Prostatitis with zero pain, just horrible 24/7 urge to urinate/frequency

Hello All,
I am a healthy 25 year old male with no history of urinary issues.
I am at my wit's end. Last Sunday, I started having terrible urinary frequency. I will literally go to the bathroom, urinate, flush the toilet, and then have to use the restroom again RIGHT AWAY. In other words, I have constant urinary urges 24/7... It has been this way for one week.
I don't have any pain or discomfort in my pelvic area, and there is no burning sensation during urinating. However, I feel as if the urine is at the tip of my penis - that is where the urge sensation is most prevalent. If I hold it for a long time and it accumulates, there is quite a lot of urine. However, most of the time (especially in the middle of the night) hardly anything comes out even though it feels so urgent.
I can't sleep since it feels like I have to urinate constantly... last night I was going every 5 minutes, and most of the time only a few drops came out. I am having anxiety now because of this.
I went to the urologist last Tuesday (2 days in). Urinanalysis showed no UTI and bladder ultrasound was okay. However, during the rectal exam, he said my prostate was "off" and did a test of the urine that came out after the exam. At that point, I was told that I had a prostate infection and that the enlarged/inflamed prostate was causing the frequency. In other words, there was bacteria.
I was given 500 mg of Levaquin to take once a day for 30 days. However, it has not helped after 6 doses. I'm trying Motrin (ibuprofen) to no avail. I was told it takes a while for the antibiotic to reach the prostate. I have no fever, and no chills.
I am terrified that I have the chronic pelvic pain (CPP) form of prostatitis. However, I don't know if I do, since (1) there WERE bacteria present upon running the urine after the rectal exam and (2) I have ZERO PAIN. The only symptom is the 24/7 need to urinate. Any ideas on this? I hope I have the acute prostitiis form since this is cured with more antibiotics, but if I have the lifelong CPP form I don't know what to do... I feel like I will have to quit my job since I can't sleep with the need to urinate.

I cannot believe how many people have the same symptoms and still nobody is 100% on what is causing this.
I've had my symptoms for 4 months now, had various urine tests, psa,bloods,ultrasounds etc and still nothing found. Antibiotics do however work but it is not a long term solution.
I will have the odd week which is 100% normal and then it all comes back.
I believe that in most people this is stress related. As mentioned somewhere above when concentration levels are high as is exercise it isn't noticeable. When I sleep I'm fine, when I'm not thinking about it I'm fine.
If I masterbate I also feel it relieves the urges.
Has anyone above actually cured themselves fully?? It certainly gets you down and makes you anxious 24/7

Not yet. Mine started about a week ago. I was sitting in my bed getting freaked out about another issue in my life by reading possible outcomes and BAM, it just started up. I think you're right... 100% stress related IN SOME CASES. Here's the issue though, every single urinary issue, whether it's bladder, urethra, kidneys, the tubing, the muscles, the prostate, etc... they all have one thing in common when something is off... frequent urination or urgency to go. So there's probably 35 people in this thread and they might all have different issues with just this one symptom. I'm the same as you though, in that I have no issues through the night when I get to sleep, if I lay on my side the sensation decreases, and if I don't think about it, or force myself to relax, the sensation is gone. I still think I have some sort of infection as my flanks hurt, and I did place balmex which is a zinc oxide cream on the head of my penis one night and the symptoms were gone... almost instantly. It's meant for diaper rash.
I had this similar sensation years ago right after a surgery to remove a testicle affected by testicular cancer. It was chalked up to an infection, not sure on diagnosis, and then it eventually went away. I still get the dribble sensation a lot after I pee, like I need to clean the tube, and normally once I do, it goes away, but this time, it's sticking around. I have an Rx for Cipro from my doc which I start tomorrow, but I've been doing squats and went for a jog earlier just to try and get my pelvic muscles to stop spasm'ing if that's the issue. No real change, but we'll see over time. What sucks is I when I had this issue before, I worked at a busy restaurant, so every day I was getting plenty of motion and exercise, so it wasn't nearly as prevalent, now I'm mostly glued to a chair and a desk. There's still the notion that this is entirely an anxiety effect and this is a fight or flight response. If you feel constantly stressed and overly-anxious, that might be your cause.

For everyone asking I'm about 90-100% back to normal. What I had was a weak pelvic floor due to large amounts of stress with work. The Uro said this can happen to anyone at any age and there is nothing to worry about. It usually resolves its self on its own and in my case lasted for about 5-6 months. However in some cases its not uncommon to need to go to PT to help resolve the issue. If you do go to PT make sure its someone that specializes in the Pelvic Floor or else you are wasting your time. I think a lot of people on this forum are having different issues. Make sure to find a uro specialist that can help you figure out whats wrong. Our bodies are like machines and machines sometimes need fixing. There is an answer for whats going on its just finding the right person to figure it out. In my case the uro doctor I saw was very knowledgeable and familiar with the issue I was having. Also I ended up taking Lexapro for stress since I've had really bad stress and anxiety since I was a kid and believe this is what caused my pelvic floor to become damaged. Hope this helps someone out there that has the same thing I did. Good luck everyone I know how much this can suck.

Glad to hear about your success, I've begun doing more squats and jogging, but I'm on 500mg Cipro right now and it seems to be helping. However, I need more mobility that my desk job will not permit, and unfortunately do not have time to work into my schedule right now. Any helpful tips or resources to stretches, and exercises that helped you the most? Mine is tough to corner, but I recall getting the feeling after becoming extremely anxious. I know i can over-inflate issues in my head, so I'm trying to relax more. The warm towel trick around my testicles and shaft normally helps, or has helped.

Yea sorry that's what I meant to say. I was told if you are stressed you will tighten your muscles and if done for a long period of time will cause stress on them and they will become tight/sore/damaged etc. For me it so happen to manifest in the pelvic floor muscles.

Yea sitting for long periods of time and or tightness of pelvic floor area due to high stress both which i can relate to will for sure give you these symptoms. For me I got lucky and didn't do much stretching as it started to go away slowly on its own but my uro said for some people it requires PT specialists who know about the pelvic floor and can take longer then others to go away. I took Cipro too. Don't be fooled though because my uro said antibiotics can help with non bacterial things and sometimes gives you the impression as if its going away. If it comes back after taking Cipro dont stress because it might happen. I highly recommend contacting a local PT that understands the pelvic floor.

I am having a pelvic tension and most of my current problems are urinary. Not emptying the whole thing feeling, frequent urination, waking up at night.
Masturbation makes it worse bc it contract pelvic muscles i assume.
It sometime goes away on its own, those times i believe pelvic muscles return to its normal state for a while. But then somehow it flares up with masturbation, a vigorous exercise , or sitting to much
I think it is about streching but more importantly relaxing those muscles. I couldnt find a way that i can completely get rid of the situation.
Sometimes wine helps, probably bc of the relaxation effect. But i cant drink all the time.
I will see a pelvic floor specialist in a month or so. I will update later.
What are your symptoms exactly?

Well it's weird for me, I started to see improvement a day before going on cipro. On cipro it's back and forth, sorta, but it seems better than before. I was stressing out the day it kicked up, but I am able to see some release of the tension when I just relax or don't think about it. So I'll try and few pelvic relaxation techniques, and finish out the cipro and see how it goes. I used to have a painting job where I could walk around all the time, and now I don't have that much freedom, but I'm going to try to walk or jog more if it'll help. I just can't afford to search out expensive PT trainers so hopefully I can manage this.

My symptoms are a leaking sensation of urine, and at first there was a strong urgency to go. The urgency crops up typically after urinating first thing, or even throughout the day, or after I masturbate. So it's either an irritated urethra, UTI, or pelvic floor. I have no real pain in my pelvis besides on my butt where it meets the seat, but when standing there's no sensation really, or issues. It just feels like urine is leaking or stuck in the urethra, but no matter how hard I wring it out, nothing ever comes out unless I'm aroused and it's lubricant.

I'm feeling the same as you, this has been going on for almost 2 weeks and it's driving me crazy. I went 3 days after the sensation started to the ER and they did an urine test and didn't find a bacteria or anything, they told me to just drink a lot of water. The feeing didn't go away so I went to a clinic and the doctor thought it was an UTI so he gave me "Sulfamethoxazole" but it didn't work, I took it for like 4 days and the feeling was the same, so I did an urine culture and he didn't find any bacteria so he gave me "Doxycycline" I took it for 7 days and nothing changed,I still have the same feeling of having pee in the tip of my penis and it is so annoying, I pee and 30 seconds letter the urge comes back. I have already waste so much money. I have an appointment with an urologist in 3 days. does any one know any medicine to cure this.
