Is this Balanitis? I'm 21, had it most of my whole life, no treatment has worked

Hello, I've seen a doctor many times about a mild condition I have on my penis glans, have been prescribed 3 different creams including Betnovate and a Hydrocortisone, a course of antibiotics and a swab which said I was clear and still the condition remains. I used to have a tight foreskin up until last year which is now sorted from stretching, and I think the condition began during my teenage years when I wasn't able to wash the glans. It often feels dry and can sometmes feel slightly painful when the foreskin retracts, almost like its rubbing and the sensitivity everywhere is quite high still, not sure if this is due to the condition. The urethra opening is always red and inflamed and red areas appear frequently around the glans itself. Any advise on what I should do would be much appreciated, have attached a photo too. Many thanks!

That's a really interesting article. It describes a lot of symptoms I don't have but some I do.
The stress bit is what is interesting. When this happened to me I was under a reasonable amount of stress. I was not getting much sleep due to travel with work across different time zones.
So there maybe is something in this. Since I have been going to the gym regularly and eating better things have improved. I have also been under a lot less stress. However I had a flare up at the start of the corona virus epidemic. I have had the virus and the whole epidemic has caused problems with my job. Thankfully I did not get seriously ill with the virus and the government help has meant my job is safe (for now). Things have improved again with my condition.

okay guys so results from my next gen sequencing test are in.
99% bacterial load of E.Coli
Escherichia coli
Ureaplasma parvum
This is after so many normal urine tests showing nothing....
i tested positive to mycoplasma gen back in like 2016 or so but a test just last month said i was negative to it. now ureaplasma is shwoing up...
i am excited but nervous

private message me. And cant you see calebs message? we both have same symptoms and agreed it was best to contact this specific company regarding urine/semen cultures and they have found bad bacteria.

Just to update this post with more information now that im on PC. The URINE (didn't do a semen test yet) test said i tested negative for Chlamydia trachomatis, Mycoplasma genitalium, Neisseria gonorrhoeae, Trichomonas vaginalis. This is not to say that others don't have those.
So down to the nitty gritty.
Escherichia coli showed up in the initial PCR testing, then they did the level 2 Next Gen Sequencing test over the next 5 days. It showed that 99% of the bacteria was E.Coli. Ureaplasma parvum didnt show a percent so i'm assuming the 1% . They also do resistant testing to the bacteria and it is resistant to Quinolone drugs. turns out also E.Coli is resistant to many antibiotics, when you take a course of antibiotics and say dont finish them, that bacteria then becomes immune to it like doxycyclne (which i took for a long time as a teen for acne).
Ureaplasma parvum from what i can see is in most people, even urine testing. However this bacteria is generally not harmful and stays dormant it can cause issues. But seeing as E.coli is the dominant 99% id say that is what is causing my issues but thats not to say it might not be.
SYMPTOMS: 26/yo male 10 years ish with a red meatus just like OP, however i think this was caused from phismosis as i didnt retract my foreskin my whole life.
before all of these issues i dont recall having trouble urinating etc it seemed to come on after i started having sex. My first ever b******b was rough and i remember my foreskin being pulled back forcefully. Over the years then my banjo string snapped etc. I don't remember a certain time when the burning etc started as i have dealt with this for years but over the last 2 years has been the worst. I have had the same partner for 5 years now, but before that i had sex with multiple women (probably around the time i started getting symptoms and tested positive for mycoplasma gen). I had unprotected one night stand with an Australian woman in Bali in 2014 and then that same trip i had protected sex with a sex worker in Bali with a condom (so stupid i know) and i think after that my symptoms slowly started when i tested positive for mycoplasma gen.
I tried to add as much information as i can remember but i will keep updating as i find more out. I will head to my doctors over the next few days and go from there. I suggest everyone get tested through this method to find out if your problem is from bacteria. If it isnt it could be something like Chronic pelvic pain.

Nope no doctor or urologist i purchased the kit myself. Im in Australia but they ship world wide. Its expensive but the normal samples that are done at the doctors etc are old ways of testing. Ask any questions you want ive always got notifications on.

Thanks for keeping posted. So are you think it maybe e.coli that's the issue?
Have they suggested one of the floroquinolones then? I bailed on taking Levofloxicin after reading potential side effects. Heavy duty antibiotics also used to treat anthrax and the plague!

fostomycin, carpbapenem, augmentin but it really depends on the person as they said i had a resistance to Quinoline where you may not.
E coli is the main culprit of UTIs which is what i have always had symptoms of but they never showed up in urine samples.

Ecoli makes a lot of sense.
I am going to PM you about the test.
I will then go and see my doctor and show him all the information.
I don't have exactly the same symptoms as you but I do certainly have some of them.

Interestingly since my last posts re HPV, I got really unwell one morning with
nausea, vomiting, abdominal pain, diarrhea, high temp. Two weeks prior to this I started feeling a sharp feeling pushing out of the tip of my meatus.
Long story short - had a telephone appointment with a new GP and luckily they listened and treated it as a UTI. So I was prescribed flomax and co-amoxiclav for two weeks. So, after a week I started to see gradual improvements. Skin around head of penis not as bad, less sensitive to rough clothes dragging over, less pain. In fact I went a day without any longtec (oxy).
The GP has now switched me to trimethoprim, however after reading the posts by caleb04109 I think I'm going to order a test kit. I'm reluctant to take the trimethoprim as I think this is E Coli theory might be right. The likes of co-amoxiclav might work temporarily, but the theory of the E Coli becoming resistant stands up.
Like you pitsonhead, I had what I was told was a next gen test, looking for Mgen. I'm questioning exactly what I had done. Can anyone confirm if there are different grades of Next Gen tests?
All of my issues started after what I believe was a muscle tear in my abdomen in January 2019. I had a really itchy (but sore itchy) side - the doctor pooh poohed it saying it was a spasm. My urinary problems, including epididymitis followed on, them my skin issues on my penis.

I questioned my test with the 1-1 centre doctor, who I was scheduled an appointment with following repeated visits back to the clinic with repeated skin issue problems. The doctor called me back today to confirm if was a next gen test, but that is was specifically looking for mycoplasma genitalium (mgen).
I asked if it would have highlighted e coli and she said no.

interesting, thanks for posting. I have some co-amoxiclav at home I was prescribed for an ear infection which was mis-diagnosed by an emergency room doctor, An ENT specialist told me not to take them.
I think I am going to take all this new evidence and info to a private GP I have seen previously who seemed the most knowledgeable physician I have seen. Then see what he says. He may be able to so a next gen test for e-coli.

i have just looked up which antibiotics work against E.coli. nitrofurantoin is one and I had a 1 month course of it following my last urine test. It didnt work. It will be interesting to see if after a gen2 test I have e.coli.

Just to let you know, I pushed for and managed to get a booking in October for a brush swab and or biopsy should it be required. Thankfully they wouldn't need biopsy the head, some of my scar tissue from my circumcision is also affected. So they will biopsy that.
They're pretty certain the brush swab will be sufficient, this technique is sometimes used on the cervix during smear testing.

I'm going to give the Levofloxin a shot I think hopefully next week. If that doesn't shift it i might have to give up on the bacteria front as would have tried Doxy, Co-amoxiclav as well (Plus Trimovate which has antibacterial qualities).

Re me keeping you posted, please remember all of symptoms could be completely unrelated. I keep reiterating this point. You all need to get yourself tested for HPV. Not quite sure why folks don't - non of us have the answers yet, therefore its about us each doing our best as individuals to find out.

at least in the US, there is no approved hpv test for men, as far as i know.PLEASE correct me.
