Blotchy red rash/irritated skin on penile head (lasted for over 3 moths now)?

Part of the skin on the head of my penis has become slightly red and is causing some pain/irritation.

It first appeared the day after I used a condom so I think I must have had a reaction to the latex or some other substance in the condom.

The tip of the penis became super sensitive and noticeably hurt whenever it touched my underwear.

The skin was and still is noticeably red close to and directly around the meatus. This redness gets worse and bigger after I've come out the shower or masturbated.

An hour or so after ejaculation, I get a slight burn/irritation at the very end of the urethra when I urinate and this can last for a few days before subsiding. This has put me off from masturbating as much now.

This has lasted for about 3 months now with no signs of improvement.

I saw a doctor and was prescribed timodine/hydrocortisone cream and this didn't have much effect and am slightly worried it may just make things worse so have stopped using it for now.

It almost seems as if the redness comes from within the skin rather than on the surface.

I don't think its balanitis as I haven't had that for over 12 years when I was a kid and that was with the usual smegma discharge etc, whereas this is just redness. 

I've attached images to show how it looks straight after showering, which is when the redness is at its worst and the contrast between the normal skin colour and the redness can be clearly seen.

    I wrote a long post before, but for some unknown reason (for me) it was deleted. Anyway, I had some improving since then.

    Like the most of the guys here, I have some kind of irritation and redness on my glans and foreskin that comes and goes for about 10-12 months now.

    I've never had any problem on my penis before, except a bacterial infection (chlamydia probably) that was treated successfully long ago. But about 12 months before from the present time, I got a candidiasis from unprotected sex with my wife, whom was taking antibiotics for a teeth surgery. The Dr. prescribed me a cream called Trok (ketoconazole + betamethasone) and I used it for 10 days. The infection cleared out, but at the time it seemed the redness was still slightly present. I don't know for sure how much time after that, I started noticing the redness and/or the red spots on my penis, but maybe 1-2 months.

    I read a lot of things since then, including many papers on this kind of issue. I considered many hypothesis like a lot of people here, including herpes. I didn't feel much discomfort, except a little stinging sometimes. The glans sometimes was a little swollen on inner edge and the foreskin was always red where it covered the red spots on the glans. To make a long story short, I went to the doctor and he thought it was a resistant Candida albicans infection and prescribed me a 1-month treatment with fluconazole, one tab per week. It didn't make any difference. After that I decided to change my laundry detergent and softener to the least allergenic possible on the market and started using them to wash my underwear, plus rinsing it twice (sometimes I don't even use the softener). Since then, the problem got a lot better and the redness appears less often and when it does, it's considerably slighter. The foreskin appears more whitish than the pink/red pattern presented before. After that, the same doctor diagnosed allergy and prescribed me an oil called Dersani, that helps with healing any skin injury and is anti-allergenic. Using it for about a week now, it seemed to help heal the skin. I've been thinking it is/was thin and dry for a long time. I'm also using an anti-allergenic moisturizer soap, with no dye and scent that seems to relief the redness when it appears - especially after sex. I also only wash the penis with cold water.

    Here's what I think of all this: the betamethasone of the cream used a long time ago harmed the skin making it dry and thin, thus sensible (that's why the allergy of dye or scent began maybe). For some reason (maybe not being circumsised, the sweat, the urine contact - I always have a drop that remains after peeing - and the allergy from detergent/softener) it never healed properly and now I hope it's slowly healing.

      How's it going now? Interesting write up thank you! I hadn't thought to switch to a hypoallergenic washing powder because I'm not circumsized so I thought I was protected, but if you say it helped I will definitely make the change asap.

      I think it's worth a try changing your laundry washing powder or detergent. I use a liquid one instead of powder. Even not being circumsised, I've noticed the foreskin was always red, specially the part in contact with the underwear. After the change, the foreskin came back to normal.

      Now the redness/red spots hardly appear and when they do are really hard to notice. Even after sex or masturbation, I can see only a very light redness.

      The soap I'm using seems to help a lot, it kind of help to calm the skin (it's labeled skin calming, moisturizer and designed for babies with sensitive skin, and it's free of dye and scent). The Dersani oil I've only used a few times, but I felt the skin less sensitive after that too.

      I got to say that, after all this time, the relief to see a substantial improving is huge.

      Can I ask what moisturizer it is you're using?

      Ive beenthrough all the sexual testing. Found nothing. I had the pipe and camera put down my pipe for them only to find nothing wrong. Also been tested for parasites to find nothing.

      Does anyone have in addition to the redness, these very small dark/black spot like things?

      And has anyone had extreme sharp pains shooting from the anus ?

      The only moisturizer I am using is the Dersani oil (it's basically a sunflower oil), but I only apply it when the redness show up or before practicing sports, since I've noticed sweating is a trigger. I've tried using Bepantol Sensicalm, but even it being without scent, it has an unpleasant/strong smell.

      What I think made the most difference is the soap I'm using once a day, from a Brazilian brand called Granado, labeled "derm-calming" for babies with sensitive skin. It's also moisturizer.

      Obs: I'm from Brazil, and I think Dersani is a local brand. But it's essentially a sunflower oil, recommended for dermatitis and skin ulcers. I bought it at a local pharmacy, without prescription. I'm pretty sure you can find it under different labels worldwide.

      Bepantol Sensicalm is from Bayer, maybe is available in other countries.

      Hey mate,

      I have some similar symptoms as you.

      Those dark/black sport like things on the glans, they appear only on the lower part of glans (farther from the tip) and look like skin pores somewhere under the skin. Some times they become more visible (darker), sometimes lighter. Easier to observe by pressing on the glans. I think i can see two dots forming something like a line, which reminds me about morgellons that @CanadianMale81 was talking about. I hope it's not. It also might just be the skin pores which became more visible because of the irritation.

      I used to feel some pinching in anus area, not extreme but sharp. It's one of the symptoms for prostatitis, but I guess any irritation in urinary tract could cause it. After all this nonsense started I kind of thought that infection also went into anus because of this pinching and wise versa, when something irritates anus (spicy food, a fissure and etc) I can feel the symptoms get worse in groin area.

      I also feel pain in groin area, I'm definite that its lymph nodes even though they are not visibly enlarged. The pain comes and goes, but all groin area is kind of irritated and too sensitive, I can feel it when some hair in the area gets pulled accidentally.

      You must understand that they test you only for top 5 maybe 10 things. And if it's not found that doesn't mean it's not there which makes things even more complicated.

      Youngbuck, please see my answer above about Dersani. In addition, I don't think I could find a link of a website that sells it abroad.

      I don't have any symptom related to my anus nor the black spots on glans. I strongly believe live that I have a kind of dermatitis, although for an unknown reason (for me). Nonetheless, I remember having the whole glans terribly red a day when I had an oral herpes (cold sores on lips) flare up.

    Here is my update. I had the condition for over two years in varying degrees of redness and irritation. I also discovered I had low-risk HPV around that same time (due to unprotected sex with a random girl).

    In December of that year, I started to lose weight and had digestive problems. After a few months, I was diagnosed with a bacterial overgrowth in my small intestine.

    Since then I have been treated for it, had every procedure, blood test and scan you can think off. Everything STD related came back negative. While the treatment for the bacteria overgrowth helped my groin skin issues, it didn't resolve them.

    I have seen four board certified dermatologists since then and have been prescribed every cream, lotion, anti-fungal and skin ointment out there.

    Yes. I had black specs (one derm just burned them off and said they were broken blood vessels) I have also tried lanolin, oatmeal, coconut oil, honey, charcoal... you name it and, while some of it helped lessen it, not of it resolved it.

    I have changed underwear type to cotton boxers, switched to sensitive skin detergent, installed a shower filter, a hair dryer after showers, etc.

    Here is what has almost completely resolved it for me:

    1. I have had 2 of 3 shots of the Gardasil 9 vaccine (so far)
    2. I am on a low dose round of fluconazole (200mg, 1 week for 4 weeks) and Nystatin cream after I found I had a secondary candida overgrowth.
    3. I used an SLS based shampoo on the head for a few weeks in the shower.
    4. Most importantly... i started using Chaga Medicinal mushroom oil on my penis head and red areas of my groin twice a day about the same time I started the other two treatments.

    I am happy to report that it is almost completely resolved. Most of my formula has to do with HPV because I personally feel that was the cause of my case (it happened around the same time as the skin issues).

      A few warts showed up around the same time as the irritations did. My dermatologist removed them and sent them for biopsy to rule out pre-malignancy, they came back as general condylomata acuminata. My wife also got a pap smear and they performed an HPV DNA test and determined it was low risk type (HPV 6).

      1. Odd request but could you post a picture of your... member, before you started all this medication?

      2. What does the SLS based shampoo have to do with your routine?

      3. The vaccine only prevents future infections, not current ones surely?

      4. Are you sure the fluconazole and nystatin cream aren't the main reasons you've managed to solve it?

      5. How did you apply nystatin and the chaga oil at the same time?

      6. How did you treat your bacterial overgrowth problem? I have digestive issues at the moment too and I'm finding that low carb high fat (keto) with veganism is working for me but unsure if it will solve.

      i am having digestive issues too. i found gluten free food is good.

      can this just be candida as all the so symptoms are similar to candida overgrowth. appreciate your help in advance.

      1. I don't really have any photos of when it started (don't typically take pictures of my junk). However, I do have a few once the red irritations subsided and the extreme dryness began. I can post those with a side by side of it now sometime soon.
      2. I used an SLS based shampoo made by American Crew. I went the SLS route after reading several case studies on SLS and its ability to kill the virus as a cleaning agent (in soaps, etc.). I was so damn desperate by then that I figured what the heck... can't make it any worse.
      3. Yes, that is the official position of the medical community. However, as I meantime before, there has been more recent evidence that it may help with people with recurrent symptoms (such as with oral warts, etc.). I asked the opinion of my PCP and my dermatologist before I chose to do it. There is also a HPV Reddit group with many people who have had it for quite some time. Most of the people in that group say that getting vaccinated should be part of your treatment plan.
      4. No, I started taking the fluconazole about 3 months after I did the SLS treatment and 2 months after starting the Chaga oil twice daily. I only used the nystatin on my lower scrotom and perianal area where the fungal infection took root earlier last year(for two weeks). I mentioned it only because it was part of my the entire treatment. The Fluconazole was part of the same treatment for that area. However, I did try nystatin on the little fella around two years ago that was recommended by the dermatologist who removed the warts and it didn't do anything.
      5. I didn't use nystatin on my penis (explained above).
      6. In February of last year, I went through a lactulose hydrogen breath test and the results showed I had a combo of both hydrogen and methane dominate Small Intestine Bacterial Overgrowth (SIBO). I had lost 35 pounds, couldn't gain weight, had loose stools, inflammation (elevated Ferritin), Costochondritis and a lot of gas. They prescribed both Xifaxan (for the hydrogen) and Neomycin (for the methane) antibiotics for two weeks simultaneously. I also had a colonoscopy and endoscopy performed a few weeks after I finished that regimen (only one diminutive polyp was found). Between the colonoscopy prep and the antibiotics I have slowly gotten back to normal with the help of a temporary low-fodmap diet plan for about four months.

Here are the photos; I found one that I took not too long after the really red irritations subsided but you can still see a hint of it if you look behind the head near the foreskin.       Here are pics from this morning, with no oil applied yet. The entire area just stays naturally moist and healthy looking. However, I still apply the Chaga oil at night, just because I'm OCD. I likely stop once I run out completely.imageimageimage

      Glad you find a solution. The second picture just look like my painful (and not changing) redness i suffer for about 18 month and described in this topic. Can you tell if the redness on picture 2 (foreskin) was changing or did you suffer a constant and not changing redness to? Was your redness painful?

      It was red, irritated and painful until I found a product called Suragel. By that point, I had tried hydrocortizone cream, Aquaphor, Terrasil Balantis creme and Nystatin ointment (none of which worked all the well). I even tried a hair drier directly on my entire groin area for a few days, which I highly do not recommend!

      The Suragel sounded like hype but it actually made the foreskin and back of the meatus area look much better. It was still red but it was less pronounced. It is also what stopped the pain, literally within a few days of starting it.

      However, it seemed like the Suragel started the extreme dryness on the glans area once the inflammation subsided so I stopped using it after three weeks. I then switched to pure lanolin first (for a week) and then on to Skin Fix for Babies, which is an oatmeal based creme.

      The Skin Fix lotion kept things moisturized but the dry skin condition would come back as soon as I stopped using it. That continued for about six months off and on until I started on the treatment plan that I mentioned previously.

      That was (so you don't have to search for it on this thread): SLS shampoo in the shower for 2 weeks and Chaga oil twice a day (morning and night). I left the SLS shampoo on the area for about a minute before washing it off.

      Again, this was in addition to starting the Gardasil vaccine and other antifungal treatments I took parallel to the other formula (200 mg of Fluconazole, once a week for 4 weeks and Nystatin creme for 2 weeks) The antifungal treatments were for other skin problems (in other areas of my groin) and were not applied to my penis head or foreskin. I have one dose of the fluconazole left to take next week.

      That's it gentlemen, I think I've said quite enough. Thanks for indulging me and for listening to my story. Best of luck to all of you for complete relief from this condition in 2019.

      jtheige, thanks for sharing. I want to ask you a question about the first picture. I can see some darker dots on the head (not papules) which look like skin pores. I coudn't see them in your other pictures. Are they gone now?

      I assume you are talking about the very first picture in the entire set? Where you can see a bit of my black jeans to the right? If so, those irritations started after using a lotion called Man Oil 1. They went away shortly after stopping its use. However, it did absolutely nothing for the red irritations; it actually may have been what made them worse.

      Just look at the ingredients list, if sodium lauryl sulfate is one of the main ingredients than it should do the trick.

      However, I am not sure it is a great idea if the skin is still irritated, unless you're as desperate as I was. Most of my irritation was gone by that point so I was using the SLS on dry skin on the glans mainly (because it has been shown to kill HPV on surfaces). Sls may end up making an irritated patch worse so it's best to use it very carefully at first. I would leave it on for 20 to 30 seconds only after you confirmed it doesn't make matters worse (work you way up so to speak).

      As far as the Chaga oil, I use it liberally all over my body (including my foreskin). It seems to help many blemishes and other irritations (even warts).

      I have also had luck with pure vitamin E oil (especially on warts), however, it didn't solve the dry skin issue on the glans.
