Penis foreskin cracks during sex, peeling skin, dots and spots [documented]

Hi,

First of all thank you for your time and sorry for the long post,

I'm a healthy 29y old male and in the past 8 months I've been having some problems when having sex. Around February I've noticed some cracks on my foreskin image, at the time i didn't thought much of it, and it eventually healed up, after that, every time i had my foreskin would always crack in the same spots with the same pattern, and it started to appear even more cracks imageimage, when they happen during sex, i can clearly feel them and it stings a little, but after the act, i would clean it with lukewarm water and i would only feel an occasional sting or 2, and, along with the cracks, i also feel the area where the cracks are very very raw and sensitive to the touch, the rest of the penis is fine. In the healing process, after 1 or 2 days, my skin would feel very dry, accompanied by some peeling skin where the cracks where imageimageimage, that area of the skin, eventually they would heal up, but the skin in that area looks a bit darker than the rest image, the dryness i would fight it with some moisturizer.

In all this months I've tried some approaches to tackle this without success, i used a cleaning solution with neutral PH and no fragrances, anti candida fungal ointment, topical steroid cream, fusidic acid and also elidel, already went to 2 different dermatologists, and the problem still persists, with a down side, that now, it seems to be getting worse as some dots started to appear on the glad imageimageimageimageimage and also on the foreskin area imageimage. Along with all of the this, the redder area of the skin sometimes when is dryer it also looks shinier. I'm posting here in hope of some advice of someone who has gone through the same thing. Before all of this, i had a normal sex life with no problems whatsoever.

Best regards, Thank you,

It seems to be a fungal infection. Applying some coconut oil twice daily can give some relief. If it doesn't help, look for an antifungal cream. For me, Fugacil clears the infection within several days to a week.

Hi, have you read the full post? i've already tried several antifungal creams , and the problem is still here . even with the latest treatment it does not seem to be getting any better, since the red area still shows very clearly with no signs of getting better.

I went down this path for similar symptoms. Four urology visits with three different urologists, two primary care visits. tried about six different antifungal creams for the balanitis and candiasis diagnoses that I was given, as well as fluconazole, and betamethasone steroid cream. ultimately I went out of network to get in with a dermatologist sooner than a one year wait for an appointment and ask them to not be conservative and to help me figure out what was going on here because I had been fighting this for a year and I did not to be jerked around any longer. We took a biopsy of one of the affected spots where my skin kept cracking after any intercourse regardless of intensity. turns out it is cancerous, squamous cell carcinoma, and I will have surgery to have the spots removed in a couple weeks. If you're having these problems and an antifungal isn't getting it done, I would suggest seeing a dermatologist and getting a biopsy done sooner rather than later and advocating for yourself and a more aggressive plan to diagnose rather than a wait and see approach. Also of note, I am a trained medical professional, a registered nurse with a masters degree in nursing. I know that the idea of this is pretty intimidating from a guys perspective, but if a dermatologist is willing to remove the spots versus a urological surgeon, you are probably in pretty good shape because that means the lesions would generally be surface level, rather than invasive. Good luck guys.

Hello Blacklab. I was wondering if it was Squamous cell carcinoma (SCC) or more like Penile Intraépithélial neoplasia (the precursor to SCC)? Like Joe here in the forum.

On the long term, its almost the same thing I agree. I just want to understand a bit better what is happening.

A year seems a little quick from those symptoms to show up and fall directly in the SCC stream? But again, we are all different.

I am not putting your word in question, I just want to understand a little better, to orient the discussion with my healthcare provider.

Looks painful. Check out balanitis see if it fits your symptoms. Good luck hope you can get it resolved

If i engage in sexual activity it will crack and it stings. Aside that no pain at all. I think it was balanitis in the beginning, but i might have overlooked it for so long (almost 7 months) that now it might have evolved to something way worse and more difficult (or even impossible) to reverse. I can almost relate the beginning of some symptoms, like dryness of the skin, to a infection i had 1 and a half year ago, where i was treated with some antibiotics. At the time i noticed the infection cause i had blood mixed with urine, but eventually with the medication it went away. I'm really freaking out with this. 😦

The disease can absolutely turn into something else. From what i just read some require circumcision or a biopsy of the skin around foreskin to rule out other things . also found cream that is suppose to help. Sorry you're going through this.

Well, this is getting quite frustrating, but, 1 month in the treatment went again to the dermatologist since the condition as shown no signs of improvement, every time i start to manually stretch the skin, it immediately starts ripping on the same spots. At this point i was told by the doctor to do 1 month cycle of corticosteroid medication (Methylprednisolone), and if it shows no effect, i will need to undergo a skin biopsy. The worst part is that I've been experiencing some light hematospermia and it seems to be worsening.

Hey Buddy. Just signed up to answer you. Its the use of steroid cream that thinned your skin. Steroid creams can cause skin atrophy which means the skin gets thin and also dry. So do not use steroid creams ever again.

I don't think I used steroid cream, but i'm pretty sure I have the same problem as the guy who made the post. What do I do???

Hey man, have things improved at all? I think Im experiencing the same thing as you and I havent found a solution. Gone to 2 Urologist, 3 dermatologists, and 2 general practitioners and am still dealing with it. Tried pretty much everything you have mentioned above too. Thanks!

Hey Bro, I was also facing same issue but my condition was worse than yours. After visiting several dermatologists, skin specialist and sexologist no luck. but a home remedy works like a charm (no side effect).

      ingredient

      1. 250mg of ginger root
      2. 250mg of raw turmeric ( not powder)

      Method

Make a small ball by grating them and swallow with luke warm water. Twice a day, 3rd day you will be fine. Please don't sex for 21 days else it will reappear.

man same here, I signed up just to reply to you! i went through similar nighmare. The cause was stupid doctors diagnosing Fungi and prescribing antigungal with steroids (like betamethasone). That s**t thins out the skin and provokes cracks which get infected. time and time again. the steroid cream relieved the pain so i kept using it almost daily. for a freaking year!

I finally met an experienced urologist that spotted the issue right away prescribed antibiotics to stop infections on the cuts and now I m on the path of recovery.

I came here looking for answers into how to heal the skin faster with vitamin A etc but be aware that retinol creams may have an abrasive component cuz they are used to heal wrinkles and scars, so DO NOT try that.

Vitamin A supplements, collagen, a lot of patience and love. we will all be ok soon 😃)

if anyone else has any topical cream recommendation please advice.

if u need to read more about it here is the article that helped me understand the issue:

      https://dermnetnz.org/topics/topical-corticosteroid-withdrawal#:~:text=The%20duration%20of%20acute%20topical,return%20to%20its%20original%20condition

      good luck to you all

this is basically a great home remedy to fight infections.

I would make sure you understand the cause of the skin cracking. in his case (and mine too, it was the overuse of prescribed steroid cream to treat inflamations.

so whether we do turmeric or antibiotics, the important thing is to discontinue betamethasone component creams or similar ones
