from  the pharmacist to test whether there IS protein in the urine - a good indicator infection may still be present

      Dont forget there are some antibiotic resistant bugs out there- so you MAY need a connoction.

      You should ask for a referral to a consultant (uro gential)

      If you can print off all your replies so that you have a record of the advice and your queries - it is easy to lose the threads

      Try coconut oil so many on this site swear by it - it has antiseptic properties too as well as soothing

      I can't see anything like white patches just a bit inflamed! Urine is strong smelling, last test said their was infection! But was advised to wait a week as I had just had antibiotics. Have got bicarbonate to try. I drink water and cranberry juice every day, have for years.

      Alison, it sounds to me like you should be seeing a urologist. Chronic bladder infections can turn out to be interstitial cystitis. My mother has it. That's the thing you need to get straightened out. If you do have LS, strong bacteria-laden urine is hell on the vulva. Maybe even on healthy vulva.
