Taking amitriptyline and ropinirole together can increase the risk of side effects such as extreme sleepiness, dizziness, and confusion. It may also increase the risk of 'serotonin syndrome', a rare but serious condition, and can lead to a sudden drop in blood pressure when standing up.
Why this happens
Both medications affect chemicals in the brain and have 'sedating' effects that add together. Amitriptyline can also interfere with how the liver processes ropinirole, potentially leading to higher levels of the drug in your system. Additionally, both drugs can lower blood pressure, increasing the risk of falls.
What you should do
Speak to your doctor before taking these together. If your doctor prescribes both, be very careful when performing tasks that require alertness, such as driving. Move slowly when standing up from a sitting or lying position. Seek medical help immediately if you experience high fever, muscle stiffness, or agitation.

Suicidal thoughts and mood changes
Amitriptyline can increase the risk of suicidal thoughts or self-harm, particularly when first starting treatment or changing doses. Seek immediate medical help if your mood changes or you feel depressed.
Impulse control disorders
Ropinirole can cause unusual urges, such as excessive gambling, binge eating, or increased sexual desire. If you or your family notice these behaviours, contact your doctor immediately.
Sudden onset of sleep - ropinirole
You may suddenly fall asleep without warning or feeling tired beforehand. Do not drive or operate machinery if you experience this, and inform your doctor.
Heart rhythm disorders
Amitriptyline can affect your heart rhythm. It must be used with extreme caution if you have a history of heart disease or a recent heart attack.
Food and drink warnings
Alcohol
Drinking alcohol while taking these medicines can make you feel very sleepy and unsteady on your feet. It can also increase the risk of side effects like dizziness and blurred vision. It is best to avoid alcohol, especially when you first start treatment or if your dose is increased.
Caffeine (coffee, tea, energy drinks)
Ropinirole can increase the levels of caffeine in your blood, which may lead to feeling jittery, anxious, or having a racing heart. Conversely, if you suddenly stop or significantly reduce your caffeine intake, it could change how ropinirole works in your body. Try to keep your caffeine intake consistent.
