Prostatitis with zero pain, just horrible 24/7 urge to urinate/frequency

Hello All,
I am a healthy 25 year old male with no history of urinary issues.
I am at my wit's end. Last Sunday, I started having terrible urinary frequency. I will literally go to the bathroom, urinate, flush the toilet, and then have to use the restroom again RIGHT AWAY. In other words, I have constant urinary urges 24/7... It has been this way for one week.
I don't have any pain or discomfort in my pelvic area, and there is no burning sensation during urinating. However, I feel as if the urine is at the tip of my penis - that is where the urge sensation is most prevalent. If I hold it for a long time and it accumulates, there is quite a lot of urine. However, most of the time (especially in the middle of the night) hardly anything comes out even though it feels so urgent.
I can't sleep since it feels like I have to urinate constantly... last night I was going every 5 minutes, and most of the time only a few drops came out. I am having anxiety now because of this.
I went to the urologist last Tuesday (2 days in). Urinanalysis showed no UTI and bladder ultrasound was okay. However, during the rectal exam, he said my prostate was "off" and did a test of the urine that came out after the exam. At that point, I was told that I had a prostate infection and that the enlarged/inflamed prostate was causing the frequency. In other words, there was bacteria.
I was given 500 mg of Levaquin to take once a day for 30 days. However, it has not helped after 6 doses. I'm trying Motrin (ibuprofen) to no avail. I was told it takes a while for the antibiotic to reach the prostate. I have no fever, and no chills.
I am terrified that I have the chronic pelvic pain (CPP) form of prostatitis. However, I don't know if I do, since (1) there WERE bacteria present upon running the urine after the rectal exam and (2) I have ZERO PAIN. The only symptom is the 24/7 need to urinate. Any ideas on this? I hope I have the acute prostitiis form since this is cured with more antibiotics, but if I have the lifelong CPP form I don't know what to do... I feel like I will have to quit my job since I can't sleep with the need to urinate.
Please help!

Just curious, do any of you have high anxiety/stress? I'm wondering if there's a link there. High anxiety can cause very weird and many physical issues, i know because i had them. I went from prostatitis type issues, to chest pains and panic attacks that morphed into nausea and dizziness for weeks. All one after the other over the course of a year. Im focusing on reducing my anxiety and stress, hopefully positive results continue for me. Good luck everyone.

Been a few months since my last update. I can see that a lot hasn't changed for everyone on here and I may as well update my current situation. Currently on Solifenacin 10mg and Mirabegron 50mg which I've been taking for a few weeks. Probably around 2 months now. Maybe more since I had my Urology Camera Inspection up my bladder back in July. They found no damage, no tears.
Doesn't appear that the 'Sounding' I did was the cause potentially. Just the timing of it all was a possible coincidence. Though that said some have said it could be Masturbation. I do it a lot. I can't stop. I'm like really addicted. Do it once or twice a day. Maybe stop for a day or two then do it again. Longest I managed to go without it was 2 weeks but it didn't really improve anything.
The drugs are 50/50 for me at the moment. I've gone days at work where I manage to hold my bladder in for a good three hours and very rarely four hours. At home where I'm majority of the time sitting down on my s****y chair I go to the toilet a lot, like every hour. There are weeks where I go every 2 hours and then I have 'Days' where I'm going 24/7.
I try not to drink a lot these days especially at night. Maybe just sips of water. Currently I got the FLU But HERE'S THE INTERESTING THING. Whenever I've gotten the Cold or Flu in these past year I obviously feel like crap but my bladder or urethra or whatever you call it feels…basically I don't get the sudden quick urge like I usually get 24/7 to go to the toilet. And Sleeping Pills sometimes help to. Almost like it relaxes the muscles inside? I don't know.
I also notice more that I do intact get pain from my anus that sort of travels under my pelvis to my bladder/urethra sometimes. I've been wondering since the beginning whether or not it is a possible Prostate problem? I don't know. My GP won't listen and says I'm too young (28 years old now) to be having Prostate problems. My Dad does say its a mix of masturbating and stress.
I've not gone out in a while and when I have once or twice its been…okay. Still needed the toilet after every 2 hours though. These last few days though, especially today I've had to use the toilet every hour. Its gotta annoying. If it is masturbation related then I need to know how to stop it. I mean, how does any addict stop what they get into? I get way too excited from just seeing basic porn to hot girls on TV and s**t. A lot of things set me off. Its a headache and annoying.

Sounds like it might be prostate or pelvic floor dysfunction really, but it could be something like overactive bladder too.
To be honest, if you doc won't check everything, including prostate, bladder, etc... time for a new doc. That's bulls**t, and being young is not an excuse to not check your prostate. You might've had an easily curable bacterial infection which might be far more difficult to cure now due to it being established for so long. Literally zero excuse for that kind of attitude towards a patient's concerns. You might want to try more exercise, and look into a new uro who will check the prostate and do a scan of the lower torso for any issues, MRI/CT Scan. If nothing turns up, try pelvic floor physical therapy with a PFD specialist.

This angers me. If you are having pain in your perineum, they should be checking out your prostate.
While the masturbation might be adding to your discomfort, it isn't the cause of your problem.

I have been dealing with a tingling sensation in my tip with a frequent urge to urinate for 2.5 years now, but it only affects me when I'm at work or when I'm driving. It doesn't matter if I use the bathroom before I leave the house. 5 minutes into a drive, my pelvis area feels like it swelling up and the only way I can relieve the pressure is to rush to the bathroom when I finally make it to work. It's a weird kind of pain. It feels like I have on pants that are too tight around the waist? My urine tests have come back clean. Nothing was found in my bladder. I don't have kidney stones. My prostate is fine. I recently bought a standing desk at work. My pelvis doesn't swell up at work like it used to because I'm standing, but I still get the tingling sensation and have to use the bathroom every 30 minutes. I guess I need to go see a psychiatrist? I have seen two urologists, and they both are baffled on why I feel normal at home, but my bladder rages when I leave home. Like others have said, it's not my bladder that feels full. It's just that tingling sensation that makes me feel like I have to use the bathroom. I even walk around with adult diapers on for peace of mind just in case I can't find a bathroom when I'm away from home. I also keep an empty bottle in my car just in case there is a traffic jam, and I don't make it home in my usual 30 minutes. Any insight would be helpful.

Your situation seems a bit more unusual in comparison. I don't think I've ever had a pelvic swelling feeling, but to me, you clearly define the urgency sensation to being apparent when sitting. You didn't confirm on if your bladder is actually full when you do go or not. If it's not full, and you just get a weak dribble, then I would suspect some form of pelvic floor dysfunction. Especially if every other test has come back normal. However, it is also possible you have some form of Interstitial Cystitis, which usually affects women, but can affect men just the same. Pain in the bladder and surrounding areas of the pelvis, pressure, urgency, over-active bladder, etc.
When do you recall first noticing these symptoms? Was there some sort of triggering event or did they come on gradually? Do you work out, or ride a bike often?

The tingling feeling first started when I was riding in the backseat of my co-worker's car. I knew for sure I was about to ps my pants and had a panic attack, but I didn't ps my pants. This was also around the time when we were starting a huge project at work. I didn't feel stressed out. I didn't workout at the time this was happening. I haven't rode a bike in years. My bladder isn't full when I get the tingling feeling and have to use the bathroom. I usually have a stream that lasts 3-5 seconds every time I go to the bathroom. Most of the time, I only use the bathroom to get rid of the tingling sensation because it feels like urine is just sitting at the tip about to fall out. But like I said when I get home, I'm almost 100% normal.

Almost positive this is pelvic floor dysfunction. It sounds like you're tensing or holding a ton of stress in your pelvis, and if you were sitting in a tight space and somehow managed to tense or strain a pelvic muscle, it's possible it's stuck in a perpetual state of strain. This muscle might be close to, or part of the group of muscles used to release urine from the bladder, giving you a false sense of urgency, which is further supported by you not actually having a full bladder even when it feels overwhelmingly full.
I would highly recommend you see a pelvic floor physical therapist and see if they can help you find relief.

I don't feel anxious when I'm driving or at work. My commute isn't bad, and I've been driving for years. Is there a such thing as hidden anxiety?

gentlemen, I too have been suffering for around 4 months with both penile and rectum pain/sensation, was diagnosed with chronic non bacterial prostatitis. i have recently downloaded a book called a headache in the pelvis and it has opened my eyes. I now believe my condition is linked to my pelvic floor muscles and have started seeing s physical therapist who has been helping me and i am starting to feel better already. Please go and read this book as it will enlighten you as to what may be going on.

Out of curiosity, how frequent do you feel pain? Is it a constant, or just intermittent? What would you rate your intensity of pain at on a scale of 1-10?

first couple of months it was pretty much all day, only relief was when i was sleeping, past two months it has been a bit intermittant during the day, i dont really feel pain as such but a horrible urge/nip in both penis and rectum. Since seeing the PT two weeks ago, had two sessions with her doing massage i have noticed a slight improvement in my symptoms. A long road ahead but i see light at tbe end of the tunnel whereas before i saw none

I have had cytoscopy, bloods, ultrasound, urine tests none of which have shown any infection. lots of antibiotics which havent worked either. google the wise anderson protocol and read about it, it has certsinly helped me

Arizona, I am not 100% better but maybe 70% both physically and mentally with the latter being the most important in my opinion. Before knowing anything about chronic pelvic pain/dysfunction my mental state was not good. After reading that book and seeing PT i can now see that there might be a way out of this hell that ive been in . It might bit work for everyone but deffo worth giving it a go.
