Prostatitis with zero pain, just horrible 24/7 urge to urinate/frequency

Hello All,
I am a healthy 25 year old male with no history of urinary issues.
I am at my wit's end. Last Sunday, I started having terrible urinary frequency. I will literally go to the bathroom, urinate, flush the toilet, and then have to use the restroom again RIGHT AWAY. In other words, I have constant urinary urges 24/7... It has been this way for one week.
I don't have any pain or discomfort in my pelvic area, and there is no burning sensation during urinating. However, I feel as if the urine is at the tip of my penis - that is where the urge sensation is most prevalent. If I hold it for a long time and it accumulates, there is quite a lot of urine. However, most of the time (especially in the middle of the night) hardly anything comes out even though it feels so urgent.
I can't sleep since it feels like I have to urinate constantly... last night I was going every 5 minutes, and most of the time only a few drops came out. I am having anxiety now because of this.
I went to the urologist last Tuesday (2 days in). Urinanalysis showed no UTI and bladder ultrasound was okay. However, during the rectal exam, he said my prostate was "off" and did a test of the urine that came out after the exam. At that point, I was told that I had a prostate infection and that the enlarged/inflamed prostate was causing the frequency. In other words, there was bacteria.
I was given 500 mg of Levaquin to take once a day for 30 days. However, it has not helped after 6 doses. I'm trying Motrin (ibuprofen) to no avail. I was told it takes a while for the antibiotic to reach the prostate. I have no fever, and no chills.
I am terrified that I have the chronic pelvic pain (CPP) form of prostatitis. However, I don't know if I do, since (1) there WERE bacteria present upon running the urine after the rectal exam and (2) I have ZERO PAIN. The only symptom is the 24/7 need to urinate. Any ideas on this? I hope I have the acute prostitiis form since this is cured with more antibiotics, but if I have the lifelong CPP form I don't know what to do... I feel like I will have to quit my job since I can't sleep with the need to urinate.
Please help!

i tried AZO, and it didnt work, but apparently there are different kinds of azo. the one i tried were pills that would turn my urine orange.

from all the things so far, the only think that sort of works for me is alfuzosin. id does not completely clear it but it does reduce the sensation in my penis. i also dont have any side effects from alfuzosin.

Everyone is different from what I gather. Some have prostate or urethra/ureter issues, and some have deferred nerve pain, or pelvic inflammation. You need to analyze your life style, any changes prior to this. Stress level, body posture, injuries, diet, etc. It could be an array of things, or one simple thing. There is no cure-all until you determine the drop off point where your symptoms began. For me it was working out my lower body incorrectly, which caused a sciatic flare up near my sacrum. Couple months of light stretching and long walks, time away from the sitting and laying position and it lessened to almost nil.

You may also want to check into a urologist with your symptoms, and if that proves fruitless, see a chiro or PT specialist to check into your spin posture, and pelvis.

Hi all,
I eliminated my issues many months ago and have been symptom free. I cant say for sure what helped, but it was a lifestyle change for me that i think helped. Better diet, exercise, daily stretching, meditation, and reducing stress. I really think my issues were caused by stress and inflammation from that stress. I hope everyone finds their own peace with this struggle.

Thanks for the message. How long did it take from the time it started and when it stopped and what were your symptoms?
Mine has been going on since end of October. It could be stress but not sure. Exercising is more annoying now since i have that sensation feeling all the time.

Hi all, Firstly this is something that cannot be explained unless you have experienced it hence why lots of people cant seem to get answers from doctors. My personal experiences with this have been going nearly 2 years, had all sorts of intrusive tests and meds, yet no success until i met Doctor im
seeing now who has pinpointed stress an anxiety and put me on medication and its been 8 weeks since starting and i am feeling much better, long way to go but fingers crossed getting there, hope everyone else finds a solution too. peace

Hi Craig,
I have been thinking that this may be a stress/anxiety problem but im honestly not sure. I have a urologist appointment in a few weeks but thats most likely getting cancelled/pushed to a virtual appointment.
What kind of medication have you been put on and has it taken away the constant sensation? Thanks!

Hi mate, im now on stuff called lexam, low dose for anxiety, my symptoms went from feeling like needing a pee all the time to feeling like i needed to poo all the time. i too saw urologist who said straight away i had prostatitis even tho no infections and prostate still of normal size. I have honeslty no ideas what caused this initially but i do believe better diet more exercise and less alcohol is a good way to start recovery from this, also mental health with the stress of it all makes it worse so maybe seeing a professional in that field too might help

it took a few months for my symptoms to subside once i reduced stress. I did mild exercise, walking and stuff like that. I went on a site and thats where i realized that for me anxiety and stress was the likely culprit. Its amazing what stress can do to your body and there are 100s of different annoying symptoms.

I guess my issue is how can you reduce stress? Im in school and obviously get stressed at silly things like watching fav sports teams and stuff. lol
