Sudden bifurcation of urine and itchy urethra

Hello, I'm 31 years old male. I'm posting here because maybe i can find answers.
Around a year ago, I suddenly while sitting infront of the pc started to feel pain in the left testicle. I remember vaguely it happened after I masturbated like 3 times in a day or after i slept with my wife when my bladder was full. Hard to remember after so long. Anyway I went to a GP and he thought i have epididymitis and i have been given doxycycline for 2 weeks. About 3 days into treatment I suddenly felt constant itchiness and burning in the distal part of my urethra and it goes away for couple of seconds while i Pee. The itchiness and burning is in the area of the fossa navicularis, maybe a bit below the glans penis. Anyway not only i felt constant itchiness and burning I started to have two streams of urine, the higher one was strong and lower one was dribbling. Usually when i have alot of urine the stream was more less as one. But as the voiding came to stop the urine started to bifurcated again. I also started to notice my flow is less strong and the diameter of my urine is less wide like it was. Before it i remembered i peed with no problems at all, maybe went more to the toilet than other people but i drank a lot of coffee. Anyway after finishing my treatment the pain in the testicle went away and I went to the Urologist who thought I have prostatitis and gave me ciprofloxacin. I must say I thought it wasn't the prostate because I knew the pain is only in one place in the urethra, even when i ejaculated the semen came a bit bifurcated (gushing bifurcated not shooting. sorry for the extreme details). Anyway I took the the antibiotics and did vast STD blood test, urine culture and US of kidney and bladder. Everything was perfect and normal. after 3 weeks of taking the pills i was still suffering from itchiness and bifurcated stream. My urologist thought there is nothing he can do and so he thought I have chronic pelvic pain syndrome and the urine burning and itchiness is a refereed pain and the split stream urine is because of the prostate.
I do not think it is a CPPS. I think it is something with my urethra, I once put inside Dermacombin ointment and it felt a bit better afterwards but my urologist didn't recommended it. He said there is nothing he can do for me. and so I have been suffering for the last year the same thing. very very mild itchiness and tingling in the distal part of the urethra, sometimes when i study and work i don't feel a thing but if i touch the area its like inflamed again. I try not to touch the area so much not to irritate it. I still ejaculate with a bit bifurcation and it still not shooting and only gushing. I wonder if i'm stuck with this thing for all my life. I noticed my stream is quite the same, I can be quite strong if i drink a lot of fluids but there's always in the end of voiding 2 streams of urine and one the lower one is more dribbling. And like i say i think the diameter of the urine is smaller than before.
And now, 1 week ago I started to have the same pain in my testicle so I went to another urologist because now the left testicle is much higher than before i thought i have a torsion so he did an ultrasound and saw very mild inflammation of the epididymis (some fluid and thicker epidiymis). So im back to ciproflaxcin for 10 days. I told him about the split stream urine and the very mild itchiness and he think it is nothing.
I would love to ear your thoughts.

Hi all did you get to the bottom of this?! Ivr had the exact same as all
of the above with similiar in my uretha. completed endless STI checks in the region of 10+. i was diagnosed with chronic prostatitus after a cystcyscopy, ultrasound and countless bacteria checks. how have you guys got on?

so is it a for sure prostitus for you ? ive done everything and the cant find answers doctor diognosed me with prostitus urethra issue sometimes it burns bad when im trying to sleep and i do feel a wierd feeling around my prostate sometimes what about you ?

Yeah diagnosis was chronic prostatitus which had the potential to be bacterial. i dont believe it was bacterial but that was the diagnosis. I have flare ups still usually due to stress like you causes a pain or feeling in the prostate like im sitting on a rock. Severe flare ups cause all the issues i had previously but after every check under the sun, I feel more at ease with it. i also had the burn when sleeping and sometimes when urinating - If youve been diagnosed and have checked all other things for bacteria, STIs and prostate issues I say try and get in a routine of exercising like walking/running everyday it really helped me. If anything it took my mind off it, think the worry/stress constant thinking about it made it worse for me and made flare ups more difficult than they needed to be. Nowadays i have a flare up i put it down to feeling low and focus on other things - it soon calms down. hope this helps you, even in the slightest i remember i really struggled the first few months it sunk me to my lowest with worry not knowing what it was but its manageable now.

so what has helped your glans to stop being inflammed? right now im on a 30 day treatment for prostatitis bacterial right now

Are you on clarithromycin and amitriptyline? my urologist gave me amit but in all honesty i didnt find it helped with anything and seems to be the gontonfor consultants to put you on and hope it helps. Daily regular exercise and finding things to take my mind off it helped me personally. Being less stressed was my main item, long term antibiotics should reduce inflammation but need to be longer than 6 weeks i believe (im not a medical trained but this is what i was told) i think it does help especially if its a bacterial infection. As mentioned though i dont think mine was but its sort of last resort diagnosis in the UK

the meds im on is sulfameth which meds is the one that worked for your inflammed glans ?

ya its driving me and many of us crazy i see alot of people on here going crazy even wanting to suicide just trying to figure out what it is i tried everything, but ya i do feel like something is inside my area where the prostate is pulsating slightly and after hott baths its really red hopefully it is prostitus

Hello all, after almost a year and a half it went away by its own, no more pain, maybe once in two weeks for couple of mins or hours but usually no pain.
The urine is still not as strong as it was, when my bladder is full and I have a strong flow, I can say it's allmost 90% of what it was. When I have weak flow because my bladder is not full, I still have split urination. Maybe I have scar tissue somewhere near the fossa navicularis? Probably it healed... Even when I ejculate it's split semen haha
Because I usually don't have pain I didn't do the cytoscopy. Maybe I will do it someday.

Hey mate.
I have been searching all over the Internet for a case like this.
I have had this for 4 weeks now. My life is currently a living hell. I can see a slit line deep into my utheria. Have tried doxy and macrobid and nothing has come worked so far.
Have done every sti now and all come back negative so far via urine.
Today they took a swab for herpes on the redness but the nurse is not confident it is hsv.
Can I ask, is there any particular med or cream you felt soothed the utheria?
Atm I am on naproxen and constant hot water bottle when I'm at home, its made it half bearable.
I have to get every drop of urine away when I pee. But the act of peeing doesn't actually hurt. Its just after.
If you have any ideas at all please get in touch!

hbing the same problem no positive stds no hiv no herpes but penis wont work no senstation constant feeling of pressure in my bladder and a whitish sore in my urethra doesnt burn when i pee but burning does come and go and itchy feeling

how did you go with the hsv test mate?
Was it negative?
I have similar symptoms, but after reading heaps suspect it's nerve related or prostatitis. Burning comes and goes. Feel heat in the scrotum and penis and an urgency to pee with a burning feel, but not while actually peeing.
Some one tell me you all tested negative for hsv as that's my worst nightmare.

just to add to that - I've tested negative to hsv three times and all other STDs multiple times too. No HIV or any of the common STDs either.

I have the exact same issues marks inside meatus i think it's either Cpps as one doc me i have CPPS but other doc just put finger in my ass said you don't have prostatitis, but i think its either prostatitis or It's some HPV strain.

Thanks for sharing your experience.
What treatment did your doc recommend for CPPS? Has it worked in relieving your symptoms?
I have an appointment with a specialist tomorrow, so hope to find put what's going on.
On a side note, did you mean to say HPV or HSV? And why do you think it's that?

Brother still suffering from this at first i have burning in urine and penis glan got red inflammed like hell my symptoms appear after 2 months of exposure and first i got uti burnung in urine inflammed glan and foreskin and redness after tsking doxycycline my burning got reduced after a week, i got pain in testicles majority in left side and burning has come back a little then i go to a physical doc who just saw my mark but not very attentively after listening my symptoms he said i have cpps and gave me sensival my burning and pain in testicles gone then i show him picture of marks he said do circumcision but i didn't do that i think it can be some HPV strain as i have taken swab test of that white mark it's neg for both hsv1-2, one thing i have Chlamydia igg blood test positive i haven't taken igm so i can't say it's because of Chlamydia. if you want any further information msg me we can connect so that we can find the solution for this together
