Taking these two medicines together may slightly increase the level of simvastatin in your blood. This can increase the risk of side effects, particularly muscle pain or weakness.
Why this happens
Both medicines are processed by the same enzyme in the liver (CYP34A). Because they are 'competing' for the same enzyme, the body may break down simvastatin more slowly than usual.
What you should do
You can usually take these together, but you should be vigilant. Report any unexplained muscle pain, tenderness, or weakness to your doctor immediately. Your doctor may choose to monitor your liver function or adjust your dose if you experience side effects.
Important precautions
Suicidal thoughts and self-harm
Antidepressants like venlafaxine can increase the risk of suicidal thoughts or self-harm, particularly when first starting treatment or changing doses. Monitor your mood closely and seek urgent medical help if you experience any distressing thoughts.
Pregnancy
Do not take simvastatin if you are pregnant, planning to become pregnant, or breastfeeding, as it may harm the developing baby. Use effective contraception while taking this medicine.
Muscle problems (Myopathy/Rhabdomyolysis)
Contact your doctor immediately if you experience unexplained muscle pain, tenderness, or weakness. This risk is significantly increased if you drink large amounts of grapefruit juice or take certain other medicines.
Serotonin syndrome
Venlafaxine increases serotonin levels. Taking it with other medicines that affect serotonin can cause a life-threatening condition called serotonin syndrome. Seek immediate help if you experience high fever, agitation, or muscle twitching.
Bleeding disorders
Venlafaxine can increase your risk of bleeding, such as bruising easily or nosebleeds. This risk is higher if you also take blood-thinning medicines (like warfarin) or NSAID painkillers (like ibuprofen or aspirin).
Food and drink warnings
Alcohol
It is best to avoid or strictly limit alcohol while taking these medicines. For simvastatin, regular heavy drinking increases the risk of liver damage. For venlafaxine, alcohol can worsen side effects like drowsiness, dizziness, and impaired coordination.
Grapefruit and Grapefruit Juice
You must avoid eating grapefruit or drinking grapefruit juice while taking simvastatin. Grapefruit contains compounds that increase the level of simvastatin in your blood, which significantly increases your risk of serious side effects, such as muscle damage (rhabdomyolysis).
Caffeine (Coffee, tea, energy drinks)
Venlafaxine can increase your blood pressure and heart rate. Consuming high amounts of caffeine may worsen these effects or increase feelings of anxiety and restlessness.
