Prostatitis with zero pain, just horrible 24/7 urge to urinate/frequency

Hello All,
I am a healthy 25 year old male with no history of urinary issues.
I am at my wit's end. Last Sunday, I started having terrible urinary frequency. I will literally go to the bathroom, urinate, flush the toilet, and then have to use the restroom again RIGHT AWAY. In other words, I have constant urinary urges 24/7... It has been this way for one week.
I don't have any pain or discomfort in my pelvic area, and there is no burning sensation during urinating. However, I feel as if the urine is at the tip of my penis - that is where the urge sensation is most prevalent. If I hold it for a long time and it accumulates, there is quite a lot of urine. However, most of the time (especially in the middle of the night) hardly anything comes out even though it feels so urgent.
I can't sleep since it feels like I have to urinate constantly... last night I was going every 5 minutes, and most of the time only a few drops came out. I am having anxiety now because of this.
I went to the urologist last Tuesday (2 days in). Urinanalysis showed no UTI and bladder ultrasound was okay. However, during the rectal exam, he said my prostate was "off" and did a test of the urine that came out after the exam. At that point, I was told that I had a prostate infection and that the enlarged/inflamed prostate was causing the frequency. In other words, there was bacteria.
I was given 500 mg of Levaquin to take once a day for 30 days. However, it has not helped after 6 doses. I'm trying Motrin (ibuprofen) to no avail. I was told it takes a while for the antibiotic to reach the prostate. I have no fever, and no chills.
I am terrified that I have the chronic pelvic pain (CPP) form of prostatitis. However, I don't know if I do, since (1) there WERE bacteria present upon running the urine after the rectal exam and (2) I have ZERO PAIN. The only symptom is the 24/7 need to urinate. Any ideas on this? I hope I have the acute prostitiis form since this is cured with more antibiotics, but if I have the lifelong CPP form I don't know what to do... I feel like I will have to quit my job since I can't sleep with the need to urinate.
Please help!

ive been suffering with something very similar for about the last 4 months, all started after sex, felt like cystitis. It eased up alittle but never fully went. Since Feb I have had numerous tests, psa, urinex3, full array of sexual health tests , prostate exam, bladder and prostate scan, everything is normal.
for me I have the feeling there is urine in my penis, getting the doctors to understand this and not assume it's an uncomfortable bladder is almost impossible. i have the expected dripping after peeing, I have to use paper to blot the tip or it will cause a soreness and a red rash, like peeing acid. takes 2/3 days for external soreness to go.
Doctors have suggested its a response to caffeine or a strong urine, drink cranberry and loads of water. I don't want to admit it as I will feel helpless but I'm basically being told they can't do anything so deal with it.
It is making the prospect of work meeting hell as I worry I may need to leave and have ppl ask questions. I can't compete at crossfit or do the long training runs I need to the races I have booked, worst of all no sex. Life was great in January.
on a plus is does ease up and has gone for a week or 2 at a time, it's not so bad that I am awake all night as I was when it kicked off. I'm sure it is partly psychological the worry created makes it worse.
im going to go totally caffeine free and increase water but plan of visiting the doctor again and demand a referral, there is a reason for this and I want to know what it is.

I have had similar problems with irritation at the tip of my penis on and off for years. I'd also have trouble sleeping because of pain in my bladder and every now and then is get these sharp stabbing pains in my bladder that were almost unbearable, but would only let a few seconds. After many rounds of antibiotics prescriped by my general practitioner what he thought was prostitis prostatitis,I finally went to a urologist. He diagnosed me with spastic bladder and said it may be caffeine, acidic foods or gluten irritating my bladder and urethra. Stopping caffeine and spicy foods didn't have much of an impact, so I quit gluten, and after years of going through these symptoms, I've finally overcome them!
I strongly recommend giving up gluten for a couple weeks and see if that helps. It's changed my life for the better!

its calmed down over the last week with the help of cystitis relief meds, wife has to get them for me as they are not licensed for men in the uk.
Still have a sensation but I can sleep and relax for a change.

Hello there. I've been facing the same exact problem for almost 3 years now.
For the first time I did urine culture, and got some bacteria. Doc says it's uti. Ultrasound, pelvic mri, etc tests are okay. I had minor bph as well. I'm 33 years male.
I'm having 3 meds for almost 2 weeks. 1. Dubinor, 2. Levomac, 3. Geripod. Each tab once daily.
But I'm getting no relief. Any help?

Thought I'd give a update on my situation because I seriously need the help now and its driving me insane.
Since the end of January I've been having urine frequency. That hasn't stopped. Its gotten to the point where I go to the bathroom every hour on good days and more quicker on bad days and when I'm work I can barely just manage 2 hours.
I finally had a flexible cryoscopy done the other day and they found nothing in my urethra or bladder. There was no narrowing or stricture like I and my GP assumed because of my sounding. In other words this wasn't the sounding that caused it and the only thing I can think of now is either the over masturbation caused it or I have an infection they''re just not finding. I've already sent urine samples for them to culture but considering how s****y the NHS is these days I'm a bit scared that they haven't done their job properly. The fact they don't detect any infection with the urine stripes is confusing because when I use the stripes at home I get the very first block of pure white that indicates a 1.5 whatever infection most of the time so I'm confused.
My Dad says its stress, anxiety and depression that's causing my problem.
After the cryoscopy I was told to lay off the coffee and tea but I only drink herbal tea and I only have one half a cup of coffee (now changed to de-cafe) in the mornings.
I'm taking drugs that are meant to relax my muscles so I can pee but the past few days I've had trouble peeing because my penis feels blocked and its a struggle to pee. I'm drinking plenty of water especially now in this heatwave we've got in the UK. I eat chicken mostly and lots of ice cream. I did blood tests and no diabetic signs in them.
I'm convinced it is a mix of over-masturbating which I'm having trouble stopping and stress and depression. I just wanna be able to go back to how i used to be, managing to go on without using the toilet for a good two to three hours. Its agonising and I don't have a social life anymore because of this.
I'm also wondering whether heavy lifting is also a cause because of the retail work I do. I don't know anymore. Its driving me insane.

Hang in there man, only someone that has/had this issue understands the frustration. There's a chance there is an infection there somewhere hiding that's not showing up, that's what I think happened to me. I ended up taking 2 rounds of antibiotics that helped. You could have inflammation that's contributing your issue, do you take anything for that? That also helped me I believe, I took a natural prostate pill but also magnesium supplements my chiropractor gave me. The chiropractor along with deep tissue massages also helped. You gotta hit it from all angles in my experience until you see improvement

If you get better, decrease your masturbation activity to 1/week and work on streching and flexing pelvic. Get 0.25 xanax at night for a week it will help. Also try 5mg cialis for a week, dont take every day, every other day is fine.

But once I finished them I was little bit better. After few days it started again. My symptoms are, when I go to pee, I pressurise to empty my bladder. When I go back to room, I feel like I've to go again.
I've had a culture for urine done earlier and had some bacteria growing there, hence I had those 3 tabs above for 15 days. After I finished the course the infection was gone, but looks like it has came back again after 15 days. I'm consulting urologist and he said I'm not into any disease. As all my ultrasound, mri pelvic region, etc are okay.
It's driving me nuts now. Is there any doctor who can read this and really help us?

Yes, 3 days earlier. So I'm also looking forward to know if I got that infection again.
And I saw you ask about those 15 days. Yes in those days as well. But my doctor says that everything is allowed. Even nothing to stay away in food etc.
Today is horrible for me. Since morning I'm having the issue. Also to update you, I'm eating those two medicines daily at night, but looks like not being helped a lot. Alprax 0.5 and tryptomer 25 mg. Google it, if you want to know details about those.

I think 1 of the med I'm taking I shared above is alpha blocker. If I guess it right.
