Prostatitis with zero pain, just horrible 24/7 urge to urinate/frequency

Hello All,

I am a healthy 25 year old male with no history of urinary issues.

I am at my wit's end. Last Sunday, I started having terrible urinary frequency. I will literally go to the bathroom, urinate, flush the toilet, and then have to use the restroom again RIGHT AWAY. In other words, I have constant urinary urges 24/7... It has been this way for one week.

I don't have any pain or discomfort in my pelvic area, and there is no burning sensation during urinating. However, I feel as if the urine is at the tip of my penis - that is where the urge sensation is most prevalent. If I hold it for a long time and it accumulates, there is quite a lot of urine. However, most of the time (especially in the middle of the night) hardly anything comes out even though it feels so urgent.

I can't sleep since it feels like I have to urinate constantly... last night I was going every 5 minutes, and most of the time only a few drops came out. I am having anxiety now because of this.

I went to the urologist last Tuesday (2 days in). Urinanalysis showed no UTI and bladder ultrasound was okay. However, during the rectal exam, he said my prostate was "off" and did a test of the urine that came out after the exam. At that point, I was told that I had a prostate infection and that the enlarged/inflamed prostate was causing the frequency. In other words, there was bacteria.

I was given 500 mg of Levaquin to take once a day for 30 days. However, it has not helped after 6 doses. I'm trying Motrin (ibuprofen) to no avail. I was told it takes a while for the antibiotic to reach the prostate. I have no fever, and no chills.

I am terrified that I have the chronic pelvic pain (CPP) form of prostatitis. However, I don't know if I do, since (1) there WERE bacteria present upon running the urine after the rectal exam and (2) I have ZERO PAIN. The only symptom is the 24/7 need to urinate. Any ideas on this? I hope I have the acute prostitiis form since this is cured with more antibiotics, but if I have the lifelong CPP form I don't know what to do... I feel like I will have to quit my job since I can't sleep with the need to urinate.

Please help!

    my urologist told me that he is not prescribing antibiotics for nonbacterial. he recommends quercetin to address a possible inflammation.

    this link you provided, how do we know it is credible?

    streching wont harm anyway. can be tried at no cost. every person is different. less harmful than antibiotics for sure

      have you seen an urologist, if yes what tests have been done , any treatment you have taken?

      and tell me your symptoms, like frequent urination, burning, pelvic region ache, etc,what symptoms you have?

      thanks man im sorry this has just been freaking me out these past days and it seems that only people that i could relate to are on this forum, i dont have any painful crazy symptoms besides that ill pee and ill leave and just sit there with the same exact feeling like i need to pee and its dreadful i just wanna chill and hang out, id kill to not have this feeling anymore, i notice it to be most active when im trying to sleep or relax , excuse me being all over the place im just quite stressed about this right now im looking on here seeing that some have it to be lasting for months and even years i cant even begin to imagine.

      Explain your symptoms, and how/when it started. Give us background, do you sit for long periods each day? Are you physically active or sedentary? After having posted here for over a year, I'm free and clear from my issues, but I've been fine for a while now. Depending on how serious or severe and whether or not this is actual prostatitis and not just some weird back or pelvic issue, I'm sure you'll be fine.

      well its been going for a month now it seems, im decently active im 19 and i skateboard atleast 3 times a week for about an hour or two other than that i dont usually do any sports. I do sit at a desk playing video games for long periods of time and i dont have the best chair or posture. Ive had this had this issue in the past but its never lasted for more then an hour i always assumed i was just dehydrated but this has come to last almost a month now, at the beggining i grew very worried as you all know its extremely uncomfortable sitting trying to relax with the urge to urinate right after you just did, so i made an apointment with my doctor, they said i didnt test positive for a UTI or STDs nothing results came clean they said they found trace blood in my urine and that trace blood is not abnormal, i felt especially uneasy after this apointment due to me being sent home with still the horrible feeling i let some time pass and i tried to ignore the feeling to the best of my ability, it became mild and i feel as if the more i thought about it the more i felt it so i thought it was just me overthinking and stressing my self a d for the wrest of the month and it felt as if it was going away, the feeling became off and on through out the day but up until 2 nights ago im 100% sure something isnt right it hasnt ever felt so severe, it feels as if theres just urine trapped at the tip of my penis and during urination i most definitley find some relief but right after im done the same feeling just doesnt go away its driving me crazy, i feel it the most when im trying to relax or sleep i just want it to stop. thankyou for your time and reply.

      i had the exact same problem i took Spring Valley Cranberry extract 15,000 mg walmart has it the bets medicine that helped feel normal and to urinate normal the first 2 days everything will be gone try it only 10 bucks at walmart and let me know how u feel !!!

      bro, as far as i understand, yours is mainly urinary symptoms. i know the feeling very well. i got it when i was your age , and i was good like 20 years , then it came back, this time slightly different, but i have urinary symptoms like not emptying feeling.

      look here are your options. try these in order , so you know what works. I will list some stuff that work with some people . but I am not a doctor. everybody is different.

      • if is is bacterial or acute inflammatory antibiotics might work. you can try a short course of antibiotics, but talk it over with your doc and dont go over 1week lets say , max 10 days ( sometimes antibiotics work with some people even no bacteria is detected, use safe antibiotics with less side effects) - eat plain yogurt when you are on them
      • some people find relief from alphablockers ( like flomax), i think these people might have a muscle problem because these flexes smooth muscles and improves blood flow as far as i know) - i personally have an allergy so i cant use
      • some people get benefit from antimuscarinic drugs ( this again has to do with smooth muscles)
      • some people get benefit from a low dose Cialis ( 2.5mg , or 5mg) - relaxens smooth muscles
      • some people get benefit from quercetin
      • some people get benefit from anti-inflammatory drugs like advil or voltaren sr 75 ( go easy on these , dont stay on them more than a week)
      • some get relief from anxiety drugs like xanax or antidepressants
      • some get relief from streching , relaxing pelvic floor ( you can go see a pelvic floor therapist)
      • sitting on your ass too much is not helpful
      • hot water sitz baths with epsom salt is good
      • over masturbation triggers my sypmtoms, so try refraining like 5days and see what happens , masturbate less frequently, if it s a muscle problem, this should give your sore pelvic muscles a time to recover

      i have also started to use cranberry pills, seems to help ! mine has always come and gone, so still not sure if this is the reason, but certainly doesn't hurt taking them!

      Sorry you are going through this. It's miserable, and frankly getting a doctor to pay attention as a young guy is going to be a challenge.

      When you speak with a doctor next, be assertive. You have a problem that is interfering with your life and work. This isn't mildly inconvenient. It is a major problem you need to address.

      Ask them "how do we address this" "What is our next step" "What is causing the problem" "what is a possible cause for this problem". Don't leave again without some step forward. If they want to send you for testing, also ask them "what should I do in the meantime while we wait for results - this is unbearable".

      It is important to make them understand. I'd even suggest keeping a daily journal so you can document

      1. severity of symptoms
      2. Whether a given course of medication or treatment is working
      3. what has been tried already

      When I was younger, drinking large amounts of water - basically sitting on the toilet for a few hours drinking water - would help reset my muscles. But later in life, it required medication. There are a bunch of hoops you might have to jump through - so documenting it is important. When 5 years from now this issue pops up again, you want to be able to show the doctor what did ( and didn't ) work in the past.

      Please write back here what you find. what works and what doesnt;. It's also a good place just to comment on your symptoms. good luck

      i take that advice to heart man thank you so much for taking the time to help me out, i will most definitely start wrighting in a journal i think thats a really smart idea and im gonna start that the second i finish the message i will keep you and others updated, im more then great full to have some really good people comment and help out with this mess i will definitley try everything.

    hey everyone if you havent already saw just a few days ago i came on this forum freaking out because i was feeling these symptoms for about a month , so far ive gotten tested for a UTI and STDs both came clean i as well did an x-ray and ultra sound both cane back normal, yesterday i finally went in for a prostate exam, wasnt fun but they did notice my prostate had some imflamation, ive been perscribed some antibiotics (ciprofloxacin) so far im not sure if its just in my head but it seems like whatever it i had is starting to go away yes i do have the same urges just not as severe anymore and last night i was actually able to get a goodnight sleep for the first time this month, ill keep everyone updated on how the treatment is going, once again i cant thank everyone who came to me with all this helpful comforting information enough, its been so very helpful.

      I would urge you to be careful on cipro. A lot of people have horrible reactions to using it. I would've hope they would've tested your prostate fluid at that point for bacteria instead of feeling around and determining that throwing an Rx at it was the answer. If the cipro runs its course and the problem persists, I'm willing to wage the prostate is fine and you're looking at a sacrum/sciatic issue, or tightening of the pelvis. You said you were fairly young right? It's super unlikely you have any prostate issues.

      I echo the same concern as Ryan. Handing out antibiotics without any test showing an infection is very common when it comes to issues like this. I was given one myself, of course. I know i have read that antibiotics can sometimes have an effect where it seems like they may be working initially, but really arent. Hopefully it really is the answer for you, but i doubt it. Like Ryan said, its very unlikely this is a prostate problem. This is just the typical diagnosis from almost every urologist, because its what they were taught. The one time i went, the guy even flat out said it was a "general diagnosis based off the symptoms", or something to that affect, while handing me an antibiotic prescription without doing any testing other than urine which came back clean. If this doesnt work for you, you need to pursue other avenues. Ive mentioned his channel before (i am NOT him so i have nothing to gain), but youtube UptownMike.

      some people especially who are young, they get benefit from antibiotics' anti-inflammatory effects. they dont know if the inflamation is bacterial or non-bacterial, so they are just trying, but they could have prescribed sthng better cipro in terms of side effect profile

      i will 100% keep all of that in mind i can agree just by feeling that its not prostate related, so far my symptoms have become less severe, i dont remeber if i stated this in my last reply but ive actually been able to get a goodnight sleep. You said that people where having horrible reactions to the cipro do you mean like side affects wise or long term affects, as far as i know from the 5 days ive been taking it, i havent felt a single side affect from them.

      Hi guy im suffering from this for three months and for me is all started with google search and i get the result that it was cancer when i search up there white spot in my mouth and i have stress and depression and i have anxiety. That how i start to feel this needed to pee symptom . But i think is all in your head im trying to stay positive and it got a lot better. and i did get tested for everything and came back good. so i really think is all because of stress or anxiety. Let me know what you guy think. Or tell me how it all started for all of you.

      Well for me, I've figured out that by using cipro for an extended period and then not properly reintroducing good flora into my gut has caused me to be diagnosed with Hashimoto's Thyroiditis. For others it can range from small intestinal discomfort to permanent nerve damage. Everyone reacts differently, or not at all.
