Constant feeling in head of penis to urinate

45 male. 3 weeks of constant feeling of urine in head of penis - pressure. Urinating is a weak stream but get brief relief. Some hesitancy. No burn when urinating or pain anywhere else. No feeling in bladder just in the head of the penis. Going to the toilet a lot. Started after masturbating. All UTI, STI test negative. Painkiller and anti-inflammation pills don't work. Going crazy with the feeling. No previous penis trauma or surgeries. Had no meds. Dr won't give antibiotics as now infection noted.

    After surgery in addition to burning I had other discomfort, mostly in/near the bladder for which I was prescribed Amitriptyline. This works to relieve neuropathic pain. It worked a treat for me and may well

    work for you.

    Ask you GP to prescribe for you as he did for me, a low dose.

    Here is my story (35 year old male), with a success ending (so far at least). I registered just to share that if it helps anyone. I have added an TLDR in the end if you are bored to read the whole thing.

    A month ago I was at a boat at the sea and the only way to pee was in the sea. For some reason, possibly had to to do with the flow of the water, it was very hard for me to pee and I applied much pressure to do it and did not get the pee out fully as well. The same night, this feeling at the head of the penis starts, a lone symptom.

    Initially I did not link the sea event with the feeling and thought it would be an UTI due to bacteria from sea or a sexual infection since I had a few new partners in the last month. Gave urine for analysis and culture but everything negative. The feeling was not constant but comes and goes within the day, so I visit an urologist/andrologist. He said that it is most probably NGU, did not think it was necessary to place a swab to test me further and prescribed my Azithromycin 1g, and Doxycycline for 1 week. Did not believe the sea incident could cause that. Things started to get better, and I noticed I have the feeling usually when I was not distracted, meaning when I went to bed to sleep or when I hanged out in front of TV. Therefore I believed it could be bacteria but now it was mainly psychological; nevertheless issue was still there but lesser.

    In the meantime a noticed a redness in urethra opening, so I visited a dermatologist/aphrodisiologist. He said nothing to worry, prescribed my an antifungal/antiflammatory cream. But he believed it was the sea event which caused this exterior NGU as he called it.

    A couple of days passed, the cream treatment ended and redness was still there so I am getting stressed starting again to think about the whole problem and its like I triggered the whole thing again. The feeling in the penis returns and makes my daily life miserable. I am calling another urologist I knew, told me to do a semen culture, but again negative.

    And there, as I was searching online for this (read all pages here btw), I do not know how but I am making the magical link in my mind. Before the sea event, and after that I was having a couple of lower and higher back problems. My whole back was very stiff, and when I got up sometimes it gave me like razor pains of 1 sec. I had back issues in the past but I work out a lot in the last 2 years, so these have improved. And I am starting to consider this whole thing to be of muscle/neurological nature. I also remembered that one night I felt weird spasms inside my penis when I went to sleep.

    I am booking an appointment with a chiropractor that I trust and explain him the issue. He confirms to me that it is probably due the a blocked/pinched nerve in the back. And it sounded very natural to him and explained me how the nerves from the back also link to the ones going to down to urethra. The sea event could have aggravated this even further. Possible bad coincidence to have back strain and this event together, who knows?

    Anyhow, he unblocks with his moves my whole back from top to bottom. He suggested to drink as well much cranberry juice for 1 week to clean my urinary system. Said even if this is not infection, cranberry fluids will do good work.

    24 hours after the chiropractic session, the feeling started to disappear. Now I am in the 9th day, and I have not felt it again (maybe 1-2 times for a couple of minutes only, but it was really mild compared to before). Before that I only had two good straight days without the feeling. I am heading this week for my second last session with him to assess it again.

    TLDR: It seems to be a neurological issue, can be due to back strains. Even if you do not feel having back problems, maybe there is a trapped nerve there in the background. Discuss this with a chiropractor and hopefully you will see results. It is also partly psychological but cannot be entirely, something is surely there and we also aggravate it with out minds.

    The fact that many people here in this topic disappear after complaining about this issue is possibly on favour of the neurological reasoning. Time healed it for them, sooner or later. But if it is indeed that, with an adjustment from a chiropractor you will see immediate results.

      Hi, this is so interesting. I had a slip a few years ago and hurt my lower back. I recently noticed that these strange penis sensations occur with a dull pain in my very lower back. My symptoms are also accompanied with body sensations that feel like cold water splashes on random parts of my body.

      All of this could very well be attributed to say a trapped nerve or something out of place that's sending bad signals or something along nerve lines etc. I would never have combined the penis sensations with this until I read your post.

      I'm very curious what your symptoms were if you wouldn't mind being specific.

      For me I'm fine when laying down or in the bath, but certain positions like driving, sitting at my desk or on our couch cause the symptoms. Ive associated it with harder surfaces as i can sit on my soft outside couch and I'm fine. If i'm up and about or excercising i'm fine.

      The main symptoms i feel are a wet or cold feeling in my penis tip, the sensation that i need to urinate but might not in reality. Sometimes i feel a fullness inside my butt, and sometimes my entire crotch feels wet/cold but is perfcetly dry when i touch it. The lower back pain at the very bottom of my spine, and these strange body zaps that happen randomly and for no rhyme or reason (i thought these were due to anxiety but they happen out of the blue when i'm perfectly calm). I sometimes feel i have less sensation when ejaculating and have noticed what i believe is less semen when i ejaculate.

      The strange thing is that if i drink alcohol I'm usually symptom free. It seems to numb whatever is happening.

      Adding it all up and based on your post im very curious if it is all connected to a trapped nerve/slipped disc.

      Please share you're exact symptoms and an update if you would be so kind 😃

      My main symptoms were the feeling of wetness in the penis, a couple of penis spasms every now and then and mainly the feeling that I had to urinate. The latter one was more obvious when I relaxed since it was easier for my mind to remember the issue. When I was outside with friends I forgot about it. When I lied down to sleep I always felt it, then changed positions and felt better until I sleep. Also, when ejaculating I felt it kind of sensitive, not painful.

      Luckily I also felt a couple of lets call them electric shocks in my lower back for 1 second, and I say luckily because this is how I started connecting this with my back.

      It is a combination of a trapped nerve and the psychological aspect of course (stress), which aggravated it. The fact that you forget it when you exercise is due to your mind focusing elsewhere, or because indeed when sitting your back gets pressured. Or a combination. I was also trying to understand when it annoyed me the most but it was not easy. Mostly, in the situations you say as well.

      I am not a doctor or a chiro, but from what you are describing it is pretty obvious that your back issues are the root of your problem as well. Have you visited and discussed this with a chiro? He will explain to you about the nerves of the pelvis which are connected with the ones in the back as well. Mine told me also that it can mimic completely symptoms of a UTI, even pee burning.

      As I said before, people here disappear and do not let us know if they still have the issue. I suppose they don't, since time healed it and the nerves came back to normal. As back issues come back to normal with time. Just the chiropractor can fasten this!

      Thank you so much for responding. Your post helped me a lot. I'm finding that there is a HUGE psychological effect since reading your post. Ive changed a few things in my life (gave up alcohol, gluten and coffee - all stress triggers for me) and that started to give improvements but I still experienced the sensation in my penis. So i started to use a tennis ball on my hips and lower back, it was super painful but a nice release at the end. Im also doing various stretches and working out again, and im not experiencing much if any lower back pan since. All the above has brought my symptoms down drastically to a more managable place.

      I then started to realise wait, I'm not 100% sure IT IS a wet feeling, maybe its a COLD feeling. My line of thinking is if theres a trapped nerve or slipped disc or something then when im sitting or squatting maybe theres no blood flow to the penis and it feels cold. Something like that. So i googled my symptoms and instead of writing wet i wrote cold, and I found a forum with guys with my exact symptoms and the general consensus seemed to be stress, lack of sleep, which I suffer incredibly.

      After reading your post and findiong that other forum a load was taken off and instead of thinking i have some incurable disease the whole things felt more mechanical and solvable and ive been doing really well. Im paying attention to what exact positions triggers it. You know what is funny... I can sit on the couch now and when i feel it come on, i tell my mind "No, go away, its not happening "and it will dissipate. if thats not psychological then i don't know what is. It does come back, but i just shoo it away again. Ive also noticed that sitting with my feet up (normally a trigger on my couch) with my left foot crossed ver my right will trigger it wheras the right over the left wont.

      So yeah, i think I'm getting close. and i really hope this helps other people. Ill always check back and if i find 100% success i will post my journey for all.

      So in a nutshell: Either a trapped nerve/slipped disc mixed with lack of sleep and stress.

      I am going to a urologist next month just to have them check my prostate etc what a long crazy story I will be telling him, lol

      Ok I can confirm everything your saying and what I'm going to tell you is not speculation as i've already been to urologist , got cystoscopy, ultrasound on kidneys and bladder all came back clear. I'm scheduled to see a PT in a few weeks.

      I have the same symptoms as you, cold/ wet feeling sometimes and just slightly sensitive tip. I did have a range of other symptoms although these have resolved slowly but surely over the course of four months.

      Essential the pudendal nerve which runs through your pelvic floor right to the tip of your penis is just slightly irritated which can cause a range of symptoms from - feeling like pee stuck in penis, frequent urination, sensitive tip, electrical shooting pains etc. This happened shortly after a period of edging and gripping a little too hard but was also incredibly stressed around this time so it may have also been the cause. Now either I Irritated the nerve my gripping too hard which caused my pelvic floor to tense up irritating it further or the stress caused my pelvic floor to tense up which has pressed on the nerve.

      Pelvic floor stretches are in order to relax the muscles along with deep belly breathing. She will also check for and relieve any tension. She did say over the phone it will resolve and if my symptoms are anything to go by she's right as they have eased.

      Stress is a big factor for muscles and nerves to try to relax. It will resolve. Nerves take time to heal. Wanted to share with you.

      Just saw your post. I've been up and down the past couple months. I saw a urologist and he wasn't great. Didn't listen and then told me he wanted to do a cystoscopy. I haven't done that yet as I'm starting to feel this is more nerve related. I'm getting body sensations daily, that come and go. I call them zaps.. just random electrical sensations all over my body, sometimes in my legs, sometimes in my arms, face, torso.. Anywhere really. Just a quick ZAP then it's gone.

      I've noticed that sitting at my desk I'll start to feel the penis sensation. Like I need to pee, cold/wet penis tip sensation. If I get up and move around it dissipates.

      I found another thread online and a guy said it's absolutely Pelvic Floor Issue” Apparently there's a technique called "Lajin '' that is useful for this. It's a stretching practice and there are youtube videos. I'm going to try this and wanted to share in case it helped other people.

      Im responding to you because, this all started for me by GRIPPING my penis really hard. These symptoms started the day after 2 years ago. Sounds like we are on the same page and I'm so over it. I would be so happy if you would keep in touch with updates and I'll do the same. Same goes for all of you. This is torture but i really feel we are getting close.

      I don't seem to have any of the symptoms of prostatitis other than a urologist saying my prostate felt a little boggy a couple years ago. I can pee fine, no pain when i ejaculate, no burning, no pain in my crotch or prostate area .. the only slightly weird thing is that my semen doesn't seem to have the same smell it used too (gross sorry) and i sometimes feel im not ejaculating as much semen as i used to or there's less sensation when i do. That all may be psychological.

      But yeah, cold/wet penis tip in certain positions that go away when I'm active. Body zaps that are random and will go away for days then suddenly reappear with a vengeance. Desk job, bad posture etc.... I'm pretty sure this is some lower back/pelvic floor issue.
