Prostatitis with zero pain, just horrible 24/7 urge to urinate/frequency

Hello All,

I am a healthy 25 year old male with no history of urinary issues.

I am at my wit's end. Last Sunday, I started having terrible urinary frequency. I will literally go to the bathroom, urinate, flush the toilet, and then have to use the restroom again RIGHT AWAY. In other words, I have constant urinary urges 24/7... It has been this way for one week.

I don't have any pain or discomfort in my pelvic area, and there is no burning sensation during urinating. However, I feel as if the urine is at the tip of my penis - that is where the urge sensation is most prevalent. If I hold it for a long time and it accumulates, there is quite a lot of urine. However, most of the time (especially in the middle of the night) hardly anything comes out even though it feels so urgent.

I can't sleep since it feels like I have to urinate constantly... last night I was going every 5 minutes, and most of the time only a few drops came out. I am having anxiety now because of this.

I went to the urologist last Tuesday (2 days in). Urinanalysis showed no UTI and bladder ultrasound was okay. However, during the rectal exam, he said my prostate was "off" and did a test of the urine that came out after the exam. At that point, I was told that I had a prostate infection and that the enlarged/inflamed prostate was causing the frequency. In other words, there was bacteria.

I was given 500 mg of Levaquin to take once a day for 30 days. However, it has not helped after 6 doses. I'm trying Motrin (ibuprofen) to no avail. I was told it takes a while for the antibiotic to reach the prostate. I have no fever, and no chills.

I am terrified that I have the chronic pelvic pain (CPP) form of prostatitis. However, I don't know if I do, since (1) there WERE bacteria present upon running the urine after the rectal exam and (2) I have ZERO PAIN. The only symptom is the 24/7 need to urinate. Any ideas on this? I hope I have the acute prostitiis form since this is cured with more antibiotics, but if I have the lifelong CPP form I don't know what to do... I feel like I will have to quit my job since I can't sleep with the need to urinate.

Please help!

    I'm 24 years old and from the USA and have found this forum to be the most accurate and helpful out of all the research I've done so far. For me the symptoms are like some of you on here. I don't have any pain urinating or any back pain etc. like some people are explaining. The only feeling I have is the urge to urinate at the very tip. It's been about 5 days now and It's hard to say what caused it however after a lot of research I believe there is a good chance it happened after masturbation. I have gone in for a urinalysis and everything came back normal. I talked to my physician today and she said to wait a few days and see if It would get better. When I take a hot bath it gets rid of the sensation almost completely and like one of you said in the morning it doesn't seem to be as bad. Once I'm asleep I'm not waking up. Its just this really annoying sensation at the tip of the urethra (in or around it) and the feeling to urinate. To be clear I'm not having any ache's or pains and I only feel it at the tip nowhere else. I'm going to try to see a urologist because the feeling is very distracting. I'm hoping this can be solved soon and have a answer because I've got to work and I cant be going on like this. Based on some of what you all have said there is a chance that it's some sort of nerve pinch or damage? If anyone is having just the sensation and urgency at the tip and has found a solution or suggestions please let me know. It's good to know you are not alone in this.

      Any change in your symptoms or have you talked to a urologist. I'm 27 and my symptoms are the same as you although it's been 5 months for me and I still haven't found a cure.

      30 days of Bactrim seemed to cure it but the sensation of having to pee came back. Another 30 days was less success as symptoms came and went. Took a month to just use supplements and essential oils with no luck. Urologists did urine test, culture, prostate exam and bladder ultra sound. All were fine. I'm on Cipro and it's not really helping. My next steps are to see if I can get my semen cultured to pin point the exact bacteria causing the problem. Taking random antibiotics without knowing the cause is pretty aggravating.

      Hope you've found some success.

      I have a appointment with the urologist in two day's and have already talked to the urologist on the phone. I was told to try out Saw Palmetto and Zyflamend but I've only been taking those for two day's so I doubt I'll notice anything anytime soon if at all. I went in yesterday for another urine test specifically for a less common bacteria called ureaplasma but haven't gotten back those results yet. How long would you say Bactrim cured you for? Seems like some people have better success with certain things then others. I don't know about you but for me the sensation kind of traveled/spread to more near my bladder I guess you could say so its not so concentrated on the tip of the penis anymore. I would say the sensation though is very bad and incapacitating (had to skip a week of work) so If you are truly feeling the same as I am I have no idea how you've lasted as long as 5 months (if so I really feel your pain). I'm hoping and praying they actually find something that they are familiar with because so far (i'm sure for you as well) its extremely frustrating when no one really seems to know whats going on. I'll keep you updated.

      Hello, I hope it gets resolved for you.  I am 23. I have been suffering from these  same symptoms for about a year now.  For me, it was triggered by excessive masturbation, at least that's what I thought.  But my physician says that it is not masturbation that leads to this condition but rather one's indulgence in sexual thoughts, whether via porn or our own conjured fantasies, because one is often suspended in the state of arousal for more than what should be a moderate duration , and this can be really damaging for the bladder. 

      Turns out he was right. Ever since I have cut down on porn and other such surrogate fetishes, my symptoms have relatively subsided. And yes, stress willy only augment the symptoms, whatever the cause, so make sure to avoid it. 

      Anyway, do visit a doctor, especially a general physician with some specialization in urology rather than a someone strictly urologist, who are, believe it or not, preoccupied with their own narrow set of diseases, and cannot help but pigeonhole you in some category. Feel free to pm me!

      I went in for an appointment today with what I guess was a urologist assistant. I got two more urine tests, a x ray of my bladder and a prostate exam. All came out fine. I honestly was really disappointed that they didn't know what was wrong.

      The only good thing is the lady I'm working with (uro assistant) came up with a plan we both think is the best thing to do going forward which is process of elimination starting out with easy less invasive stuff before just taking antibiotics for weeks which would be hard on my body.

      I'm taking AVO and Saw Palmetto right now. about 3 day's in with Saw Palmetto and today is the first day with AVO (only supposed to take it for two days) no significant difference that I notice so far but in a week I'm coming back in for a visit with uro assistant and go from there. In the meantime I'm contacting a chiropractor/homeopathic dr. who I have worked with when I was little along with contacting another urologist specifically who is a family friend. I think coming in at all angles and using process of elimination is my best bet like someone above said.

      This week honestly has been hell for me as I have had to skip my work as this feeling is so hard to focus on things. The hardest thing right now is not knowing exactly what it is and so far no one knowing how to fix the problem. I also think having bad anxiety (not taking meds for that right now) are making things worse. It does make me feel a little better knowing I'm not alone in this.

      I have tried cranburry pills from azo so far so good please try them and let me know please go for it only 12 bucks at walmart its called azo cranburry pills u take 2 pills a day let me know the results 🙏🏼 god bless hope everyone here feels better i know that pain is the F**** worst.

      Five hours into taking AZO and I've found about 85-90 percent relief (I'll look into those cranberry pills though). This is the first time in ten day's I can kinda relax. While on this I'm going to continue taking Saw Palmetto as I've heard since it is plant based it can take several weeks to really kick in. I've made an appointment with a homeopathic Dr. to see what she has to say.

      Hoping that I can knock this without having to take antibiotics. Also I've kept abstinent from any masturbation and even sexual thoughts. However I notice even if I have sexual thoughts this will flare up. Even though the uro assistant and my physician think this most likely isn't from masturbation or my prostate (they are leaning more toward a possible allergic reaction to something) I don't fully trust them. That's why I want an opinion from other forms of medicine. Another thing to note is I did notice after my prostate exam yesterday I felt as if this sensation flared up afterwords so I'm not sure what that means but something to remember.

      I took the AZO urinary pain relief "value size" over the counter tablets. So far it has given me about 80% relief. Not sure if or how long this will last or whether it will really cure my symptoms.

      Alright I went to another specialist today (outside of my primary care/health plan). She found that I have BPH that has been in my system for a while and that must of flared up due to excess masturbation and diet. I'm on a diet (no sugars, bread, most meat like chicken and even fruit) so not to provide the bacteria anything until its cured. She also has me on non antibiotic pills. One is for your prostate and the other is olive leaf extract that helps with bacterial infections I guess. The Dr. believes If i keep to veggie diet and take pills for 2 weeks things should improve. She mentioned antibiotics should be avoided if possible for obvious reasons. Hoping this all works.

      Any more updates on your situation? Seem awfully young to have BPH. Are you sure it just wasn't a general condition she assigned to you given your symptoms? You said your prostate exam and scans came back normal and they would've noticed an enlargement from those for sure.

    I thought it was time i post that i too have the same problem. As mentioned above constant need to urinate in the tip of penis. as if ive not finished. Although i intially get the feeling of release. within a min or closing the bathroom door. i feel like ive not finished. 

    Ive had this for a few weeks now. Initially saw my doc. who has checked for UTI and prostate. Both came back ok. I had a blood test to check kidneys and liver. I have to go back for another test as apparently my liver showed something not 100% perfect. But was advised probably nothing could be one of those things. I used to drink alcohol but havent for years. However i used to drink a lot of filtered coffee and diet soda. 

    I then finished work and have been at home largly sitting down for long periods. Normally with coffee and soda. I have since stopped this and largly drink water. i also drink a milk thistle extract daily. and doc pescribed small dose of oxybutain? smth like that. i also stated walking and moving more. trying to not be to long in one place. 

    the problem i think was getting better. possibly diddnt notice it for a few days. I think maybe psycological. so i have asked family to not ask about it. in an attempt to forget about it. i then got laxy for a few days. thought as it felt better id have a coffee . only 1 and the next day i had a diet pepsi. Initially felt 80-90% happy . after a few days of this however i am now back at square one. 

    So im back with my tablets, which i stopped taking for a few days. (i felt better lol) im hoping after a few days of getting back in my pattern ill feel better.,

    My advise would be see a doctor. see if there is an underlying problem. but deffinatly there is some salvation to be found in diet and natural products. 

    If id just developed this problem id book a docs appointmant. immediatle quit alcohol and coffee. try and drink water.  dont masturbate to much. i found that sometimes im ok afterward and sometimes it kicks off again. try and walk and stretch and get fitter. 

    I also noticed as i read from someone above. i would wake up and think hahahaha im fine today. i feel great. 1 trip to the toilet and bang here we go. 

    so i thought about this and based on the idea that people with tourettes can avoid the problem if the do smth really engaging. same with a stutter, people can sing fine. 

    i tried a few things.. i can drive my car. which is rather sporty. and of course my concentration levels are running high . i dont even think about the need to urinate. i play guitar. so i tried that. half arsed not engaged too much and i could notice. turn the amp on , play some rock really engaging and challenging and i dont notice a thing.

    i also played all day with my nephew who is five. and i was in looing after him. again concentration levels high . didnt remember i had an issue. 

    so i dont know where im going with that stuff. but short term if you want a break from it. go for a drive, play an instrument, bake a cake, anything that engages you. do it. try not to think about it. stay with what your doing.

    And when you do notice its back... has it not been longer than normal? i found it worked a bit for me .

    Just started this morning. I haven't done anything different from what I've done most my life I guess. I play PC games most my free time, jerk off occasionally into a sock, drink lots of soda/sweet tea, stay up late. These are the constants in my life that I would relate to possibly cause my issue aside from some kind of genetic issue. I do also have some issue peeing in public bathrooms when others are close by. Maybe a little bit of social anxiety on top of that. It did begin maybe a few hours after I tried to go to sleep after jerking it so maybe there's some cursed porn out there. If you share any similarities let me know. And to the dude that dealt with this for 20 years holy s**t.
