That doesn't look good I'm afraid, and I'm glad you're seeing a doctor. It's not just the sore on the right hand side that look unusual, but also the colour and texture further round. When was the last time a doctor saw your penis, and did they comment then?
Could be lichen sclerosus, but it's not clear. If it is, then these links may help:

https://patient.info/doctor/lichen-sclerosus-pro (written for doctors)
There is also a lichen sclerosus forum, but it's mostly women that post there.
Good luck, and please let us know what the doctor says. I suspect you'll get referred to a urologist.

Thanks for reply.
Saw doctor in 2015. Yes he said it was segma.
gave me a mild steroid but didn't do much.
Sore only came two weeks ago.
i think that was caused my heavy masturbation.
will update you later re what doc said.

Did your penis look similar back then? That doesn't look like just smegma.

Yes but didn't have the sore.
I did have a heavy masturbationa couple of weeks ago.
the sore appeared shortly after that.
Also used a White cream last night
The head is not as white as it looks on photos,

It's not really the colour of the glans that worries me - it does look pale, but that could just be the lighting. Your foreskin looks an unusual colour and texture though, and it's not clear if it retracts normally, so I'd have recommended you see a doctor if you didn't already have an appointment.
If you could post pics from the front and back with the foreskin retracted as much as possible, that might help.
What was the white cream you used?

Saw doctor. Very good, was in there for 30 minutes.
as soon as he looked at it he said "infection."
he he said it is Balanitis x erotica.
gave me a steroid ointment. Take for 3 weeks.
if not gone by then, refer me to Erolicgy at hospital.
might need to be .circumcised?.
asked him him about Cancer. He said I don't
have enough synptoms for that.
I'm not passing water with any difficulty.

Thanks for the update. Balanitis xerotica obliterans (BXO) is another name for lichen sclerosus, so those links I posted earlier may help.
Sadly, circumcision is sometimes necessary, but good luck with the steroid cream, and I hope things get back to normal soon.
You meant you might be referred to a "Urology" department?

full and
sometime just cut slits at top of foreskin.

There are different techniques for circumcising and variations in how much tissue is removed, but the slit operation would be preputioplasty or a dorsal slit rather than circumcision. You'd need to talk to a urologist, but I think those first two are more for phimosis than what you have. If you do need surgery, I think they'd probably have to remove at least some tissue.
Did you see the same doctor in 2015 btw, and did your penis then look similar to the way it is now?

Women get lichen sclerosus more than men, so many of the hits you get on Google will be for the female version. Those links I posted earlier are very good, or If you search for BXO or "balanitis xerotica obliterans", you'll only get male forms of lichen sclerosus.
There is a lichen sclerosus forum here btw, but it's mostly women that post:

Thanks for surprising how many women are on there. As you said.
they seem to get worse symptoms and numerous problems with treatment re their 'lady parts'
