Just had Kenalog Protocol

I’ve just had Dr. Feingold’s Kenalog Protocol in New York. I’ve suffered with a fissure for a year and half and tried dilizatem and nifidipine ointment. I eventually developed an allergic reaction to the diltiazem so kept on the nifidipine for about 8 months. It provided a little relief but didn’t heal the fissure. My colorectal surgeon in Toronto eventually offered the LIS surgery as he said it was unlikely to heal after such a long time. I was very hesitant to do that surgery as I’ve read about possible complications and being in my early 30’s I didn’t want to risk it. I researched and found the Kenalog procedure. Where the fissure is cleaned out, cauterized and injected with kenalog steroid. No cutting of muscle so not worry about being incontenet. The only doctor to do it is Dr. Feingold who invented this alternative to LIS in New York City.
I contacted the hospital that he works at and got my colorectal surgeon in Toronto to provide me with all my documentation regarding my condition. I sent it to the hospital via global services and after a fairly long process (4 months) I got word that Dr Feingold was willing to take my case. I flew to New York and had the procedure done yesterday. So far I’m in very little pain and am optimistic that the procedure will be a success. I’ll update throughout my recovery and am happy to answer any questions for those that may be interested this procedure.

Has anyone located Daniel L. Feingold, MD? I am considering contacting him as I've had LIS surgery in 2012 which solved my problem until 2017, then had a recurrence when I had to take pain meds after being on pain meds for a broken leg. Doctor gave me the creams until I finally screamed 3 months later than I needed something more to fix this pain. Doctor finally did Botox which worked and healed it in about 4 weeks. Then recently, the fissure opened back up again after a slightly hard bowel movement. Had the Botox a week ago, but it doesn't feel the same as the last time. Hoping that I get the same success, but the recent Botox has flared up Hemorrhoids and the fissure pain is worse now.
I'm desperate. My doctor blows it off as I'm being too dramatic. I get that I can't have LIS surgery again. But I'm still blown away that there aren't other options out there. I think mine has become ulcerated skin by this point. Why can't they cauterize the area? Dr. Fengold is the only person on the net that I've found who seems to get it that there is more to fixing a fissure than just cutting the sphincter muscle or shooting Botox into the sphincter. At some point the reinjured skin down there just doesn't want to heal anymore!!
Any help would be appreciated.

sorry to hear about your pain.
it is sometimes too much to handle.
same here dictor dont seem to interested and serious about pain.
i had the Lis, it help but diet is key.

Is your fissure healed now?
how long did you have to stay in New York for after procedure?
and what kind of reaction did you get from the ointment ?

I was only told to take stool softners during my recovery. I'm lucky that I didn't really have hard stools besides the one that caused my fissure in the first place. I took the stool softners for about 10 days post surgery.
He did not recommend I follow any special diet since I don't have bowel issues.
I'm now pain free! The surgery was the best decision.
I flew home the day after surgery which he recommend since I was still numb and felt no pain.

I was international too. It is definitely an expensive procedure. It's sad that no other doctors do it, especially with all the success it has.

I told my Anal Surgeon at the local hospital here in California and she told me that she is familiar with the Kenalog Procedure.
So she did it for me two weeks ago and I feel great.

i hope to fly over to america to have this done. how long did you have fissure for?
are you following any diet or taking stool softeners ?

I am taking Metamucil twice a day it helps with the stools but it also stimulates it.
I don’t have hard BM but more loose and to often. I use Nifedipine every day 2,3 times on a Q tip to apply inside and another numbing cream helped me a lot. Also I took CBD oil. I stop taking pain medication this week;)
I have pain for many years but I found out that I in addition to hemorrhoids have fissure about 3 years ago. Not sure how I got it.
it is 3 weeks since i got the procedure done and i feel great. I will see my doctor for a follow up very soon and will report how are things going.

Botox did not help me either.
Had the Lis and it help quite a bit.
Now following diet to keep stool soft.

oatmeal every morning with some fruits, chia seed,
soups for lunch
raw vegetable and hummus
fruits like grapes apples bananas
salad with olive oil
chicken and fish
i avoid dairy and gluten
