Sudden bifurcation of urine and itchy urethra

Hello, I'm 31 years old male. I'm posting here because maybe i can find answers.
Around a year ago, I suddenly while sitting infront of the pc started to feel pain in the left testicle. I remember vaguely it happened after I masturbated like 3 times in a day or after i slept with my wife when my bladder was full. Hard to remember after so long. Anyway I went to a GP and he thought i have epididymitis and i have been given doxycycline for 2 weeks. About 3 days into treatment I suddenly felt constant itchiness and burning in the distal part of my urethra and it goes away for couple of seconds while i Pee. The itchiness and burning is in the area of the fossa navicularis, maybe a bit below the glans penis. Anyway not only i felt constant itchiness and burning I started to have two streams of urine, the higher one was strong and lower one was dribbling. Usually when i have alot of urine the stream was more less as one. But as the voiding came to stop the urine started to bifurcated again. I also started to notice my flow is less strong and the diameter of my urine is less wide like it was. Before it i remembered i peed with no problems at all, maybe went more to the toilet than other people but i drank a lot of coffee. Anyway after finishing my treatment the pain in the testicle went away and I went to the Urologist who thought I have prostatitis and gave me ciprofloxacin. I must say I thought it wasn't the prostate because I knew the pain is only in one place in the urethra, even when i ejaculated the semen came a bit bifurcated (gushing bifurcated not shooting. sorry for the extreme details). Anyway I took the the antibiotics and did vast STD blood test, urine culture and US of kidney and bladder. Everything was perfect and normal. after 3 weeks of taking the pills i was still suffering from itchiness and bifurcated stream. My urologist thought there is nothing he can do and so he thought I have chronic pelvic pain syndrome and the urine burning and itchiness is a refereed pain and the split stream urine is because of the prostate.
I do not think it is a CPPS. I think it is something with my urethra, I once put inside Dermacombin ointment and it felt a bit better afterwards but my urologist didn't recommended it. He said there is nothing he can do for me. and so I have been suffering for the last year the same thing. very very mild itchiness and tingling in the distal part of the urethra, sometimes when i study and work i don't feel a thing but if i touch the area its like inflamed again. I try not to touch the area so much not to irritate it. I still ejaculate with a bit bifurcation and it still not shooting and only gushing. I wonder if i'm stuck with this thing for all my life. I noticed my stream is quite the same, I can be quite strong if i drink a lot of fluids but there's always in the end of voiding 2 streams of urine and one the lower one is more dribbling. And like i say i think the diameter of the urine is smaller than before.
And now, 1 week ago I started to have the same pain in my testicle so I went to another urologist because now the left testicle is much higher than before i thought i have a torsion so he did an ultrasound and saw very mild inflammation of the epididymis (some fluid and thicker epidiymis). So im back to ciproflaxcin for 10 days. I told him about the split stream urine and the very mild itchiness and he think it is nothing.
I would love to ear your thoughts.

Hey guys, thank you for sharing your story! I guess I'm stuck in the exact same situation and it really helps reading your review.
Upon meatal spread my urethral opening looks the same. I wish I would have inspected my urethral opening before the symptoms started because now I am left with nothing to compare it with. My doctor however, told me multiple times that it looks completely normal.
I was wondering, how are you feeling these days? How often would you say does your itch "once in a while" occurs exactly? Once a day, a week, a month? And most importantly what did you do to overcome it? Did it just went away by itself at some point?
I got tested multiple times, just as you guys did, with no result what so ever. I also took Doxy, Metronidazole and Moxifloxacin (similar class like cipro) but it did nothing to be honest. Symptoms just started to shift from one penile spot to another.
I really look forward to hearing back from you, thank you!

Hey dude, I hope you are doing better. I am going through almost the EXACT same thing! Itchy, really sore feeling in my urethrea that is just where the shaft is and to the tip, sometimes it feels worse sometimes a little better but constantly there.
I got tested for counteless STI's, Mycoplasma, Ureaplasma, Trichomonas etc etc all negative, I tried Doxycycline 100mg 2 x per day for a week without changed.
I am seeing a urologist in 3 weeks, its been 2 months with this awful discomfort. I like you tried a topical agent, I used instillagel which is Lidocaine and it numbed the feeling so I know its local to the urethra, sometimes the prostate hurts a little when I urinate now and again, but if my urine is concentrated it burns the area in the urethra. I was sure I had an STI as it started after a sexual encounter, however I got tested 3 times for every STI on the web lol all negative.
So I think its some sort of prostatis or urethra infection, maybe Ciprofloxin will help me too so going to ask my uro!
Over all, how are you now? What helped most? The Cipro?

I love to see that I'm not alone with this "however I got tested 3 times for every STI on the web lol all negative.". I am about to get a cytoscopy in two weeks, let's assess the damage.

Guys, I have the exact same issue for over a year. Doctors think it is prostaitis and got many antibiotics. Some helped with the prostate. Meropenem for 6 weeks. Cipro / Levo for multiple weeks. But, nothing seems to be help it permanently. Prostate is better but not urethra. Also, taken Doxy but no improvement. I also developed balanitis probably due to antibiotcs over use so was given Fluconazole for two months. Didn't help. All STDs came back negative. After being without antibiotcs for eight weeks, I did a urethral swab culture. It showed heavy growth of Klebsiella pneumonaine. Cefixime helped it a bit but did not solve it. I have also heard these statements from doctors this is referrred inflammation from the prostate which I don't agree with or its due to CPPS. It also gets worse with ejaculation. Will keep you guys updated of my progress. I will ask the doctor to consider cystoscopy so far he is relucant bcoz my urine flow rate is normal, there is the bifurcation and dribbling as described by t14010.

In addition to Klebsiella, it showed few colonies of Staph species (not Staph aurues). Infectious diesease specialist said, I will ignore Staph as its normal flora and gave me antibiotics to target Klebsiella.

Have you ever gotten tested for herpes/hsv. It's not a typical std test, you have to request it separately.

Did you experience any sort of mild clear discharge from urethra?
