Penis foreskin cracks during sex, peeling skin, dots and spots [documented]

Hi,
First of all thank you for your time and sorry for the long post,
I'm a healthy 29y old male and in the past 8 months I've been having some problems when having sex. Around February I've noticed some cracks on my foreskin , at the time i didn't thought much of it, and it eventually healed up, after that, every time i had my foreskin would always crack in the same spots with the same pattern, and it started to appear even more cracks
, when they happen during sex, i can clearly feel them and it stings a little, but after the act, i would clean it with lukewarm water and i would only feel an occasional sting or 2, and, along with the cracks, i also feel the area where the cracks are very very raw and sensitive to the touch, the rest of the penis is fine. In the healing process, after 1 or 2 days, my skin would feel very dry, accompanied by some peeling skin where the cracks where
, that area of the skin, eventually they would heal up, but the skin in that area looks a bit darker than the rest
, the dryness i would fight it with some moisturizer.
In all this months I've tried some approaches to tackle this without success, i used a cleaning solution with neutral PH and no fragrances, anti candida fungal ointment, topical steroid cream, fusidic acid and also elidel, already went to 2 different dermatologists, and the problem still persists, with a down side, that now, it seems to be getting worse as some dots started to appear on the glad and also on the foreskin area
. Along with all of the this, the redder area of the skin sometimes when is dryer it also looks shinier. I'm posting here in hope of some advice of someone who has gone through the same thing. Before all of this, i had a normal sex life with no problems whatsoever.
Best regards, Thank you,

Do you know what caused the condition for yourself?

I recently started using regular Lotrimin and its the first thing that seems like its actually making a difference! Major improvement after about four days. I was going to keep using it but I just saw my dermatologist and he gave me a prescription Hydrocortisone-Iodoquinol cream that he thinks will likely be more effective. I'll let you guys know if it works.

i understand what you are going through. Its been 2 months for me and its making me nervous. I have a appointment next week and ive had to refrain from sex . too sensitive afterwards . My penis had gotten raw to the point where the whole thing dried up and started peeling like a snake shedding. If i get fully erect it cracks in the skin around the head and below but not the shaft for the most part.

Hey guys.. I think i have the solution.. I fix my foreskin just using 100% organic coconut oil and Lotrimin AF, every 12 hours and Azitromicin 500mg twice a day, since the first day I saw the changes and in 2 days it was gone..
I just created this account to share the same way you guys did..

please try changing your laundry detergent to non biological and buy new pants.. DO NOT USE THE OLD ONES as biological washing powder enzymes stay in the fabric. They eat bacteria but unfortunately do not distinguish so if you sweat they activate on your skin. I had many years of going round in circles with DR's/SPECIALISTS until one very elderly but senior one took one look at me and diagnosed a reactive dermatitis caused by this.

hey buddy ,
just signed up to give you the solution, i had the same issue for almost 5years . i have tried fungal creams and antibiotics and cortosteroid creams that was of no use.
finally i found a doctor whom could help.
Drs advise:
allergy case(most probably latex allergy(condom))
wear 100%cotton breathable loose underwears
wash under the penis shaft and dry it.
use ***triamcinolone cream *** for 1 week but i used for 2 weeks.
remember if you wash your penis apply again after drying your penis.
do not allow your groin to sweat.
hopefully its been a month i dont have any issue.
edit:
stop sexual activity for the course and do not masturbate.

I have been suffering from the same condition for years. I've tried all the same creams, medications, antifungals with no permanent solution. Witch hazel is the best so far for helping clear up outbreaks faster. I am a type 2 diabetic and have thought that glycourasia might be the root cause combined with circulatory issues. Since nothing seems to cure this, I've recently accepted that circumcision is what I have to do.

I too signed up for this because I was dealing with exactly the same issue. For about 2 months, I was putting Cortizone-10 cream and all that did was dry and crack the skin. It would also peel off too. I then tried Lotromin Ultra after thinking it was a fungus and no luck. Went to urgent care and they gave a me perscription of Clotrimazole and that didnt help either. Lastly, I tried Aquaphor and Coconut oil and saw some improvement but not quite healing.
Finally went to a Dermatologist after almost 3 months and he perscribed me Betamethasone Dipropionate and after 1 week of use, I feel like my skin back to normal! I cant explain the relief I feel after months of this plague. So do yourself a favor and ask the dermatologist for Betamethasone Dipropionate. Hope this helps!

I have recently been going through the same thing. The comment about the thinning of the skin from steroid creams makes me wonder. I was diagnosed with Low T just last year in September and was prescribed testosterone injections. All my problems with low T have slowly diminished, but now I have this problem, along with acne on my back. I wonder if it is related? Hope both of us find answers!

Hi guys,
I have been following this thread for several months. I have the same problem since August 2021, so more than half a year at this point.
I have visited in total 4 urologists, and 7 dermatologists and tried all kind of medication that you also shared here.
Let me say something that I haven't seen as often as it should be on this post:
If you have this problem YOU NEED TO VISIT A DOCTOR, particularly a DERMATOLOGIST. The urologists were totally clueless about what this is. I was tested 3 times for STDs, nothing was found obviously. Like many of you, I had the same partner for very long and interestingly enough, this problem started after we spent 1 month apart due to work! So I was not even having sex!
Now, there is a lot of confusion about what this is and why some doctors are thinking is fungus, others eczema, etc. Well, I finally found a good doctor that took all the time I needed to understand that train of thought.
Essentially this thing looks like allergy, fungus, eczema, psoriasis, VPH, lichen sclerosus, and even skin cancer. So what they usually try is to go from more to less common. Also from less to more invasive treatments.
Why do I say this? Because some people say here that they found a cure for themselves, it might well be that they have one of these conditions and found a good solution for them. But it does not have to work for all of you. So again, go to the f doctor.
What did I learn so far about my condition, which again matches 100% the description of the original post:
My last dermatologist found the 3 first points (and how I reacted to the treatments) very important for further diagnosis.
In which stage am I now?
After trying all the common creams I have been suggested a biopsy. The last dermatologist I am visiting is quite confident that this could be lichen sclerosus which is common after VPH, which I contracted years ago. I also had a biopsy about the VPH warts a year ago to confirm that they are harmless. The key point to make them see that this is not fungal is that it persists over months, without getting worse or better. It just stays there, which is a clear indicator of a skin atrophy, pointing into certain conditions.
So, I am trying a last cream "against" lichen as a test to avoid the biopsy since the recovery of the biopsy is quite problematic down there, you know.
I also read a lot about circumcision here. Circumcision will help only if the skin where you have the problem is removed. So don't you think is going to be a miracle!
Overall, what I wanted to say is that we all here do not have the same sickness. Some of our cases might be the same, but stop coming here to tell us how a simple cream fixed your problem or even Vaseline. Man, I've tried it all and it didn't work. We all just don't have the same condition. Be smart, seek professional help and use this thread only as support to understand what's going on.
Also a mistake I made myself was to change doctors constantly because I wanted solutions fast. And to be fair, most of doctors didn't take this problem as serious as I felt they should. I mean I am not dying but I have something and is their job to help me, right? There are no fast solutions for this, there is a set of steps that you have to take (already mentioned before the train of though of the dermatologists that you can also see from other users) to discard problems and hopefully, one of them works for you.
I have a control doctor appointment in some weeks to check the progress with this treatment and will update you when I know more.

An update from my side:
After 10 days approximately using corticoids my skin was better than ever. Then I had a sex one night and the skin broke again (less than other times, but still happened).
Went back to 2 doctors next day, both of them assured this is nothing like Lichen but they have no idea of what is (again, what a surprise!).
So one of them suggested to use Elidel as long-term solution, once per day every night for a month and to come back in 3 weeks to check.
After a few days using Elidel like this, having sex does not break my skin which is amazing!
But some days my skin is irritated and it burns on the area that the scars appear.
So seems like Elidel is a patch that works on top of the problem but doesn't fix it.
I am back to believe that this is a super resistant yeast and I will insist on the next appointment to get something powerful to discard this possibility.
