hemorrhoids, Fissure? Getting a bit scared, can anyone help?

I'm a 50 year old male, fit, active, in good shape and I've been suffering with hemorrhoids for about 4 weeks. However in the last 10 days or so I've been suffering the most debilitating and excruciating pain, it's ruling my life, I can't work, can't make arrangements to see people because of these waves of pain that are close to making me weep. Previously I'd tried all the hemorrhoid medicines on the market (and I mean all!). Just over a week ago I saw my G.P. who told me he thought the piles didn't look so bad and he couldn't see anything else, I tried to explain the level of pain I was in and I explained that it was hurting me to poop in the morning so he said "although I can't see anything let's treat it as a fissure". He prescribed me Glyceryl Trinitrate which when I apply it maks the pain far, far worse. For over a week I have been taking baths and adding coconut oil which I read can help but things don't seem to be improving. Since I saw the doctor I inspected myself more clearly in the precise area where the pain felt like it was coming from and found this (what I can only describe as) very strange bright red tiny vein like thing. I basically know this is where the pain is coming from (the doctor hadn't spotted it). I'm now worried that it isn't a fissure at all but something more serious. I've uploaded a photo (yes, I'm that desperate), apologies if that's not appropriate on the forum but I really need some advice as this is ruining my life!

    At my worse I ended up in A and E unable to cope with the pain

    - I thought I was clever and seemed to think

    Not eating meant not pooping just lived on milky coffees then one morning bam ! Poop that made me faint with the pain - my body from boobs to knees was in spasms , incontinent of urine and rolling on floor in agony - on a trolley in a and e with morphine - since that day the colerectal surgeon told me to eat daily and poo daily - laxido etc - I was booked into theatre 23/12 however decided to slog it than have Botox - I finally turned a corner - lots of tears and strict intake and Butt routine - you will get there 

    Lots of support on here !

      Thanks Natasha, blimey that sounds grim, haven't got to that stage but have been close to going to A&E. That said, I really feel I've got a handle on how to manage it and fingers crossed it'll sort itself out (and quickly I hope). This afternoon has already been much easier than the past 2 days, but then I've been laid out on my bed and working from home!

    Hi everyone, I wanted to update you. My situation has deteriorated somewhat. After what I thought was a steady improvement culminating in me keeping my scheduled Doctor's appointment last Thursday where he confirmed I had a Fissure things since Friday have got much worse (I didn't think they could). I have been trying GTN, Laxido, All Bran for breakfast, LOADS of water intake during the day but after each BM I am in crippling pain for most of the day and evening, I spent the weekend lying down and had to cancel plans - this thing really is controlling my life. Yesterday (Bank Holiday Monday) I had to work and it was just awful, so bad in fact that I have taken the step of booking to see a Private Recto-Colon Surgeon on Wednesday evening. I'm worried that I may have to undergo some kind of surgery, but I have had read stories about Incontinence after Botox. I start a new job at the end of April and I must get this sorted by then, it's really getting me down.

      I am so sorry.   I can feel all the pain,agony & depression that you feel.   It’s been almost 4 months since this all started & I am in agony.   The worst part is when you feel no one can help.  It’s so discouraging when you go in to doctor after doctor with no resolution.  Nifedipine,nitroglycerin,sits baths,analpram foam,hydrocortisone,lidocaine nothing works nothing.  I had the Botox 5 weeks ago and there is zero improvement.   Tomorrow is surgeon #3 praying to God I can get this fixed.   This has absolutely ruined every part of my life.   Struggled through Easter mass praying I could make it through & then rushing out of my families dinner because I was in agony.   I will update if he can help

      Are you taking sitz baths ? 

      Tried pooping in the sitz bath sounds horrid but helped me ! 

      Anything is worth a try

      The bert bees lip balm 

      Sitz bath with Epsom salts and coconut oil- please try 

      I still take them 

      Plus laxido 2 a day

      There is a cream guys that I’ve tried amongst many it is mayinglong musk cream from eBay from

      China it’s great - smells but is good - also proktis m is good and forces of nature fissure 

      I’ve tried hundreds and these did help 

      Also a codeine 30mg tablet in the morning after BM 

      Fruit smoothies

      I completely appreciate the suggestion but I’ve bought so much stuff over amazon that hdone nothing for me that i am praying to God this appointment is a solution.

      People need to understand there is no quick fix, quick cure, the only thing you can buy from Amazon are things that can try make it bearable. 

      Most of us have had chronic so know exactly what you going thru, you need to try manage it vs trying to fix it. Once you start managing it, it will repair itself over time.

      Drink lots of lots of liquids so you body can climatize, be consistent and don't change your routine. Your proctologist should have given you some creams to at least hopefully keep your pain at bay. Keep up with the sitz baths or whatever helps keep the pain.

      Try to eat less food, avoid foods like bread that make you constipated, try drinking more fluids and soups to help things pass.

      Surgery should be the absolute last resort and I see too many people on this forum recommending it .. rest assured you will heal over time, but it could take 6 months or longer. In the meantime you need to manage it.

      Look at my profile, I wrote a blog on suggestions, you might find some useful info there.

      The thing that cured me was some low blood pressure pills that my pain doctor gave me, it similar to the creams that you probably have that lowers your blood pressure to draw blood to your anus area, but it does it for the whole body.

      Realize there is no overnight short term cure, no quick fixes, no magic creams. Try teach yourself how to manage it and over time you will see it heal. I was in chronic pain for at least a year.

      Drink lots of water, use the table top method I suggest in my blog and do this daily even if it is painful, this will strengthen the muscles in your butt. Look at talking to your doctor about the pills my pain doctor prescribed if nothing else is working.

      Thanks Discourged, I appreciate your support and also really hoping for a breakthrough for you in your situation. Things have moved on for me - I went to see a Colon Rectal specialist last night. My Fissure is slightly "off at the side" - neither 12 or 6 o'clock as most tend to be (it's about 5 O'clock he reckons). I've read subsequently online that this *might* mean it's something more serious. I start a new job at the end of the month and need to be physically fit by then. The surgeon is going to give me Botox on Monday under G.A. He suggested this so that he can have a proper look and check that there's nothing more serious going on. I read what people say about Botox, he said there's rougly a 70% success rate first time so I am going for it. I'm assuming that he is carrying out due dilligence and being more thorough. I can't go on like this so I'm going for it.

      Good luck David, what awful timing with the new job starting at this time. Life has a habit of throwing things at us at the worst possible times.

      Just one thing, have you tried magnesium citrate 500 mg x 2 a day.

      Make it so easy to go.

      Hope all goes well on Monday and you get some answers, good ones.

      Yes I get on amazon using prime, they are magnesium citrate high potency 60 tablets in a brown bottle with a gold label. I would post a link but it’s not  allowed on here. You might be ok taking one a day, but I’m on drugs that slow everything down in my system so I take two, plus one sachet of Laxido a day, every day. They are safe to take every day and might give you some relief before your procedure. Botox doesn’t start to work for a few days, don’t know if you were told that.

      Thank you for your reply.   I ended up going into surgery again.   The botox did nothing & while I appreciate the comment about trying to manage it I have no idea how that would be possible? I have small kids & an example of what I mean is I took hem to an Easter Egg hunt & couldn't stand up outside for more than 10 minutes? How do I manage to leave my house when that happens? How about if I have to use the restroom & there is an issue like there always is & I have to immediately get into a sitz bath after.  I can't wait to live my life for 6 months hoping I will heal.   I went in and had it cauterized & stitched.  I have that I think under control but the hemorrhoids which is probably half he cause of this whole mess are out of control.   Now I am patiently waiting to a THD doppler guided ligation.   I am scared to death.  I am sick and tired of being in pain.   I couldn't even get out of the restroom for 45 minutes today because of an attempted bowel movement that went totally wrong. My hemorrhoids git so big and so swollen I thought one was thombrosed.  I was in and out of the ER twice in the same day.  I have been drinking water,colace,miralax,dulcolax...Hi fiber none of this has worked for me.   It gets worse not better.  Looks like the fissure wasn't the cause of most of this pain after all.   Discourged ABSOLUTELY! This is has taken 3 months of my life away.,

      Hi Discourged, it sounds awful and I know (in part) what you are going through, I can't tell you how many social events I've had to cancel because all of a sudden I have to lye on the bed and can't leave the house. I would definitely recommend Magnesiun Citrate if you haven't tried it - Laura3333 recommended it to me and it's been a Godsend, I now have painless BMs and am actually feeling better pain wise (although I'm going in for surgery tomorrow anyway). Also, A couple of bowls of All-Bran have helped me alot. Small bowl mid morning and another in the evening. I wish you well, I really do.
