Blotchy red rash/irritated skin on penile head (lasted for over 3 moths now)?

Part of the skin on the head of my penis has become slightly red and is causing some pain/irritation.
It first appeared the day after I used a condom so I think I must have had a reaction to the latex or some other substance in the condom.
The tip of the penis became super sensitive and noticeably hurt whenever it touched my underwear.
The skin was and still is noticeably red close to and directly around the meatus. This redness gets worse and bigger after I've come out the shower or masturbated.
An hour or so after ejaculation, I get a slight burn/irritation at the very end of the urethra when I urinate and this can last for a few days before subsiding. This has put me off from masturbating as much now.
This has lasted for about 3 months now with no signs of improvement.
I saw a doctor and was prescribed timodine/hydrocortisone cream and this didn't have much effect and am slightly worried it may just make things worse so have stopped using it for now.
It almost seems as if the redness comes from within the skin rather than on the surface.
I don't think its balanitis as I haven't had that for over 12 years when I was a kid and that was with the usual smegma discharge etc, whereas this is just redness.
I've attached images to show how it looks straight after showering, which is when the redness is at its worst and the contrast between the normal skin colour and the redness can be clearly seen.

I asked urologist for biopsy of that red tip. Instead, he did biopsy of bladder.
The dermatologist did a biopsy underneath the head and down where the frenulun used to be. 2 biopsies along where the frenulum used to be. I am so tightly cut I felt nothing.
Urologist said he doesn't do skin... Unless it is chopping half of it off baby boys for thousands of dollars

Did anyone get mouth ulcers or sores in the mouth? The same time I got the inflammation, rash and itchiness in the penis, I got recurrent mouth ulcers every month. I never used to get them before so I definitely seem that they are related some how.
I did think Lichen Planus (caused by HPV), but they don't really look like the pictures I seen online for them. Just food for thought to see if others got the mouth ulcers too.

yeah dude i did. I had bleeding gums around the time this all started. I assumed it was because I was very sick with the flu a week prior and my immune system was run down. maybe there is a connection? i haven't had any oral issues like this for close to 2 years now. The rash still remains though

Also in the first few days of this rash i notched a tingling in penis glans and itchiness around entire genital area. this has since faded though and I and just left with the rash.

I would get a biopsy if I were you. 2 years is a long time. It can be done out patient and doesn't hurt

I had considered this in the past. my dermatologist seems to think it is unlikely a biopsy will help determine the route cause. Also from what I have seen on here, others who have had a biopsy have not been able to identify the route cause either.

dont know why my post has been deleted, if i have important info that can help everyone here to be free from the problem???
Re-posting: i am Brazilian. There's a lot of men here having the same problem. Many foruns, blogs, whatsapp groups trying to discover. Same sympthons, same pictures, same trials, same frustations.
After 7 months , various doctors, exames, tests, postectomy, i discovered the problem after a re-test PCR MULTIPLEX. A first PCR Multiplex became negative. Dont know why. Maybe was due to recent antibiotic before.
In a second PCR Multiplex, months after, was positive for Mycoplasma Genitalium. A new world spreading decease that is only detected by PCR Multiplex. Not by Common exames.
First try with azitromicina and doxiciclina. Failed due to high resistance.
Second try with Moxiflocin.
worked!!!

Wow, so could I ask what your personal symptoms are or even if you have any pictures of when you were suffering with this? It seems many men on this post may be dealing with slightly different issues so it would be good to know what you were experiencing to clarify whether others may be suffering from the same thing.

Also, I have seen online that Mycoplasma Genitalium can cause testicular pain also. Did you have this? I have had slight redness and tenderness of the testicles since my other symptoms, such as urethra tingling, begun.

Wow. I had NAAT test 3 times and negative. Was that the test that gave you false negative?
Where did you get pcr Multiplex. FDA in USA just approved one by Hologic company but my doctor doesn't have access to it yet.

Could Mupirocin 2% work do we think? I've seen studies on Zoon's Balanitis saying that Mupirocin 2% cream worked for them (it's an antibacterial cream). I actually have a prescription for it which I've not picked up yet because I wasn't sure about trying it because I think it's still a yeast infection, but I'm going to give it a go. Thank you for relaying this information my friend, it is much appreciated 😃

I was using muprocin for 6 weeks. It did nothing. Softened the skin. Maybe it will work for you.. I had another biopsy right next to pee hole (meatus). Will know in 10 days and will report here. It might be psoriasis although it's unusual to only show up on penis. But it happens. Might be koebner phenomenon. Plus psoriasis strikes people in their 50s.
Do they allow pics of penis here? I think they deleted mine?

That test by Hologic is a NAAT test

Like you, I have no clue where to find a PCR Multiplex. The closest I have found in MicroGen DX, which does PCR and next-gen sequencing (whatever that means). I do know that they look for mycoplasma and a crapload of other things too.

"correctly identified M. gen. in approximately 90 percent of vaginal, male urethral, male urine and penile samples."
90% accuracy. I don't know how accurate the swab at my doctor's office is. They swabbed and sent to a lab for a non-FDA approved test.
