Penile Rash/Balanitis Not Responding to Treatment

This all began when it started to burn while I urinated. I went to the doctors because I thought it was a UTI, but the test came back negative. Got tested for STD's, the tests also came back negative.
Ok no big deal, I thought. Maybe this will go away on its own. I then started developing a rash on the tip of the penis and along the base that burned post urination and during ejaculation.
I did some research online and thought it was probably a yeast infection. Bought some Canesten and tried applying it twice a day for a week. I also took the pill (Fluconazole). This resulted in no change in the symptoms.
Went back to the doc, who then prescribed Fucidin H cream (Hydrocortisone 1% Fusidic Acid 2%). I rubbed it on twice a day for a week, no change.
Went to a Urologist who did an examination (but no further tests) and told me I could get circumcised, but apart from that offered no further solutions. Told me it could be prostatitis.
I then went to a Dermatologist, who looked at it and prescribed me Loprox anti-fungal cream with hydrocortisone (1%) told me to use it twice daily on the area for one month. I then switched to just Loprox because it appeared as though hydrocortisone was making the irritation worse. The Dermatologist told me to go see the urologist again if symptons don't improve.
I'm still applying Loprox two weeks into treatment, and the changes are minimal if any. The rash goes down at the tip and expands at the base, almost triangular in shape. There are small scales (or maybe cracks) in the skin, and the area is red, almost as if inflamed. I have no discharge from the penis. Any kind of friction (i.e. masturbation) makes the redness and burning much, much worse. I have been abstaining from that for now and only washing the area with warm water. I have thrown out all my heavily scented soaps.
I am in the process of attempting to get an appointment with another urologist in order to get a second opinion. Topical creams don't seem to be doing much, but I am going to keep putting Loprox on it for now. I feel like the Docs have been kind of brushing me off/don't want or don't know how to deal with balanitis. Nothing I am really doing is working, so I'm wondering if anyone else has any solutions.

i have a very similar situation.... started with burning sensation when peeing and lots of sensation. i also felt alot of itching and irritation on penis area and i began to see redness or inflammation on the penis tip area and on the foreskin. i got scared and thought uti or std but after getting tested everything was negative except for hsv 1 which doc said was common and what not, ive had fever blisters periodically since high school. so i was given anti biotics nothing worked... re examined then "diagnosed" with balantitis.... given creams but nothing worked. i was given prednisone which helped my skin heal but it came back a week later. i now have had this for 6 months where my penis head is red with irritation, my foreskin is red with inflammation and now has like a ridge from the irritation, penis tip irritated, and when i have to pee it gets super sensitive and sometimes when i go poop my penis gets really irritated. sex causes irritation and masterbation does also. kind of stuck at this point. staying glued to this thread.

i have alot of the same thing going on my doctor has said he knows its not an std that it is a male yeast infection but ive tried two differant creams that didnt work im not on this pill im two days in and if it works ill be sure to let you know!

from more than 2 months here .no problem when urinating but the burning sensation persists when i touch or try to tighten the skin to see if there is any improvements also when masturbating and even more when about to ejaculate the problem is not from the inside.According to the dermatologist its fungal so he only prescribed me loprox and desonide(higher than hydrocortisone) for 10 days i'm on day 7 and i don't see any improvement we are on the same boat bro.

Roughly 5 years ago, I started wearing tight jeans every day and I've always wore tight boxer breifs. This gives your penis no room for air and it's smooshed against your thigh all day every day. This eventually make the skin softer due to constant moisture and wrinkling of the skin and glans (head of penis). It's been a week of wearing loose clothing and I am feeling much better although there's still some sensitivity. Just search the side effects of wearing tight jeans. I hope this helps.

ive had this for quite a while. im being treated for prostatitits so interested to seemif the antibiotics make any difference. Tried numerous creams but none really do too much to help.

Hey man, which antibiotics are you taking, and for how long? Is it ciprofloxacin? Let me know if this works for you.

Hey Jack, i took Trimethoprim for 10 days. Seems to have helped my prostate issues a fair amount but its early days. What ive also been doing after ive showered is making sure the area is completely dry, using a hair dryer lol. Think this might be helping a bit as well.

Thought I'd post a short update to keep this thread active.
I've completed testing (semen and urine), and it showed nothing remarkable. It showed a number of microbes that I was assured were either contaminants, normal part of the human flora, or at low enough levels not to cause any significant problems.
In terms of treatment options, I tried moxifloxacin for 7 days just to see if it would help most recently (completing my course several days ago) and there's been no real change in my symptoms.
Given that all of my cultures have turned up negative over the two years that I've had this, I really doubt that whatever I have is caused by an infection.
It could be prostatitis, but likely not bacterial, as I've done a couple of semen cultures and also a prostate secretion test, which all came up negative. Unless the microbes are pesky and require a 6-week course, but without a positive diagnosis of bacteria, that treatment option is unlikely (though I haven't completely ruled it out in my head).
Since last updating, I saw another urologist but he was completely useless. Refused to dig deeper into the issue, was extremely rude and gruff with me, and told me only to come back if I saw blood in my urine (I have unfortunately experienced this a couple of times with different doctors).
The dermatologist I consulted with on this told me nothing more can be done from a skin perspective, as he doesn't think it's anything serious and has, on a couple of occasions, refused to do a biopsy. Not to mention I've tried many creams.
I even saw a rheumatologist recently to see if this was caused by an autoimmune disorder. I haven't followed up with him but I did receive my test results back. The only thing that came back abnormal was low creatine levels, all other indicators were fine. I will call in the new year to see if there are any updates on that front.
So now I'm leaning towards a diagnosis of trauma of some kind on the tip of the penis - maybe an ulcer that hasn't healed that's causing inflammation and discomfort, non-bacterial prostatitis, viral or unclassified, or something else entirely that I do not know at this point. It is for sure not fungal and very unlikely bacterial.
Two years on and my symptoms are very much still there. This has been incredibly frustrating and challenging mentally, but I haven't given up. I refuse to believe that I just got this out of the blue and there is no cause. I will keep updating this thread if I find out anything new. I'm trying to see another urologist and I am also very much still going to appointments.

Hey Jack,
have you looked into bacterial vaginosis (BV)? its really common amongst women and not so common among men, but since you are uncircumcised there is a possibility for bacterial issues.
Also, have you tried muporicin ointment? if you have how was your experience?
Lastly, have you considered just getting circumcised and having them do a biopsy of the foreskin?

I've had the same symptoms (as shown in your pictures) with the burning after urination and ejaculation for 2 years now, and based on everything I've read in the Face book groups and Reddit, I'm inclined to thinking it's non-bacterial prostatitis / CPPS. I think that red rash happens below the skin and it's more like really small veins blushing because of inflammation caused by pelvic floor disfunction or something that gets worse after masturbation / sex

I had sex a month before symptoms started, it was protected. Symptoms began with burning urination and evolved into everything else you've seen written.
Yes, honestly - CPPS is where I'm leaning right now. I have done many, many tests and almost everything has come up negative. The only positive result worth mentioning was a urethral swab which came back positive for enterococcus faecalis, which I was then subsequently treated for with Ampicillin.
I would agree with you that the redness / irritation I'm experiencing is below the skin, it is present basically 24/7 but the intensity dies down if I don't masturbate. When I do, it almost instantly gets 1000 x worse. My meatus often swells up after masturbation as well.
I'm trying to see if I can get a trans-rectal ultrasound done, just to see if it's something physical that's causing this, maybe a blockage of some kind or a stone somewhere. Really not sure at this point, as I've also had this for more than 2 years and the doctors I've seen have yet to provide any kind of definitive diagnosis.

Do you still have pain/burning?
I've been reading CPPS groups/forums and some guys do have the same symptoms of redness/rash on the glans and tip. However, their main concerns are more towards the severe pain that comes and goes.

Just know you aren't alone. I've been experiencing similar symptoms for almost a year and have had the same reactions from Doctors. Urologists/Dermatologists are less then helpful and seem that they don't want or care to find out the root cause. Most will brush you off or insist it's a skin condition only even when other symptoms are present. I'm basically convinced Doctors just don't know what they're dealing with. Be weary of just using antibiotics esp long term. Unless you are 100% sure it's bacterial I wouldn't even risk it. I'm leaning toward CPPS/ Hard flaccid syndrome which can most likely be caused by the pelvic floor muscles or the pudendal nerve. Unfortunately there isn't a lot of research on this either. You can probably find a physical therapist that specialized in it for male pelvic dysfunction but it may be out of pocket unless you have good insurance. Mine is terrible so unfortunately I'm sticking to at home stretches and using a foam roller/ ab roller to target muscles. Look up "Cure CPPS" on youtube this guy's channel is a good source of information. I'm also trying to stick to an anti-inflammatory diet as well. Good luck everyone.

Yeah, I still get burning. It comes and goes with urination but is always there with ejaculation. It's like a burning sensation.
