Prostatitis with zero pain, just horrible 24/7 urge to urinate/frequency

Hello All,
I am a healthy 25 year old male with no history of urinary issues.
I am at my wit's end. Last Sunday, I started having terrible urinary frequency. I will literally go to the bathroom, urinate, flush the toilet, and then have to use the restroom again RIGHT AWAY. In other words, I have constant urinary urges 24/7... It has been this way for one week.
I don't have any pain or discomfort in my pelvic area, and there is no burning sensation during urinating. However, I feel as if the urine is at the tip of my penis - that is where the urge sensation is most prevalent. If I hold it for a long time and it accumulates, there is quite a lot of urine. However, most of the time (especially in the middle of the night) hardly anything comes out even though it feels so urgent.
I can't sleep since it feels like I have to urinate constantly... last night I was going every 5 minutes, and most of the time only a few drops came out. I am having anxiety now because of this.
I went to the urologist last Tuesday (2 days in). Urinanalysis showed no UTI and bladder ultrasound was okay. However, during the rectal exam, he said my prostate was "off" and did a test of the urine that came out after the exam. At that point, I was told that I had a prostate infection and that the enlarged/inflamed prostate was causing the frequency. In other words, there was bacteria.
I was given 500 mg of Levaquin to take once a day for 30 days. However, it has not helped after 6 doses. I'm trying Motrin (ibuprofen) to no avail. I was told it takes a while for the antibiotic to reach the prostate. I have no fever, and no chills.
I am terrified that I have the chronic pelvic pain (CPP) form of prostatitis. However, I don't know if I do, since (1) there WERE bacteria present upon running the urine after the rectal exam and (2) I have ZERO PAIN. The only symptom is the 24/7 need to urinate. Any ideas on this? I hope I have the acute prostitiis form since this is cured with more antibiotics, but if I have the lifelong CPP form I don't know what to do... I feel like I will have to quit my job since I can't sleep with the need to urinate.
Please help!

Hi guy im suffering from this for three months and for me is all started with google search and i get the result that it was cancer when i search up there white spot in my mouth and i have stress and depression and i have anxiety. That how i start to feel this needed to pee symptom . But i think is all in your head im trying to stay positive and it got a lot better. and i did get tested for everything and came back good. so i really think is all because of stress or anxiety. Let me know what you guy think. Or tell me how it all started for all of you.

Well it's not cancer, that's for sure. Whatever else is going on is separate from the issues you're experiencing. It's not abnormal for anxiety to cause a heightened sense of awareness so now you're more dialed into the desire or urge to urinate especially now that you're thinking about it. Could also be that anxiety caused tightening in your pelvic muscles and you pinched a nerve and this is the reaction. I would try to move more, get some walking or light jogging in. See if that helps. Have you had any tests done? Might be related to a UTI or something as well.

i got hiv test done uti and other std but all come back normal burni only feel the tip of ky penis now but if i dont think about it. i domt feel it much but still there but it has gotten lot better

just found this thread and wandering if you ever got it resolved. i have been having similar symptoms for almost a year without being able to get help from anywhere. all tests normal, healthy otherwise, 41 years old, exercise daily, mountain bike, hike, trail run, lift weights. same sensation at the tip of penis after i pee. sometimes if i dont pee for 3 or so hours it will go away, but is back right after i pee again. also some dribble after peeing. the sensation in the penis goes away with erection, when erection is gone the sensation returns back. i did bactrim that didnt work, flomax doesnt do anything, also tried oxybutinin that did nothing. not sure where else to look. seems like riding my bike makes it go away, but maybe i am just more concentrating on the ride and dont think about it. anyways, basically open to any ideas, a year is way too long and it has been rough.

I don't know that there's a definitive solution to this problem, but what worked for me was time... a few months of lighter lifting/workouts, destressing, more cardio or movement in general eventually solved my issue. After a year, things were pretty much back to normal. You can try this for yourself, but I would advise a serious case to be seen by a urologist, and if nothing is present as far as bacteria in the urine, etc, then visit a physical therapist for pelvic disorders.

I did alot of test all kind if sexual test all came back negative went to see urology and he told me there nothing wrong because they tested my bladder and my kidney and all my pee test was normal but I did have anxiety and depression. just like today when my anxiety came back I noted the symptoms becaome worse . and if i do something to distract my self I dont feel it but the thing is my mind is thinking about it every day that why it never goes aways so I'm thinking is my anxiety what do you think?

Hi there,
Since end of October I've had issues with frequent urination. A feeling like there is always pee at the tip of the penis needing to get out. Constant pressure on bladder when walking and stuff too, only time its not that bad is when im laying down. However, since it started i have not had to wake up in the middle of the night to urinate at all. Its daytime frequency.
It has made me super sad because I exercise and play sports such as hockey and soccer regularly and this issue I can barely play. Obviously my mind is constantly on this.
It was bad in October/November, saw my doctor who told me to wait and see if it continues till after christmas. It actually wasnt too bad over december/january as much but got worse at the beginning of February again, so I saw the doctor again and he has me taking a natural vitamin daily called Saw Palmetto.
Ive been stressing, and its making me depressed. I dont know what to do now

have your prostate checked ( ultrasound and manual doc check) - there might be a size issue
have your bladder checked
get tested for stds
if it all comes out normal, try hot siztz baths, ibuprofen, cialis 2.5 mg, quercetin, and maybe an antidepressant ( not all at the same time) !
also try streching and realising your pelvic region

ive had several urine tests, ultra sound and my bladder empties completely, everything is clear std and everything.
Does the ibuprofen work? im only 21

Sounds like an overly tightened pelvic muscle sling, or a sacrum/sciatic injury. I would recommend getting to a chiropractor for an adjustment at least since you do play aggressive sports. These sorts of injuries aren't uncommon to say the least, but they fly under the radar often since they come and go and people normally blame them on internal organs. I assume you're young, in which case your prostate is probably fine. Bladder is also likely fine too.

like you probably have already noticed, there are several threads with people having this tip of the penis issue, me one of them. the problem is, none of the threads contain any success story, seem like people dont post once they get better i guess.

Not true for me. My posts are above, and I've posted after the fact. My first instance with these symptoms occurred after a pelvic surgery which required an incision in my inguinal area. Shortly after that, I recall complaining to my urologist that I had "prostatitis" type issues. Urine frequency, post-void dripping, urge to go, etc. Eventually it cleared up until I recently started working out my lower body, which called for heavier weights and a lot more pressure and stress in the pelvic floor and sacrum. My symptoms came back and I came here to report on it. A year after, I came back to inform the thread that my symptoms were gone and have stayed gone. Keep in mind though that every one of us might be suffering from different issues, but it signals the same nerves and response. Not everyone has interstitial cystitis, prostatitis, or a pelvic muscle issue, or lower spine compression, but it's likely to cause similar symptoms.

Yup. I started doing late night walks and jogs to exercise the pelvic sling a bit more, and loosen up tension down there. Eventually within a few weeks or so, the sensation dwindled or disappeared entirely. I'd have to scroll back up and see the actual time progression, but sacrum and sciatic injuries will heal over time.

thanks ryan, i must have missed your posts. for how long have you had your symptoms before they improved. i am about ready to ask doc for pelvic floor therapy as nothing else seem to help. i wonder if all the mountain biking i did ( and still do) messed up my pelvic muscles balance.

I would definitely look into a pelvic floor therapist if you have the means to. Youll probably have to research it on your own though as I doubt a urologist or regular doctor will even consider this as its not their area. Thats why so many people never get better because in the end, they are just told they have "chronic prostatitis" and never realize its possibly a muscle issue.
I have experienced some of the worst symptoms a guy could have, but it has definitely improved some since i started doing stretching. I no longer feel completely helpless. Since i was interested in getting in a better shape and was stiff everywhere, I got into DDPyoga. I also do a few pelvic specific stretches like deep squat holds. Thats something you could just start trying right now. Squat all the way down (i mean ALL the way down, this isnt a quad exercise) with your feet flat . You can point your feet out a bit and spread your knees wide if you want. Do it with your back against a wall or couch if you fall backwards. Try to totally relax with deep breaths through your belly. What i like to do is first do it with my upper body more leaned forward, so my triceps are between my legs. I do the deep breaths, and for lack of better terms, kind of send the air down through my pelvis and breathe into my balls. I know that sounds ridiculous lol, but its basically a reverse kegel. After a couple minutes, I shift my upper body to being alot more upright. I find in this position, its easier for me to breathe through and relax the back of the pelvic floor, which is basically your pooping muscles. Stay squated there for few minutes well.
I have no idea if this would help you at all, but its worth a shot. If you feel any relief at all, definitely look into more stretches. Your best bet though as i said initially, is to seek out a pelvic floor therapist. I never did because i dont have insurance for that, but theyll be able to evaluate to see if its a muscle problem. At least youll know one way or the other. If youve gone through the run around of tests from a urlogist, and still have no answer, its unlikely to ever come and you need look somewhere else.

really appreciate ryan. i spoke with uro about pelvic floor therapy a while back and he was open to it, actually sounded like something he was familiar with ad said i could try it. i then traveled and never got into it and now trying alfuzosin which seems to help, sort of, but i would hate to take pills daily, especially when i am just in my 41.
thanks for the tip on the squats, i am going to start doing it.

helphelp, like i mentioned above, the worst i get is right after peeing, getting this wet sensation in my penis. while i dont get worst by actually having sex, i do ejaculate and that triggers the same feeling as after peeing. basically, everytime something pases through my urethra i get the feeling. i might try to ask uro for cialis. any known negative side effects? so far, with alfuzosin i dont have any.

the wet feeling is the worst! sometimes i feel like i have peed my pants, but when i check there is nothing therei have gone to the urologist and they do not seem to have answers, they want to scope my penis, but other people have had that done and it sis not solve anything, so i'm going to pass on that!

the 2.5mg cialis is the minimum dose, you should be fine dont mix drugs tough try them once you are done with alfuzsosin
