Penile Rash/Balanitis Not Responding to Treatment

This all began when it started to burn while I urinated. I went to the doctors because I thought it was a UTI, but the test came back negative. Got tested for STD's, the tests also came back negative.

Ok no big deal, I thought. Maybe this will go away on its own. I then started developing a rash on the tip of the penis and along the base that burned post urination and during ejaculation.

I did some research online and thought it was probably a yeast infection. Bought some Canesten and tried applying it twice a day for a week. I also took the pill (Fluconazole). This resulted in no change in the symptoms.

Went back to the doc, who then prescribed Fucidin H cream (Hydrocortisone 1% Fusidic Acid 2%). I rubbed it on twice a day for a week, no change.

Went to a Urologist who did an examination (but no further tests) and told me I could get circumcised, but apart from that offered no further solutions. Told me it could be prostatitis.

I then went to a Dermatologist, who looked at it and prescribed me Loprox anti-fungal cream with hydrocortisone (1%) told me to use it twice daily on the area for one month. I then switched to just Loprox because it appeared as though hydrocortisone was making the irritation worse. The Dermatologist told me to go see the urologist again if symptons don't improve.

I'm still applying Loprox two weeks into treatment, and the changes are minimal if any. The rash goes down at the tip and expands at the base, almost triangular in shape. There are small scales (or maybe cracks) in the skin, and the area is red, almost as if inflamed. I have no discharge from the penis. Any kind of friction (i.e. masturbation) makes the redness and burning much, much worse. I have been abstaining from that for now and only washing the area with warm water. I have thrown out all my heavily scented soaps.

I am in the process of attempting to get an appointment with another urologist in order to get a second opinion. Topical creams don't seem to be doing much, but I am going to keep putting Loprox on it for now. I feel like the Docs have been kind of brushing me off/don't want or don't know how to deal with balanitis. Nothing I am really doing is working, so I'm wondering if anyone else has any solutions.

    Glad I found this page, of course, I wish i didnt have to under such circumstances. So I had symptoms that resembled balanitis. Long story short, I'm not circumcized -- I tend to have sex with my partner without a condom, so, after one of those sessions i realized that partner wasnt as "primed" for me to enter, if you will lol ... but i proceeded with my own form of lubrication (im sure you can guess). Anyways, after all of that was done, i noticed that i pretty much had sex with minimal lubrication causing me to pretty much have sex until it was raw -- wasnt a nice feeling afterwards. A few days passed by and i started to see similar symptoms like you guys; turns out i over stretched my foreskin while penetrating raw and semi-dry causing micro tears, allowing bacteria to permeate the surface and grow. i then Took an STD test and it came back negative. Although i did test positive for ureaplasma/M.hominis which is quite common as it turns out.

    So antifungals didnt/wouldnt work. Hydrocortisone kind of helped the symptoms but not too much tbh. So i saw a doc and he gave me a week of metronidazole -- -and some topicsl antibitoics called Likacin gel which helped alot, I think, but it coudlve just been the oral antibiotics as the glans on my member looks a bit red and seems to have developed almost like a blister like thing -- so im stopping that altogether.

    i have a follow next week. as far as cleaning goes i use aveeni emolliant body wash -- super nice and no irritation.

    im going on 4-5 weeks since the onset of this whole ordeal.

    jacko05468 have you gotten an xray or mri of the penis head of the. could it be the dorsal vein?

      I haven't had an MRI yet - that's the next step for me if the pelvic floor exercises and physiotherapy doesn't work. Seems like my urologist wants me to give it a good amount of time so I'll see him in 5 months from now. The only other thing I haven't tried at this point is a long dose of antibiotics (6+ weeks).

      Antibiotics might be the only solution. I hear Ciprofloxacin at 500mg for a month with 800mg Ibuprofen is effective. Just a bit hesitant to go on a antibiotic ride with everything going on. If anyone has done this pls let us know if it is effective.

    hey man i'm so glad i have found this. i know this was 2 years ago how is it going now? im looking at your pictures and i am experiencing the exact same thing right now. looks identical. seems to hve strted put of nowhere. I have had STIntests and bloodwork.

    started several months ago. thought it was a yeast infection had red scaly skin, wirh some peeling skin. did like 3 cycles of clotrimazole cream and took fluconozole. seemed to help a bit but not really. skin looks really dry and wrinkly after i wash and dry it. have also been prescribed fucidin but have my doubts it will work. i am racking my brain trying to figure out what it is or what caused it.

    how is your situation now?

      Glad my post helped. Unfortunately I'm still experiencing symptoms 2 years on but you could say that it's not intense as it once was (some of the redness has gone down for example). However, by and large I still get burning, swelling, stinging, and redness. Still trying to find solutions. How are you doing?

      ive recently have this problem... ive been using clotrmizale cream for 3 weeks and going to ask my GP for something stronger...

      also fortuntately there is no pain or burning just the same redness however im determinded to solve it as is becomes inflammed with any irritation.

      have you been diagnosed with anything and any tips?

      @zachary25002 hey man, this sounds exactly like what I have going on. Red irritated dry wrinkly skin, sometimes peeling as well. Tried clotrimazole creme as well as hydrocortisone but that did not help. Been suffering from this for 4 years now and it's really getting frustrating.

      How's your situation? Any update?

    Okay, I just reread this entire thread and would like to add my 2 cents.

    So first of all I've suffered for 4 years from this and I don't really remember how it started or where it came from.

    My symptoms: Red irritated and dry skin on the glans, the skin also has a weird texture at the bottom (looks wrinkly with many small lines in it) There appear to be some bumps as well. All of this gets worse after masturbating sometimes even after showering. In addition my glans gets super dry and wrinkly after masturbating. There are weird orange spots on the glans sometimes as well (I don't really mind these though).

    I've been to several doctors. All of them prescribed antifungal and hydrocortisone cremes but they did not help.

    I never had sex so I'd say that STDs could be ruled out as a cause for this.

    Read a lot about the different kinds of prostatitis and I think chronic non bacterial prostatitis could be a possible cause for all of this.

    However I've been doing pelvic floor exercises for 3 weeks now without any improvement. I'll keep on doing them though, it probably takes time.

    If anyone got any new information I'd be grateful if you'd share it. It's really getting frustrating.

    Hi Jack and everybody else in this blog, like you I have been suffering from irritation and redness on the head of penis, mine started real small red patch, I started using Clotimazole thinking it was a fungal infection. the Clotimazole made worse and bigger, I had phone appointment with my doctor(because of covid) and I described the problem and he recommended Mupirocon for infection, again it made it worse and now covering more than half of the head of the penis and I started feeling a burning sensation post urination.

    I managed to get an person appointment with my doctor and after the examination he told me that is was probably a Candida infection and gave me Nystatin cream and said to call him in 5 days if it did not get any better, after a few days the cream was making it worse and now it covered about 80% of the head anf the foreskin was getting infected too. I called him back and he gave me a course of Fucozanole oral meds for 10 days, again after the treament nothing changed and it was getting worse. at this point I was getting worried about the whole ordeal. My doctor gave a referral with the dermatologist, after waiting a month to see him, he said it was probably dermatitis and gave me two creams, ketoconazol and hydrocortisone 2.5 with instructions to use them twice a day but not at the same time. after few days no change and I read that with hydrocortisone you need to apply a moisturizer 15 after the hydrocortisone. I used the Hydrocortisone and the moisturizer without the Ketocozonal and you not going to believe the results after 2 days, the redness is almost gone no burning after urination. im using wheatgerm oil for moisturizer and the 2.5 hydrocortisone, you will noticed that the cortisone will causes redness after applying it, but the applying of the moisturizer 15min later will nullified the redness and heal the lesions. im hoping this will work for others. good luck to all

      Hi all, following up one week after clearing of symptoms: The head and foreskin are back to normal I'm no applying anything now just cleaning it good after urination. The thing about this problem that many of us is having that appears to be yeast or candida infection or a regular bacterial infection, but is more of a immune system problem related to eczema or psoriasis and in us treating it like a fungal infection we're making worse. The penis gland are probably one of the most delicate skin on the body and putting the wrong medicine or the wrong amounts of it makes worse. in my case I learned to apply a very small of Hydrocortisone and rubbed it gently on the affected area follow 15 minutes later by small amount of moisturizer, after 8 hours I cleaned the penis good and apply a very thin coat of Ketozonal. I did this for five days and I saw results right away. I will caution in applying any of the medicaments under the head of penis because it will start hurting about the fourth day. I hope this treament may help others in this group. Health and Happiness my friends

      With your results, is the inflammation at the tip gone? My main symptoms are tip inflammation, mild amount of stinging during/after urinating almost every time, dribbling after I completely empty my bladder 5-10 minutes later as if i didnt empty my bladder.

    Are you taking any pills, medications, or using any creams for acne?

    My guess is that the urethral pain is not related to the skin issues, skin issues likely from repeated use of creams that are harming normal bacterial flora.
