Has anyone had to have a iliopsoas release??

    This is my first posting in this site so I wasn’t sure whether to start a new post or respond to someone else’s post. It sounds like we All have the same or similar problems, significant groin pain after a THR. I am about 4 months out - had little to no pain after 8 weeks and started to increase activity including stretching.  I, along with my surgeon, feel that I injured to psoas tendon.  I had a steroid injection and at that time 20cc of blood tinged fluid was removed and was placed on Mobic.  My surgeon has not been very responsive - difficult to get an appt, etc, etc.  While some of the pain is a little reduced but not by much, stiffness has increased.  MRI, of course showed nothing except fluid and No on has been able to accurately interpret the ultrasound other than there is fluid.  I’m looking for second and maybe third opinions and will go anywhere in the country.  I had a Labral tear arthroscopy at the Hospital for Special Surgery 8 years before the hip replacement with excellent results and the hip replacement done elsewhere was the anterior Approach.  I ha ve been reading the posts about tendon release, hip revision, tenotomies.  Although it’s still early, the fact that I had little to no pain and the  pai starting around 8weeks and worsening over the next six weeks, I believe the only solution will be surgical.  Two surgeons, including mine, feel that there is no problem with the placement of the prosthesis and one, my surgeon, is pretty convinced that I injured the tendon.  I am not happy with my surgeon and will definitely not go back to him.  There is one specialist in my area that I will see and I think I will also go the HSS in NYC but am concerned about your post.  After having this disabling complication, I’m out of touch with The physicians in NYC.  Would you be willing to provide any other info about your negative experience at HSS, like maybe doctors initials or some other descriptive info.  Even though I had a good experience with my larval repair almost 8 years ago, things can change.  Ideally I would like a doctor who is equally competent with arthroscopic as well as thr.  

    Also, there are a lot of older posts of people who were going to have a psoas release (the type In which the tendon stays connected or is reconnected but is stretched to a longer length allowing healing) but I couldn’t find any posts of how these people did in the long term,.,

      Hi Susan. 

      You sound like me!! I’m 48. 

      I have psoas impingement following anterior THR 7/2017. I am scheduled with a Boston surgeon following a 2nd & 3rd opinion for a complete THR again on  4/26. He going in posterior though- better view, less trauma to psoas tendon etc. total bummer and of course I’m terrified. Neither surgeon recommended the release for me... we suspect it’s the cup continuously rubbing the muscle.

      Hi Susan, I just read your post and think it maybe is meant for me, as I am not happy about my experience with HSS. I'll try to reply to you fully tonight or tomorrow. Currently on a tiny phone waiting for a City Hall agenda item...so boring!

      Hi Maysie.  Great.  I would appreciate that.  The surgeon I had many years ago at HSS that I was very happy with is M.B.  I was planning on getting an opinion from him.  Once you have something that doesn’t go well, the whole situation makes you very fearful.  At this point, I don’t even have a diagnosis except inflammation of the psoas tenon with an effusion.  I don’t know if the cause is the cup or if I overstretched or over exercised and the surgeon who did the THR has not been helpful at all.

      Hi Debhip.  I am so far just waiting for  second opinion from a surgeon in my area but will definitely seek other opinions.  Once something goes wrong, everything  becomes very scary.  I have completely lost confidence in the surgeon who did the THR.  I don’t have a clear diagnosis as to the cause other than the psoas tendon is inflamed.  How were your doctors able to determine that your problem is the cup rubbing against the tendon.  Could they see this on X-ray?  I am looking for doctors for second opinions for a confirmed diagnosis and then for treatmen.  they could be located anywhere.  Do you live in the Boston area of did you decide on this doctor through recommendation.  I was given the name of a Boston ortho surgeon years ago for a knee replacement.  The recommendation came from a highly regarded thoracic surgeon I used to work with in NYC.  I will have to look for his name but am wondering if it’s the same person who will do your hip revision.       Does sound like a bummer.  All of us who have had a problem are terrified.  It’s great to have a site like this.  I hope it’s a little reassuring that 3 surgeons have the same opinion that your problem is caused by the cup rubbing on the tendon.  If I knew the what I know now, I would have had the posterior approach.  I was actually scheduled for the posterior approach THR and canceled it after the surgeon told me I would have precautions afterwards.  Not only did this guy do the posterior approach, he was also clearly the best rated ortho for hip and knee replacements in the area.  I wish you the best and continue to post your progress.

      Hi Susan,

      Your situation sounds EXACTLY like mine . I also had steroid shot and a drain with blood in fluid. I will say that the second steroid shot seemed to work better, the doctor did it different than the first, he went in under the muscle. I don't think I will ever be right and wish I hadn't had the original THR, but not willing to go for more surgery. My doctor was Dr. Segal in Bayshore/Southside hospital.

      Hi.  Yes, it is terrible and I’m willing to do anything.  I had THR in Asheville, anterior approach.  Didn’t get much help with steroid or One month of mobic.  I was put on complete rest.  Things have changed since that time.  Pain more diffuse.  Band of tightness that goes around the entire hip with pins and needles sensation.  I started to stretch the psoas gently and do tiny amount of strengthening.  Feels better for about 15 minutes after stretching and the  tightens up again. See second opinion in Asheville in 2 Weeks and then plan on going for third opinion with Dr. Bostrom at HSS.  He did by labral tear years ago and I had great success for a number of years.  I’m hoping HSS has better diagnostics than Asheville.  Once I get more information may also see dr. Ross in Florida.  I feel like I first have to rule out the cup rubbing against the tendon.

      i was feeling a little isolated and alone over here in Oz (Syd) not having anyone to talk to or compare.

      Iv had 5 THR  2 dislocations and 2 posts tendon releases in the last 9 years.

      the last only 4mths ago.

      iv had prostheses that were to big or to small, and a crazy trapped tendon im now just starting to relaps back with the tendon pain. i still struggle to walk far, getting in and out of a car is hard works haven't driven now for 3 yrs because i don't have the strength in my leg anymore, but have been told just learn to drive with your left foot,im so angry and frustrated. i hurt to go to bed ,sleep and get up the next day.i was cut through the side of my leg about a 12in scares also have nerve pain and numbness.i see my surgeon next week.

      i guess theres many of us out there doing it tuff sad

      Why did you have 2 psoas releases? who was your doctor ? did he do a full release at the lesser trocanter or a cut at the central compartment? did you have the weakness in your right leg after your tw0 dislocations? can you lift your right leg at all? Sounds really like a lot of weakness for a psoas release...sorry this happened to you...

      Is MB ? an arthroscopic surgeon at HSS? I have never heard of him? I had a release done with Struan Coleman...good surgeon ..no weakness in leg but not enough released at central compartment...need a Full release at Lesser Trocanter...much more room..should not be much more weakness (I had weakness in my leg for one day with my first tenotomy..everything went fine except STILL HAVE GROIN PAIN but walk much better).

    I had my tendon cut along with my posterior RTHP. I'm 12 weeks post op & am just starting to notice some more flexibility in my right hip. I'm hoping, in time, I will be able to sit cross legged on the floor, which I've never been able to do. I had very little groin discomfort, but I was also going through THP recovery, which overshadowed everything else.
