Constant feeling in head of penis to urinate

45 male. 3 weeks of constant feeling of urine in head of penis - pressure. Urinating is a weak stream but get brief relief. Some hesitancy. No burn when urinating or pain anywhere else. No feeling in bladder just in the head of the penis. Going to the toilet a lot. Started after masturbating. All UTI, STI test negative. Painkiller and anti-inflammation pills don't work. Going crazy with the feeling. No previous penis trauma or surgeries. Had no meds. Dr won't give antibiotics as now infection noted.

i have had the same problem for about 2 months, I'm 15 and any time i try to talk to my parents about it they dismiss and act like its normal, so is there any medication i something i could do that wouldn't require me to go to the doctor? i am going insane, its ruining my life

Well sorry to hear that mate. It is horrible thing to have. I'm lucky. I have two sons . My problems started only just over a year ago. But I'm determine to cure myself. I believe I'm on the good path.

yeah strange, azoospermia was found back in 2013, prostate issue only started like in 2017

If I have to guess- you could be infected with bacteria. For a very long time. Like most people here I assume you never heard about Stamey's test or nobody done a semen culture test for ya (before 2013?) . Do you know your urine culture test results? Was it ,,no significant growth " ? Or was it simply "no growth " ?

any sure as before 2017 never had all this symptoms even with azoospermia present...
did have a similar test, which doc push on prostate and urine thereafter send for urine culture but nothing found/ no growth

By saying that doc pushed on prostate do you mean prostate fluid extraction? So you gave urine sample before prostate massage and one after?

yes in theory as my region havent had a doc uro suggest getting prostate fluid to test
so my doc tried push or massage maybe like 2-3 seconds , then pee in cup send for culture
did find some low amt of wbc but culture is clear
anyway did plenty of antibiotics already so clueless
i would say its some undetect bug plus biofilm causing all this

First you can't have a sex for around 1 week ( before prostate fluid extraction) . Second I do not believe that massaging prostate for 2-3 seconds will generate any prostate fluid. My Urologist was massaging mine for good 10 minutes . Checking every 2-3 minutes if the fluid is appearing in tip of my penis. When eventually did appear he took sample of it (without urine) . I gave also urine sample before and after that prostate message.

also depends on the test available
alot bacteria test are seperate or pcr method etc
my region uro just sent for culture which i believe miss to pick up the actual bacteria

Ken I get that mate. I rang like 10 different private Urologist Clinics from various parts of UK. None of them were offering or willing to do Stamey's test (prostate fluid extraction. ). Eventually I had no choice but to travel abroad to do it.
We here in UK are constantly brain washed how great our NHS service is but the truth is that NHS as whole is following the policy "cheaper treatment or diagnosis then better ". NHS standards are so low it is beyond belief. If first couple of test will not provide answers they (GP) will mark you as a anxiety person in your medical records and refuse to investigate further. They keep repeating urine test (because is cheap) and never tell you to do semen culture test or refer ya to a specialist. If you tell them that from anatomy point of view urine has nothing to do with potential prostate infection they are starting to be defensive and dismissive. Shocking.
If you can't find good Urologist to perform Stamey's test why not do semen culture test. If your prostate is infected then it is very likely that your testicle is too. Semen test are not that expensive. All you need to do is to find local IVF Clinic. Also do not take random antibiotics without knowing what bacteria you have got . You need professional antibiogram (MIC) to test which antibiotics works against that particular bacteria.
