Just had a SUPRA PUBLIC OPEN SIMPLE PROSTATECTOMY

Hi Everyone, 

I posted a few times on this site about a month ago. I was asking members about the various prostate procedures. My prostate was 263 grams with a large median lobe. I was unable to pee at all prior to surgery. I opted for the SUPRA PUBLIC OPEN SIMPLE PROSTATECTOMY. Surgery was May 2 (8 days ago). It was not pleasant or fun but it is done. I pee like a teenager now. No incontinence. Getting erections etc. Today I get my staples removed. I will go over the biopsy report with him, but he said that when he was looking at the inside of my prostate he said there was no hint of cancer. 

If anyone is interested in my surgery or how I am doing please feel free to ask.

      Most urologists that are surgeons do it. Some do it laparoscopically and some robotically. If I had known at the time of these other procedures I would have looked into it. Less recovery time.

      I agree. My prostate was off the charts in size and my median lobe was pushing way into my bladder.

      Gary, is there a lower limit on the prostate size for what you had ? My prostate is not large at all. Hank

      I am no expert, just a patient who's read posts on here (and had HOLEP), but I think this is at the top end of intervention, and I don't think you'd have this done unless the thing was so big that Holep, TURP, PVP/greenlight, or the alternatives that can work for some like FLA/Rezum/PAE or even urolift, aren't possible.  A full open prostatectomy, robotic or otherwise, is what you'd have done for cancer (I'd probably choose it over radiotherapy, just to be sure, if I had cancer there).  It has risks, happily not realised for Gary, of incontinence and impotence. Those risks are as I understand it negligible for turp/holep/pvp.

    Sounds like this went well so far. Good. Nobody has ever posted here about this as a cure for BPH. Go easy for a few weeks. If you don't mind posting something so personal, I imagine men will be interested when in a couple of months you have had sex, if the sensation was good. At 284g, it sounds like you needed a radical and very unusual cure. Being able to pee well must be such a relief

      Funny you should ask. I saw the doctor today to have my staples removed. I said I am having a problem with erections. He said it may take a month or two to come back if it does. I said NO you don't understand. I am getting them NOW. I am afraid that I should not be thinking about sex this early. I even got pre-cum after seeing some sexy ones on TV. He said that is very unusual that this would be happening at this point. He said give it two weeks and have sex. He said obviously my sexual functioning was not damaged.

      Also a funny story. I came home from the hospital after 5 days recovery. I want to the bathroom and stood touching my shins to the front of the toilet bowl. I let loose and splash all over the front of the toilet tank and on to the floor. What a mess. Also, my incontinence is gone. I was using pads in my underwear before but I don't need them now.

    HI Gary,

    I had this same procedure done back in June of 2015..So far so good. Mine was 200 cc or 175 gms.I was at the stage where even with a large cath I was almost unable to go. This procedure will take about 6 weeks to relatively cure..but studies indicate that SOSP is a most reliable solution. Also if you are on debilitating MEDS you should be able to go off ALL of them.I do not take any for the prostate today. No doubt, when done properly it is star performer ...The last thing you need is to have another procedure done in the future................Were not getting any younger and these kind of operations can take a lot out of you. Today I can hardly tell on my physical body where the procedure was..however its Not an easy procedure and HoLep would be less invasive however with a gland of our size however you would be on the table for a lot longer time.  God bless and Best of Luck

    Hi Gary, Good luck and thanks for sharing your experience. Yours was quite a size but a guy who had his op on the same day as mine had a cricket ball size so not uncommon.

       My doctor told me that my prostate was the size of a softball. Softball is larger than a baseball. I don’t know what a cricket ball looks like.

      The diameter of a cricket ball is between 2.8 and 2.86 inches. The circumference measures between 8 and 9 inches. A cricket ball weighs between 5.5 and 5.75 ounces.

      I just asked SIRI what the circumference of a softball and she said 10.88 to 11.13 inches!!!!

      Circumference = pi times diameter.  Implies diameter = 3.5 inches. This equates, if an American inch is 2.5cm, a volume = pi times 4/3 times (Radius cubed), of volume = 350cc.  Specific gravity of prostate tissue is 1g/cm3, i.e. same as water, so that's 350grammes.  3.2 inches diameter would equate to 284 grammes, that's a circumference of 10 inches.  So he was close (assuming the weight estimate given, is the correct datum).

       I always thought an American inch and a British inch were the same thing.  Don’t forget we were too dumb during the Carter aministration to go on the metric system. Too much resistance.

       No I was too busy seeing the sights. Maybe if I get back to London at some point in the future I can do that. 

       I do have a funny story to tell which shows the difference between the American medical institution and the British. I was having indigestion and needed to see a doctor. My friend who lived in London told me about his doctor. I called to set up an appointment. The receptionist told me that the surgery hours were between nine and four. I said that’s fine when can I see the doctor? She said I told you this surgery hours  are between 9 and 4 PM. I said I don’t need surgery I need to see the doctor. We went around and around and around  untill I hung up. I called my friend and he explained to me that surgery hours means office hours. Duh!

      That is why we can always spot Americans on this Forum. They go to the Doctors office even for laser surgery. We go to the doctors surgery for a medical appointment and they very seldom have any other facilities although now some super practices are being set up. For an X-Ray or a cystoscopy you go to a hospital.
