Penis foreskin cracks during sex, peeling skin, dots and spots [documented]

First of all thank you for your time and sorry for the long post,
I'm a healthy 29y old male and in the past 8 months I've been having some problems when having sex. Around February I've noticed some cracks on my foreskin , at the time i didn't thought much of it, and it eventually healed up, after that, every time i had my foreskin would always crack in the same spots with the same pattern, and it started to appear even more cracks
, when they happen during sex, i can clearly feel them and it stings a little, but after the act, i would clean it with lukewarm water and i would only feel an occasional sting or 2, and, along with the cracks, i also feel the area where the cracks are very very raw and sensitive to the touch, the rest of the penis is fine. In the healing process, after 1 or 2 days, my skin would feel very dry, accompanied by some peeling skin where the cracks where
, that area of the skin, eventually they would heal up, but the skin in that area looks a bit darker than the rest
, the dryness i would fight it with some moisturizer.
In all this months I've tried some approaches to tackle this without success, i used a cleaning solution with neutral PH and no fragrances, anti candida fungal ointment, topical steroid cream, fusidic acid and also elidel, already went to 2 different dermatologists, and the problem still persists, with a down side, that now, it seems to be getting worse as some dots started to appear on the glad and also on the foreskin area
. Along with all of the this, the redder area of the skin sometimes when is dryer it also looks shinier. I'm posting here in hope of some advice of someone who has gone through the same thing. Before all of this, i had a normal sex life with no problems whatsoever.

Had this for over 10 years, the question is is it possible to get rid of it? It seems that the only thing that works is to wash every day and apply some softening ointment. I took this seriously a few weeks ago so I treated with Cortimyk ointment for 2 weeks, then I waited 2-3 days. Then I applied Canesten ointment for 2 weeks, then it looked very healthy and good. Then stopped the treatment but after 2-3 days the skin started to peel again. So now I'm back on Canesten and it looks healthy again..

Try swapping out soap/shower gel for "ultra nourishing/anti irritation oil" instead. This got me rid of the problem. It comes back from time to time when I don't have the non soap product but it goes away after I use the ultra nourishing oil...

Dont buy the crap of circumcision. It will not help you and you will regret it. Doctors just want to make money. They dont care if they cut your arm off. It is a thing on men over 30. I found a cream on amazon its called Zero . penile cream. it helps a lot.

been having the same issue as you, brother. been an ongoing struggle for 2 months and I'm now seeing results with the things I'm doing. it's basically eczema. your foreskin is dried up and needs moisture.
you need these two things that you can get at any cvs or walgreens:
these are the steps. it's been working for me so far with good results:
now after you do that, just go about your business. DO NOT SCRATCH THAT ITCH. your skin needs to heal. if anything, pull your foreskin down or peel it, however you want to call it, but don't scratch and try not to do that pulling/peeling often. after awhile you'll need to redo the steps to keep it moisturized. overnight is a lot easier since your asleep but when you feel it itching, it's probably drying up so just redo the moisturizing steps again.
before i was just putting neosporin, but that stuff just dried up my foreskin and made it worse. on day 2 with this treatment thing and i can feel and see the difference .
also, try not to be heavy handed when showering with soap. gently pull down your foreskin when cleaning and drying. be heavy handed with the goldbond. you don't need a whole lot of the eucerin. about the length of the your index finger tip to the first line of it is good or a bit more.
just remember, if it itches, add some goldbond and eucerin. drink lots of water.

I had this problem for a few months i got something called fusilic acid from the doctors and it healed it right up in a week and never had the problem since.

Had this same issue for over a month and my penis looked exactly like the top 3 photos. It was gradually getting worse too, around 10 small cuts and 2 bigger ones, I'd use Sudocrem every day but any time I masturbated the cuts would open and go back to normal. Stings like fk when any fluid gets into it too. I was ready to go to the doctor when I got back from my trip and dreaded the idea of getting the foreskin cut, but if that's what it took then so be it, because I really did feel it was gradually getting worse.
I started using an Anti-Fungal cream called Clotrimazole, careful how often you use it because apparently Anti-Fungals can thin the skin if you go overboard, make sure you wash your hands after touching it too. Basically once a day you put it on the cuts and sit there with no pants on and let it soak in for about half an hour. Wash your penis afterwards. I used to dampen some tissue and wrap it round my penis during the day and then wrap another dry bit of tissue over that to help with the irritating chafing. All the healing properties of the cream are pointless if you just have a wk afterwards though and open the cuts right back up, you don't want it to get to the point of no return and regret not taking action sooner.
I'm no expert so don't take this comment as professional advice, but I'm convinced a lot of other people are describing exactly what i had and was very concerned about, I was lucky because I was in Europe for 2 months so I literally had only a few wanks that entire time, staying in hostels you don't really think about jerking off.
Anyway after using the Clotrimazole for just 1 week I noticed significant improvements, probably because i hadn't had a single wk in that time so my penis wasn't going through any strain and my body was allowing it to heal. after 2 weeks I stopped using the stuff because I literally had no issues whatsoever. All that was there were tiny white marks where the cuts previously were that I assumed to be scars.
The main factor here is that you MUST stop jerking off and let the cream do its magic, it's very tough at first and so habitual for most dudes, I was lucky that i was away for 2 months in hostels so it didn't really cross my mind. I'm talking full cold turkey for at least 10 days lads. I tried many different creams and things for well over a month and nothing even slightly worked, because I was jerking off daily and opening the cuts right back up 95% of the time. I highly doubt it's a coincidence that as soon as I stopped w*****g for about 10-12 days it healed almost immediately.
Anyway 9 months later and I haven't had a single issue since my trip, it's been completely back to normal for 8 months now. I never heard of this issue before I had it, so I know it's pretty scary for any dude to deal with. I'm back to jacking off usually once a day sometimes 2 if im bored but i've had sex a few times in a day too and not a single issue on my foreskin, the marks have all gone and it's back to normal.
If you look at the second and third image of the Original post, that is almost identical to what my foreskin looked like, but my cuts actually looked a bit wider and there were definitely 2 main cuts that were wider than the rest. I should have taken some pictures because it looked like hell.

Okay this is long but I thought that my case might help some of you so here we go…
I’ve been suffering from what seems like this exact condition for the past 7-8 years (M39 here).
The first time this happened, it took a while to heal but eventually the symptoms disappeared - at the time I was petrified and wasn’t really touching “down there” until it healed. After it looked fine again it became clear to me that there was no healing whatsoever - all red right after a short masturbation session. For about a year, every time I masturbated or had sex, the symptoms would reappear. It was a vicious cycle of healing and suffering. I knew immediately that there would have to be an underlying cause as the cracks would always appear in exact same spots.
Over the years, I saw many doctors and dermatologists and it was always either topical steroid or anti-fungal (hydrocortisone and clotrimazole mostly) or both. And of course some antibiotic ointment to speed up the healing.
The thing would come and go for a while but recently, the red scaly cracking patches became very persistent. I would moisturise the hell out of my penis which was giving me some relief but the core issue was always there. Not cracking as easily when moisturised, but still there.
I’ve been searching for answers for years and somehow never stumbled upon this thread. I landed here accidentally few weeks ago and started my research (by research I mean going through my detailed medical history and reading actual research papers on many of the conditions mentioned here to compare with what I am experiencing, trying to put everything in the context of my symptoms and circumstances in general).
I ruled out cancer or pre-cancerous changes quite quickly, as these would not go away or show any signs of improvement with all the different ointments and medications I tried. I would also really hope that a dermatologist would spot such serious changes. Same goes for obvious, well documented things like herpes or psoriasis. I have no reason to believe that so many dermatologists and general practitioners would not be able to recognise all of the above.
After this thread being up for over four years, I think for most cases we can agree that our predicament is not something that is clearly identified nor does it have an easy clinical diagnosis.
The comment that resonated with me the most was from one of you (apologies I don’t remember the name, I’ve read through all the comments and made some general notes for myself but without names) who said that they popped a Canesten (fluconazole) pill and it improved very quickly.
The reason I paid attention to this particular post was twofold
In my head I thought that if I pop a fluconazole pill, the least it can do would be to resolve my eyebrow dry patch which surfaced again recently. And I wasn’t wrong.
Like the previous poster said - the effects are almost immediate. Literally the next morning the patch was gone.
As for the spots/patches on my penis - didn’t notice any marked difference at that point but they seemed less “shiny”, if that makes sense.
As I said, I read a lot of scholarly articles during my research and in the context of my eyebrow what made sense was seborrheic dermatitis - matched symptoms exactly.
It is believed that an association exists between Malassezia yeasts and sebborheic dermatitis - hence the fluconazole / clotrimazole efficacy.
I started reading about Malassezia in general to understand better the mechanism of action and how to cope with it. This was where everything stated turning in my research.
I stumbled upon an article on Malassezia (pityrosporium) folliculitis which is often misdiagnosed in the context of Acne. This was eye opening to me. Acne is a condition I’ve been dealing with for 25 years with no actual cure, just a far-from-ideal management of symptoms.
Acne treatments are very antibiotic-heavy. And what do antibiotics do? They kill our defensive bacterial flora in both the gut and on the skin’s surface, making us prone to all sorts of fungal and yeast infections.
At this point I was distracted and focused on my malassesia (aka fungal ACNE) and my seborrheic dermatitis (eyebrow) to see if -by pure chance - I could imporove my ACNE situation. Compared with dry and cracking skin on my penis, my ACNE impacted my entire life way more.
Reading up on the disease, I found that the most effective treatment is topical ketoconazole and, for very severe cases, a combination of oral fluconazole and topical ketoconazole.
However, considering that fungal acne usually covers very large area (back, chest, shoulders) it is not practical to apply ointment and standard practice is to lather up some Nizoral (anti-dandruff shampoo containing ketoconazole) on the affected body areas for about 3-5 min.
So I was there standing in the shower waiting for the timer to go to zero and decided to apply it on my scaly red penis cause I had nothing to lose.
I KID YOU NOT - 24h later, my penis looked immeasurably better.
Nizoral did it. A stupid anti-dandruff shampoo “fixed” (more on that to follow) my dry cracking sore penis problem!
After all, it was a yeast infection. Just a very, very specific type of yeast. Malassezia globosa.
I have since found that zinc pyrithione is also effective at inhibiting Malassezia’s growth. Zinc pyrithione is found in another, more common anti-dandruff shampoo. Namely Head&Shoulders. Here came the kicker for me - tracking my penis issues back, it all began around the time I separated from my ex who had dandruff and used Head&Shoulders. Living together - we would always share/use the same shampoo. Prior to that I was using Head&Shoulders during my teenage years because of my dad also experiencing dandruff.
Inevitably, when you wash your hair in the shower, some of the shampoo will trickle down to the nether regions. I believe this was suppressing a lot of my symptoms without me even knowing.
The unfortunate thing is - there is no cure for Malassezia. However, it can be effectively managed. After initial two weeks of Nizoral shampoo every two days, it only needs to be done once or twice a month. For severe cases, if topical ketoconazole doesn’t work, there is an oral anti-fungal regime which, after initial treatment, requires one pill every month or so.
I will be continuing with my bi-weekly Nizoral wash and switched to zinc pyrithione based soap just for good measure.
And the best thing? I am no longer afraid to masturbate 😉 Oh, and my acne is no longer a problem!
After so many years of dealing with both THIS PREDICAMENT and acne it actually makes me emotional to think that this thread, with some random guy’s d••k pics in it, in a very convoluted way, led me to figuring out what my condition is and most importantly - to finding a solution.

i am also going through the same symptoms and problem, i have been told that it is an fungal infection. given an lulifin ointment to apply twice a day with an antibiotic med.

So I do have a Solution that worked for me. Here is a bit of background on my issue. My condition looked exactly like this and came about after a really bad Thrush infection. My Dr prescribed a cream that fixed it initially but it kept reoccurring . I worked out that the steroids in the cream thinned the skin and just caused it more.
Solution : Stop any Steroid based cream. even thrush cream has it. If you need to resolve thrush from this point on, used a tablet thrush treatment. There is a one tablet option from the chemist.
Your skin now needs to heal with help. what worked for me is an antiseptic cream twice a day, (I used Savlon as it's also used for nappy rash so kinder to that area) and through the day I used Double Base cream. I also pulled the skin back at night to allow it to stretch out... however I am not sure if this contributed or not. I was told the anticeptic cream could sting but it didn't for me. I can't imagine it's any worse than the actual pain of the condition .
After months of continuous skin rips from the over tight skin, the savlon fixed it all in a few days.
If, like me, you thinned your skin from steroid use, I highly recommend being careful and using lube for a long time. Even on your own. Skin from steroid damage can take months, even years to return to normal .

I have what sounds like the same issue, but my dry skin and cracking is usually near the base of my penis. Lately I've been having similar issues further up the shaft as well. I noticed someone else mentioning a silicon based lube possibly being the culprit, and I HAVE noticed it seems to exacerbate or cause the symptoms. Has anyone else found any info on this?

Exactly the same here and went to the doctor thinking its an STI after painful stinging sensation during taking showers. I'm not circumcised and for me the dry and flaky skin appears mainly around the slightly tighter area of my foreskin which looks like a ring of dry peeling skin half an inch down when my foreskin is pulled back. I don't remember my foreskin being tighter in one place so I don't know if this condition causes it be less stretchy or am I only noticing it now because of this. For me it started about 2-3 years ago and I use water based lubes and tried different brands but they make no difference. I've also noticed it gets more flaky after being in a hot tub or swimming pool making me think its a dermatological issue more than anything else. Hope someone will pinpoint a remedy!

if the condition is reoccurring then you should check your STD status once
as these symptoms are pretty much common in herpes simplex virus infection .
if it viral then using steroidal ointment will make condition worse.

Ok, so first time i had it i was told it was thrush...
Over the counter tablet and cream cleared it up, in two days I was back fighting fit again! Problem is it always comes back after a month or so and cracks along the same lines and causes scarring! The scars cause more problems as the skin then gets tighter still...
Anyone got any ideas??

Hi guys! i was having this issue until i started washing my penis twice a day whenever i noticed i was having an "episode" along with the following,
*make sure to peel the foreskin everytime i wash it and when i pee
*use dove soap regular scented or unscented
this next part is going to sound crazy or silly but i promise its whats worked for me. i went out to buy store brand womens yeast infection ointment and put it around the head of my penis and or you can also use neosporin . i promise this is what works for me . i hope this helps some of you because i was tired of doctors automatically wanting to test me for STDs . im not sure what this is or why it happens but yeah

Had this ongoing for a very long time now. Recently tried washing with cerave hydrating oil cleanser and its kept it from coming back. Cant believe how good this stuff has been for me. Ive tried everything and had some success but it doesnt last. This stuff is a life saver. Please try it.
