Can I take Citalopram and Simvastatin together?

Why this happens

Both drugs are processed by the liver. Citalopram can mildly interfere with the enzymes (specifically CYP3A4 and CYP2C19) that break down simvastatin, leading to a small rise in simvastatin levels.

What you should do

You can usually take these together, but you should be aware of the signs of 'statin' side effects. If you experience unexplained muscle pain, tenderness, or weakness, contact your doctor. Your doctor may choose to monitor you more closely or adjust your simvastatin dose if you are on a high dose (such as 80mg).

Important precautions

Depression and anxiety disorders

You may experience an increase in suicidal thoughts or the urge to self-harm when first starting this medicine or changing the dose. This is more common in young adults under 25. Contact your doctor or go to hospital immediately if this happens.

Heart rhythm disorders (Long QT syndrome)

Citalopram can affect your heart rhythm. You must not take it if you have a pre-existing heart rhythm problem or are taking other medicines that affect the heart's electrical activity.

Bleeding disorders

Citalopram increases your risk of bleeding, particularly in the stomach. Use with caution if you take blood-thinning medicines (like warfarin) or anti-inflammatory painkillers (like ibuprofen or aspirin).

Low sodium levels (Hyponatraemia)

Citalopram can cause a serious drop in blood sodium levels, especially in the elderly. Seek medical help if you experience extreme tiredness, confusion, or muscle twitching.

Pregnancy

You must not take simvastatin if you are pregnant or planning to become pregnant, as it may harm the developing baby. Use effective contraception while taking this medicine.

Muscle pain or weakness - simvastatin

Contact your doctor immediately if you experience unexplained muscle pain, tenderness, or weakness. In rare cases, this can lead to serious kidney problems.

Food and drink warnings

Alcohol

It is best to avoid or limit alcohol. For citalopram, alcohol can increase drowsiness and affect your concentration. For simvastatin, regular heavy drinking increases the risk of liver problems and muscle side effects.

Grapefruit juice

Avoid drinking grapefruit juice or eating grapefruit. It can significantly increase the level of simvastatin in your blood, which increases the risk of serious side effects like muscle damage.

Caffeine (Coffee, Tea, Energy Drinks)

Citalopram can sometimes cause feelings of restlessness or jitteriness, especially when you first start taking it. Caffeine is a stimulant that can worsen these side effects and may increase feelings of anxiety. It is advisable to monitor your caffeine intake.
