Turp experience and questions

Hi there,

JUst wanted to share my experience with my turp surgery. I'm 45yrs old, too young some say for me to having prostate issues but I put it down to my father side of the family (my father and uncles all currently suffer from enlarge prostates)

For the last couple of years been suffering from weak urine stream, having a full night sleep and waking up in the morning with the urge to go wee just to wee a small trickle. I also suffered from time to time having blood in the urine.

Went to see the Urologist, he did some testing, test on the strengh on my urine flow (very weak the doc said) he put a camera up my penis to see inside and we could see clearly that my prostate was enlarge and was putting pressure on my bladder which probably explained why sometimes I could see blood on my urine. I have a benign enlarged prostate, no cancer worries which is good.

While I decided if I wanted to have surgery, doctor put me on Avodart. It did help but I wasn't sure if I wanted to take antibiotics for the rest of my life. Avodart temporary reduces the size of the prostate but if you stop taking it, you will still have the same problem again and it will only get worse.

Dedided to have the surgery, I had it three weeks ago. spinal block, stayed in hospital one full day with a catheter inserted. Catheter was removed the following day in the morning, had to urine a few times into a container, followed by a scan machine pressed to my belly to see how much urine was left in the bladder. Passed the test and  was allowed to go home that day at midday.

I can see the immediate benefit of the surgery where my urine stream is very strong. When I get up in the morning with a full bladder, no more urine trickle, just a strong urine flow. On the other side, I'm still suffering from post surgery effects, for example, frequent urges to go to the toilet and pee ( I probably go every 40 to 60 minutes) blood on the urine and pain (burning/stinging) everytime I have to take a piss. The doctor says is normal for this symptoms to happen and it will take weeks before everything is back to normal. how long?  it depends on the individual. I can get an erection, it feels a bit different and I will suffer from retro ejaculation ( I was warned about this)

Anyone that had Turp done, how long it took until you where pain free? (no blood and pain)  Did you take any medicine to stop the pain or the bledding?

Thanks for reading,

Henry

see another doctor asap.  Its not a crime to get a second opinion.  Its the wise thing to do after 6 weeks of suffering.

I just happened to run into this forum and have read some interesting/useful poats. After reading some, I don't feel my situtation is unusual.

This has been my experience with my TURP surgery (I am in Chicago).  It took about 1.5 hours to have it done. In recovery I felt no pain of any kind, anyhwere. I was transferred to a hospital room for the fluid irrigation while the cather was in place (placed after the TURP surgery). The nurses hooked up a total of 6 bags of fluid (saline) over 12 hours. The mornning after the surgery (about 10 hours after the surgery)a nurse (upon the uro's ordrs) helped me walk in the hallway, about 200 feet total.  I did not have a problem. The urine voided into the bag was tainted with blood but the uro said it was the right color. I was discharged from the hospital early afternoon (about 24 hours after the surgery). I managed well at home, drinking lots of water and eating fruits as well as a regular dinner. There were no blood clots in the bag.

I had the cather in place for about 3 days. After the catheter was removed I was surprised by the force of the stream. An interesting thing about the bleeding was that my urine was clear yellow by the 4th day after the operation; it remianed clear yellow for 2.5 days. Then there was evidence of blood again but I was not concerned; it wasn't a strong red color, almost like rose when I emptied the bag (and I would empty it when it had about 200-300 mls of urine).

My F/U was two weeks after the surgery and my urine was still red. I noticed that the darkest red was in about the first 25-50 ml of urine and then it was clear yellow. The uro explained that the urine sitting close to the prostate came out first as the most bloody and then the urine that was in the bladder flushed that out. He also mentioned that the last traces of the flow could be red also.

I am in my 4th week now and there is still blood. But it varies on my activity (walking/stair climbimg (slowly)) or how long it's been since I last urinated. Since it s the fourth week,  I did become concerned about the blood traces. But reading some of these posts I feel better about it.

Now, some eposts have indicated the size of their prostate (before/after and removed). I don't understand some posts mentioning 100-200+ gram sizes of prostate! My pathology report indicted that 13 g were removed (and about half of mine was removed).. My uro said that they assume 1 cc = 1 gram (just like the density of water, but I imagine the density of the tissue will be greater than that of water). I have also read that the size of the prostate (normal) can be as much as 25 grams. So the 100-200+ grm sizes astounds me!

I have not tried having any sex yet. I think I will wait until all the bleeding has toppped (I cannot believe it will have been months without sex or masturbating!). Becasue of my situation I had a cather in place since early April and did not have the surgery until early May. And that was only because there were other things happening in my life and in my family's life that I had to postpone the surgery. I have been able to get an erection (I've noticed many nocturnal erections) so that's a plus!

I have noticed that I urinate more during the day compared to evenings. And this despite the amount of water/fluids I take in being relatively constant. I measure the volume of my voided urine and it is VERY easy to void 200-250 mls at a time, with a strong stream. I see small blood clot clumps (single ones) now and tehn. But only about 10x in a week's time. I have also seen what I thought were small pieces of tissue. And now having read some of the posts here, I realize that probably was the case and that it was not uncommon. I did notice that after doing some gardening (crouching, digging) that there was more blood in my urine. I realize that I should NOT have done that gardening! I have not seen that much blood (color) since then.

Three weeks after the sugery I was still feeling that sharp feeling when I urinated (not necessarily painful, just annoying) and an ache in my pelvic area. It was Memorial Day weekend so I dedided to go to an urgent care center and ask for a UTI evaluation. My cultures turned out negative, so that was a relief.

Presently I am hoping the bleeding will stop/subside more by the 6th through 7th week (after reading some posts here). Otherwise, I have no problems/complaints at this time. I know someoen who had the laser surgery and he has seen some blot clots 13 months after his surgery. I am hoping I will not see that after some point in time.

I wish all those who have recently had (or are planning to have) a good recovery.

Joe I believe the pepole with the smallest prostates have the easiest recoveries. I had a turp in June, 2015. My prostate was 313g., and about 100g. was removed. I am taking dutasteride every day to keep it from regrowing quickly. My Nov., 2016 sonogram showed it was up to 235g, even with the dutasteride. I would say your turp recovery has been great. Rose or even burgundy colored urine is normal urine colored with just a small amount of blood. (Hemorhaging of the prostate produces thick, spaghetti sauce color and consistency blood that clots in seconds. Your prostate is still healing, and it sounds like the blood should be gone in a few more weeks.

Joe -  I live near San Francisco. I'm 8 weeks since my TURP procedure. My post op problems are really mild compared with others on this site. The only problem that is continuing is blood tinted urine. I will have 4-5 days (even a week) of normal yellowish to light yellow urine then the next stream is pink to red tinted with a few blood clots. The next urniation starts to clear up and I'm back to normal for a few more days. This doesn't seem to have any relationship to physical exercise. It just randomly happens.

What is/was your PSA for such a large prostate ? Just curious. Thanks. Hank

      Oct. 2014 it was 6.0

      TURP in June, 2015

Nov. 2015 it was 7.8, started Dutasteride

Dec, 2016 after a year of dutasteride it was 1.6

(They say to double your score if you take dutasteride, so that would be 3.2).

      Thanks Bob,

it seems Dutasteride did better than halving your PSA. Great for you. Hank

So I am on week 5 after the TURP and my urine has been mostly yellow for 4 days now, the longest strecth of tie, so far for yellow urine. Reading more posts on here, some guys mention they have/had prostate and/or bladder spasms. When I had the catheter in ppace for ca 5 weeks before the surgery, a few weeks in I felt an ache in the pelvic area. Is this what some of you refer to as prostate/bladder spasms? If so, perhaps they were bladder spasms (since it ws before the surgery)? The ache is not painful, just annoying. And if prostate spasms are sometimes a result of post-op, I wonder if that is the sensation now?

I have a F/U next week and that will be to measure my PVR.

Currently I am also still feeling the sting/burning sensation when I urinate, but it seems stronger AFTER the urnination. And at times I feel a slight pang of sharp sensation in the urethra (not painful, just annoying). Do any of you gentleman get that feeling also?

Sweat you're feeling is normal. It will fade with time, but be patient because it will be some time ever yet before you are completely healed. 3 months isn't an exaggeration.

Auto correct......annoying! "WHAT you're feeling is normal."

It took me a good 3 months before things got back to normal. However if you feel something is wrong, contact your urologist.

I had bladder spasms while the cath was in, after the turp. I think it was the result of the balloon in the cath irritating my bladder. They were eye popping painful for about 10 to 20 seconds or so.

I also had the extreme pain at the end of urination while I was recovering from the turp. Felt like being stabbed. Also eye popping painful. Rather than take the pain pills I would hyperventilate when urinating to distract myself. Also used the hyperventilating to get through the bladder spasms.

Even I did TURP,I'm fcing the same problem. My operaqtion was trhough my penis inserting catheter. 2-3 days I was in the hospital my urine flow is normal,I get burning sensation while urinating. Now my problem is retrograde  ejaculation. Semen getting out all of a sudden but not passing through the penis.

I would like to know this, retrograde ejaculation will continue or it will get normal?

Your burning will eventually go away. I had my turp almost five weeks ago. Peeing better but still burning when urinating. The retrograde for me is most likely . However the peeing was my concern and hope it continues , I'm sure it will.

Hope you continue to heal like me. A long drawn out process but

both of us will heal in due time.

Most patients will have retrograde (or anejaculation) permanently unless a duct-sparing surgery/bladder neck sparing surgery is particularly performed; if a duct sparing surgery IS particularly performed, the efficacy of this particular operation will be lower, and it is not guaranteed that the ejaculation issue won't occur.

I had both Green Light laser and TURP. Burning stayed for a few weeks but ewent away. I was fortunate 're retrograde ejaculation.....only get it 10% of time. But I would take that over being able to pee normal.

If you can't pee right then eventually wrong kidneys will go years down the road. I did not want to use a catheter so Turp was my only choice since it was not my prostate the problem it was the bladder spincter and a inflamed prostate which was cleaned and scraped. I'm 67 want to make to my much later years with kidneys intact. Retrograde is not that bad.

Thank you so much for your comment. While most of us men would prefer to not have RE, I feel that it is worth it to not have to deal with the frustrations of BPH. We are aware of procedures that might correct BPH AND also allow for normal ejaculations, but if preserving that ability at the expense of true BPH relief is the downside, for me it is something I will deal with. As a result of my Plasma Button Vaporization I do have RE, but to not have to constantly be searching for restrooms makes it quite easy to live with.
