45 male. 3 weeks of constant feeling of urine in head of penis - pressure. Urinating is a weak stream but get brief relief. Some hesitancy. No burn when urinating or pain anywhere else. No feeling in bladder just in the head of the penis. Going to the toilet a lot. Started after masturbating. All UTI, STI test negative. Painkiller and anti-inflammation pills don't work. Going crazy with the feeling. No previous penis trauma or surgeries. Had no meds. Dr won't give antibiotics as now infection noted.

Try a home urine test. If there are leucocytes showing then you likely have an infection. Dr will want a urine sample anyway to confirm or otherwise. Do you have pain in the back below ribs? If so you probably have a kidney infection.

Hi Happyhacker
Had a urine test all clear, no pain in the kidney area. Going for an ultrasound test we will see if that shows anything. All routes point to an inflammation in either the Urethra, bladder or maybe the prostate area is taking to time to heal probably 4 months next week. I am taking Diclofenac for the pain as this is the only remedy for the pain

Hi.
With respects to urine tests/dip sticks, I never had positive results, yet had confirmed heavy growth of E. coli in semen from prostate. This led to constant irritation of urethra. Dip sticks test for leucocytes (a class of white blood cell). However, these will only be present in sufficient quantities if there is a bladder infection. If the bug is simply in the urethra there may not be enough leucocytes to be detected by a dipstick, relative to the volume of fluid it is diluted in.
I know I said this already, but I would give Dmannose a try to see if it releaves urethra irrititation. It will do so if its e.coli. It wont cure prostate infection however. That requires apropriate antibiotics. Also consider semen culture to see if can isolate an offending bug.

Hi Roy
Thank you for all your thoughts, I think that this is some kind of inflammation, my GP has asked me to go for an ultrasound on that whole area. so frustrating after what i thought would rectify the matter

HI Roy
What is the procedure for doing a semen test is you are getting retrograde ejaculation or do you have to wait until you get some semen the normal way?

Ah, ok. For semen culture they get a sample the normal way. I did not appreciate the fact that you had retrograde ejaculation. so perhaps that is not an option?
see what doc says so.

Hack if you have retro they will not be able to get a semen sample. Nothing will come out it is going backwards. So you may as well forget that one.
Have a good day....Ken

Oh thank god I found this forum. I thought I was the only one going through this, but it seems like everyone here is suffering with pretty much the same thing I've been struggling with. I'm 49 and my symptoms seemed to start a few months after my vasectomy. Every so often I'd get this feeling of urgency right in the head of the penis, like I needed to pee even though I didn't. It's not burning, but more pressure, and the tip becomes incredibly sensitive, even underwear irritates it. Peeing does temporarily provide relief. I've been diagnosed with prostatitis and I've tried several drugs that didnt work, like Myrbetriq and Levsin. I've essentially been told to live with this and that it's part of getting older, but bouts seem to be more frequent and and are usually triggered by masturbation. It's gotten so bad that i'm afraid to even have sex or masturbate anymore for fear of triggering another episode. It also affects sleep since it's worse lying down and exacerbated by stress. Looking to get a second opinion because no way I can live a normal life like this.

Did you do cultures? urine and semen?
Try dmannose. See if this gives some relief. Take a daily maintanence dose and also a heavy dose before and after you ejaculate. Ensure to always urinate immediately after you ejaculate.
Reason - If there is a bacterial element to the prostatitis then once you ejaculate you are spreading bacterial into the urethra (tube of penis). What happens me is the urethra gets irritated (and often have that urgency at penis tip). The mannose will only help if the offending bug is e. coli. Fortunately this is the most frequently offending bug (but some others do also). The dmannose helps flush the e. coli from the tube.
Google dmannose and UTI.
keep head up

I recommend everyone who has this problem to take celecoxib 100mg every 12 hours and then write here if it is helpful. It relieved my symptoms and I think it would help everyone who has this feeling because of inflammation in urinary system.I started this medicine two months ago and the relief started after 3 days and I am still taking it.I also take tamsulosin every night. Don't forget to write here the result.
